ATTENDEES & GUESTS PARKING - Free Parking is available in front of the Mirro Center. Please pay attention to lot markings.

NAME BADGES - Each individual registered for the conference will be given a name badge. Please wear your name badge during the conference. This is your identification to others and a necessary part of the attendance verification process. Sharing of name badges is not allowed.

GUESTS - Guest badges are available for a spouse or companion who wishes to join you during some or all of the conference. Names must be provided for all guest badge requests. Registered technologists, radiologic science students or commercial representatives may NOT register as a guest.

EXHIBIT HALL The Exhibit Hall will be open on Thursday and Friday.

View the displays competing in the Scientific Exhibit Competition as well as the information booths sponsored by the ISRT and the ASRT. Commercial representatives are always anxious to meet new contacts and explain their services. Please take some time to check out these booths and express your appreciation for their participation. You won’t want to miss this exciting aspect of the conference.

REGISTRATION REGISTER EARLY - Avoid the last minute rush and late

fees by registering now.

REGISTRATION OPTIONS ARE AVAILABLE - You can register online and pay by credit card or complete and mail the conference registration form included in this booklet and a check for the fees to the registration chairman.

If registration fee is being paid by an institution (employer or school), the registrant is encouraged to send a copy of the registration form to the registration chairman noting the name of the institution paying the fee.

DEADLINES FOR REGISTRATION - In order to assist the planning committee in the final preparation, a conference registration deadline has been established.

FEES - Conference fees are payable at time of registration and are based on membership status at the time of registration, registrant category, number of days attending, and additional options which are available.

LATE REGISTRATION FEES - A $25 LATE FEE will be assessed to all registrations made after September 15, 2017.

REGISTRATION ONSITE - Registration will be available at the conference. If you plan to register onsite, please arrive at least 30 minutes prior to the start of the first lecture to allow time to complete and process your registration. The registration desk will be open during the following times:

INFORMATION TO HELP YOU PLAN Please read the information below completely before registering for the conference.

BREAKS Breaks are scheduled between each speaker. Please see the program schedule for details about length of breaks and scheduled times.

A variety of beverages will be provided at the Mirro Center

MEALS Breakfast is complimentary if you are staying at the Hampton Inn & Suites.

A Light Continental Breakfast will be provided at the Mirro Center Thursday and Friday.

Lunch and dinner during the conference is “on your own”. There are multiple options available for dining near the conference site.

Last but not least... The educational program is tentative. The Annual

Conference Committee reserves the right to change the program as needed.

Please check your time zone with that of the conference facility so that you do not arrive late. Please plan enough time to locate the facility, park, register and find the meeting room.

Please bring a sweater or jacket for lectures as the temperature in the meeting rooms can sometimes vary.

We are not able to provide child care services during the educational meetings. Please do not bring children to the educational sessions.

If you have any special needs or require special accommodations, please let the Annual Conference Committee know how we may assist you.

LECTURE SCHEDULE Attendees must be seated in the lecture room prior to

the scheduled start time of each lecture. Individuals arriving after a lecture’s scheduled start time will not be admitted into the lecture room.

Individuals must be present during the entire lecture to receive credit. Leaving the lecture room prior to the end of a lecture will result in automatic loss of that lecture credit.

Lectures must meet ASRT requirements for time to receive full credit. Each hour of lecture equates to one (1) Category A CE credit unless specified otherwise. Lectures must be a minimum of 50 minutes in length to receive one (1) CE credit. The amount of CE credit awarded will be reduced in the event a lecture does not meet the minimum ASRT requirements for time.

Courses have been submitted for and are pending approval by the ASRT for Category A or A+ continuing education credit. The Annual Conference Committee reserves the right to change the program schedule as needed.

CONTINUING EDUCATION CREDIT

Lectures must meet ASRT requirements for time to receive full credit. Each hour of lecture equates to

one (1) Category A CE credit unless specified otherwise. Lectures must be a minimum of 50 minutes in

length to receive one (1) CE credit. The amount of CE credit awarded will be reduced in the event a

lecture does not meet the minimum ASRT requirements for time. ASRT Credit Determination

information:

All participants will be provided with verification of attendance for lectures attended. Verification will

be emailed to you within 30 days of the close of the Annual Conference.

Participants must be seated in the lecture room prior to the scheduled start time of each

lecture. Individuals arriving after the scheduled start time of a lecture will not be admitted into the

lecture room.

Individuals must be present during the ENTIRE LECTURE to receive credit. Leaving the lecture room

prior to the end of a lecture will result in automatic loss of that lecture credit.

Individuals are encouraged to keep a copy of the program booklet with the course descriptions if further

course content is needed for attendance verification at a later date.

It is the responsibility of individual conference attendee to submit documentation of attendance and

continuing education credits earned to the ASRT or any other agency requiring proof of continuing

education.

Individual participants are responsible for submitting attendance verification, speaker credentials, and

any other required information to state agencies requiring them to do so.

Complete the conference registration form in this

booklet.

Mail your completed form, a copy of your ISRT card,

and check or money order (no cash please) to the

address at the bottom of the form. To receive the

published conference rates, registration and payment

must be made in advance.

Mailed registration forms and fees must be

postmarked on or before September 15, 2017 to

qualify for the rates published in this booklet.

ONLINE REGISTRATION

Complete the online conference registration form

Make your payment securely through PayPal. To

receive the published conference rates, registration and

payment must be made in advance.

THIRD PARTY PAYMENT - If registration fee is being paid by an institution (employer or school), the

LATE REGISTRATION FEE - All registrations and payments made on and after September 16, 2017 must

include an additional $25.00 late registration fee. Please do not mail registrations after September 15, 2017 -

plan to arrive early and register the day of the conference.

ONSITE REGISTRATION - If you miss mail and online registration deadlines you can still attend the con-

ference. Onsite registration will be available throughout the conference. An additional $25 late registration

fee will be added to all onsite registration fees.

REDUCED RATES FOR SOCIETY MEMBERS - Members of the Indiana Society of Radiologic Technologists and other state affiliates of the ASRT (ISSRT, OSRT, MSRT, KSRT, etc.) receive lower

registration rates than nonmembers.

You must be a paid 2017 state affiliate member (ISRT, ISSRT, OSRT, MSRT, KSRT, etc.) to receive the member rates. This does not include membership with the ASRT or specialty modality Societies

such as SDMS or SNM.

You can take advantage of member registration rates by joining the ISRT. Join online at www.isort.org or by mailing a membership application with required documentation and a check or money order to

the Executive Secretary.

Individuals who joint he ISRT online please allow 3 days before registering for the conference .

Mailed membership applications must have a postmark date before or the same as conference

registration form to qualify for member rates.

ADVANCE REGISTRATION INFORMATION

CONFIRMATION of REGISTRATION - Confirmation of advance registration will be sent via email. If

you do not provide an email address you will be responsible for contacting the Registration Chairman Jane

Patton at (812)881-9064 to confirm your registration has been received.

PAYMENT RECEIPT - Individuals registering online will receive their conference receipt from Paypal when they register. Individuals registering via mail with check will receive their payment receipt at the con-

ference check-in table.

REFUND POLICY - Requests for refunds must be submitted in writing to the Registration Chairman with a postmark no later than September 15, 2017. Eighty-five percent (85%) of the registration fee will be

refunded. NO REFUNDS WILL BE MADE AFTER September 15, 2017.

GUESTS - Guest badges are available for a spouse or companion who wishes to join you during some or all of the conference. A registered technologist, radiologic science student or a commercial representative may

The ISRT Board of Directors will once again be raising money for the ISRT Scholarship Fund by holding a Silent Auction at the 2017 Annual Conference. All proceeds from the auction will be added to the ISRT scholarship funds for students and technologists. Prize baskets and other items for the Silent Auction are donated by ISRT officers, ISRT members, area hospitals, and commercial representatives exhibiting at the conference. The ISRT Board of Directors encourages all members to attend the Annual Business meeting on Friday, October 6, 2017. The meeting will begin at 10:45 AM. In addition to receiving the election report, competition awards will be presented, certificates of recognition will be handed out, Bylaw revisions will be reviewed and approved, and the new ISRT officers will be installed. This is a great opportunity to learn more about the ISRT and how you can get involved.

ANNUAL CONFERENCE SPECIAL EVENTS The Annual Conference Committee and the ISRT Board of Directors are working together planning extra events and

functions to add to the Conference. Please plan to join us for one or all of our new or updated events.

The Indiana Society of Radiologic Technologists invites all entry-level Indiana Radiologic Science students to

participate in the 2017 Radiography Quiz Bowl. Form a team, register for the competition, and start preparing

to go head-to-head against other Indiana Radiography program teams. Thanks to the very generous support of

the Indiana Radiological Society, the winning team will be awarded the Quiz Bowl Traveling Trophy. The tro-

phy will remain with the winning team’s program for one year. In addition, the top three teams will receive

monetary awards for their achievements.
