You can �nd the below forms in HPOS in 3 easy steps and thendigitally complete and submit the forms to Centrelink.
Log on to HPOS using your PRODA account.
From the HPOS home screen select � � �� � � � � � � � , then � � � � � � � � � � �� � � � � from the � � �� � � � � � � � �screen.
From the Centrelink screen choose the appropriate form.
