Inspection• It is commonly accepted that reprocessed devices should be inspected to

show that they clean and safe for reuse.

• But in practice does this mean only the outside of the scope?

• Is it practical to insect the inside of the scope on a routine basis?

• Or is sampling the scope – e.g. testing the output the answer?

Packaging• Endoscopy packaging can be divided up into two groups:

– Packaging for sterilization and maintaining a sterile state– Packaging when used as an alternative to post disinfection

storage

• Sterilization packaging - well defined standards (EN 868 Series and EN ISO 11607 Series)– In most cases the method used needs to be compatible with low

temperature systems (most commonly not cellulose based for H2O2 methods)

• Post disinfection systems - no standards!– Is it portable storage or packaging?– Lack of long-term published evidence in this area– Check the new Standards and Recommended Practices

Standards and Recommended Practices: Portable storage systems

• Should not reduce standards, but maintain patient safety.

• Manufacturers of such systems need to provide all relevant type-test data to verify and validate process effectiveness and to identify key variables that need maintaining to ensure continued operation in line with type testing.

• Where endoscopes need to be processed in a controlled environment storage cabinet first, what is the actual validated drying time required for each type of endoscope?

• Is the storage period based upon validated data and will it require retesting if a new type of endoscope is added to the process.

• Periodic maintenance and validation is required to verify performance.

• The manufacturer should provide a table and schedule of tests and identify maintenance tasks.

• Storage times of endoscopes in such systems must be verified and checked at least annually.

• Consideration should be given to keeping the validated storage time to a minimum acceptable period which meets the needs of the user.

Is it a storage cabinet or a drying cabinet?

• EN 16442: – Storage cabinet:

• equipment controlled by an automatic control system that maintains the microbiological quality of processed thermolabile endoscope

• Can be single or double ended (pass through)

– Drying:• additional feature of a storage cabinet carried out in

the sequence as regulated by the automatic controller to remove moisture

• if the storage cabinet does not have a drying phase, it has to be specified that the endoscope (outside surfaces and internal channels) has to be dried before storage

“Many outbreaks of HCAI after endoscopy and cross contamination due to Ps. aeruginosa, S. marcescens, M. tuberculosis and M. chelonae have been associated with drying of flexible endoscopes without ethanol flushing or lack of a drying procedure”

“Endoscope drying is as important to the prevention of nosocomial

infection as cleaning and high level disinfection.

Whereas wet or inadequately dried endoscopes pose an increased risk of

contamination and have been associated with transmission of water borne

micro-organisms and nosocomial infection, thoroughly dried (and properly

cleaned and disinfected) endoscopes have not been linked to nosocomial

infection”.

UK Guidelines (2017)(BSG)

• The current guidance is that endoscopes need to be reprocessed before reuse if they have been stored for >3 hours because of bacterial growth in damp lumens.

• However, if scopes are stored in cabinets with a constant flow of clean air through all lumens to keep them dry, the time before reprocessing can be extended to 72 hours to 31 days ( period must be validated).

• The use of an alcohol flush to aid drying is not recommended in the UK due to the fixative properties of alcohol.

• When transporting endoscopes to and from areas outside the endoscopy unit, they must be transferred in in a rigid container with a solid lid.

French Guidelines

• Disinfect endoscopes stored for more than 12 hours.

• Full decontamination procedure (as opposed to a shortened cycle) if more than one week has elapsed since last being processed.

• If endoscopes are not dried, they should be used within 4 hours.

• If dried within a cabinet, they can be stored for one month.

• Therefore, some bacteria are permissible and many endoscopy units carry out a risk assessment if the count is higher

I used two hours before I put it in the drying cabinet – so can I have an hour

afterwards?

Drying may not affect all types of microorganism in the same way:

– Gram positive bacteria – may survive drying anyway

– Gram negative bacteria – may thrive in the presence of even a little moisture

Drying is not a disinfection process!

Conclusions• Even basic visual inspection of endoscopes is essential to maintaining

a safe process• If sterilizing, use packaging that meets the EN and ISO standards but

make sure it is compatible with the process• Given the lack of standards, ask you post-disinfection storage system

supplier to provide you with evidence of its performance over the time period required and for the endoscope types you intend to process

• Use Storage/Drying cabinets according to the manufacturers instructions, taking note of the drying time

• Plan which endoscopes you wish to place in the cabinet in advance –the default position should be to dry if in doubt

• Cabinets and storage/drying systems should be validated regularly and………
