joining this important movement to end

COVID-19, one shot at a time.

Important things to know about your appointment:

• The vaccine is free, but if you have an insurance card for yourself or your child, please bring it.

• Adults getting the vaccine will need to show a photo ID. Children getting the vaccine do not need proof of age.

• Arrive on time, and no more than 5 minutes early.

• Stay 6 feet apart from others to maintain social distancing.

• Adults and children must wear a mask at all times. If you do not have a mask, one will be provided to you.

• Everyone getting the vaccine should wear a shirt that allows easy access to the upper arms.

• Plan to stay at the site for 15 to 30 minutes after you or your child gets the vaccine.

• A parent or guardian must stay with children throughout the vaccination process.

• Drop-off is permitted, but please do not wait in a vehicle in the drop-off area or fire lane.
