Definizione di mobilità del personale sanitario?
-Non solo trasferimento all’estero per lavorare in
strutture sanitarie per un lungo periodo
-Ma anche: mobilità per brevi periodi
- Mobilità trans-frontaliera (“Pendolarismo”)
- Lavoro all’estero in un Paese di transito per
altre destinazioni( es. UK----- USA)
- Week-end lungo in un altro Stato
- Accordi Bilaterali tra ospedali di Stati diversi
- Periodi all’estero limitati dalla durata dei
permessi di soggiorno(e.g. Badanti) o a richio
di durata per mancanza dei permessi di
soggiorno
Libera Circolazione dei Lavoratori nella UE
• La Libera Circolazione è un diritto civile per l’UE e i
Paesi dell’EFTA ( Norvegia, Islanda, Liechtenstein) e
Svizzera
• La libera circolazione dei lavoratori è uno dei “4
Diritti” fondamentali dei cittadini appartenenti alla UE
• Apposite Direttive UE definiscono il Riconoscimento
reciproco dei titoli di studio

FATTORI CHE HANNO INFLUENZANO LA MOBILITA’
DEL PERSONALE SANITARIO,VERSO L’ITALIA
• Ragioni : Povertà e instabilità politica nei Paesi di
origine, alto tasso disoccupazione nel Paese di
origine.
Ostacoli all’ingresso in Italia:
• precarietà contrattuale,
• barriere linguistiche,
• lentezza e complessità delle procedure
riconoscimento laurea/diploma, (fino a 14 documenti
da presentare al Ministero),
• difficoltà generalizzata di assunzioni nella P.A.(SSN)
• Leggi sull’immigrazione per i non UE.

• 4 medici per 1000 abitanti in Italia.
• 3 medici per 1000 abitanti in Europa.
• Stima medici stranieri: 1-4%.
• N.B.Causa esubero o maldistribuzione è difficile per medici
stranieri formati in Italia trovare lavoro stabile in Italia
nonostante vi sia carenza di specialisti in radiologia, anestesia e
rianimazione, pediatria.
• Infermieri ( post riforma ): mancanza di 70.000 unità
• Difficoltà di reperire professionalità italiane e
straniere laureate (infermieri/OSS).
• Nascita della nuova professione denominata
«Badanti» prevalentemente straniere……

BADANTI: DATI raccolti per studio PROMETHeUS
• In Italia la richiesta è 3 volte maggiore rispetto alla
richiesta di infermieri
• Stima numero badanti nel 2008 in Italia: 500.000
• Survey INPS 2007: 66 diverse nazionalità (di cui 29%
da NIS e 31% da Extra UE)
• Dati INPS 2009 dimostrano che 250.000 badanti
sono assunte come collaboratrici domestiche
• Survey in Veneto ha dimostrato che il 5% degli
anziani sopra i 65 anni nel 2005 erano assistiti da
una badante

( DATI PRE CRISI ECONOMICA)
• Destinazioni principali medici:
• UK (1692 tra il 2003-2008)
• Germania (755 al 2008)
• Francia (566 al 2007) (dati di registrazione negli ordini nazionali).
• Motivi della scelta: mancanza concorsi inerenti la
specialità ottenuta, scarse opportunità di carriera,
condizioni economiche contrattuali non soddisfacenti
e richiesta/offerta aumento nei Paesi di Destinazione
• Specialità maggiormente richieste in UK: psichiatri,
medici di famiglia e pediatri.
• «fuga dei cervelli» per la ricerca biomedica negli
USA e UK.

ALCUNE CONCLUSIONI • La presenza di personale sanitario straniero in Italia
riguarda principalmente le professioni infermieristiche.
• Il fenomeno badanti riguarda quasi esclusivamente
stranieri.
• Il fenomeno della «brain drain» per l’Italia riguarda
principalmente la ricerca biomedica (fuga dei cervelli).
• Il fenomeno della mobilità degli operatori sanitari deve
essere maggiormente indagato per programmare i
fabbisogni di personale sanitario in Italia, nella UE e
extra UE.
• La crisi economica globale incrementerà i flussi di
mobilità degli operatori sanitari nei prossimi anni.
