Implementation:

As part of the LHD Stabilization plan Dr. Davis instructed the Nurse Executive Committee (NEC) to review the PHPR revision process as well as the content of the PHPR.

After many hours of review and discussion with Division staff, the NEC proposed the new Core Clinical Service Guide. This guide is based on the Core Public Health Services and contains clinical protocol and case management for local health department nurses.

Creation of the CCSG:

Signature Page:

Your LHD medical director will sign the Introduction section. Only this page needs to be signed as all other signature lines have been removed.

This file provides you with information from program staff regarding changes in clinical guidance.

Crosswalk:

This file provides you with a crosswalk of the PHPR and the new CCSG.

This file should be used if you have difficulty finding something in the CCSG that you used in the PHPR.

As a reminder, the 2011 PHPR is also still online for this transition year.

This link allows you to submit any concerns/issues you may find while using the CCSG.

The link takes you to a survey monkey tool that will ask you specific information.

The goal of this is not to evaluate the new CCSG but for you to quickly capture a comment that you might not remember during the full evaluation.

Phase II:Conduct a survey of LHD nurses to evaluate and

identify any gaps in the new CCSG (February 2013).

Evaluate the review process of the CCSG and design a new process using various QI tools.

Develop a standard matrix for clinical protocols and recommend each program clearly identify required vs. recommended components.

Suggest and implement a full review of all forms and remove any that are no longer in use.
