    Entrepreneur Memorandum (EM) 1.0 Introduction Centre for Good Governance (CGG) designed and developed Entrepreneur Memorandum Module (EM), a web based application for online filing of EM by the applicant. The industries department will update application status of the individual applications after submission of signed copy online filed application along with enclosures. The EM module consists of filing of following applications.

    Entrepreneur Memorandum Part-I (EM I)

    Entrepreneur Memorandum Part II (EM Part-II)

    Addendum to EM Part II An Applicant can file EM Part-I followed by EM Part II and Addendum to EM Part II. However, applicant can file EM Part II directly without filing EM Part I also. The department will update the status of the application filed by the applicant at different stages till its approval/rejection. The status of the application is made available in the Applicant login account. Based on the online filed applications, the system generates following reports and is accessible to the department officials.

    1.1 Applicant Registration The EM Module can be accessed at http://industries.apcgg.gov.in through any browser (IE 6.0 recommended). A PC with Graphical User Interface (GUI) based operating system with internet connection is required to access the application as shown in Fig1.1

    The Applicant has to register his/her name and address in the site to get username and password to access the application. To get username and password,

    Click on Entrepreneurs. Button under Login o Entrepreneur Login box will be displayed as shown in Fig 1.2.

    Click on New Entrepreneurs Registration link o Applicant Registration form will be displayed as shown in Fig1.3.

    Enter Name of applicant, residential address, Contact Phone Number, Email, Username & password (as per applicant choice), Confirm password (entry of password again), Enter Code (as displayed in Verify) and Click on Create User to Get Username & Password to access the application. Note: 1. Red(*) Marked fields are mandatory fields

    2. The applicant has to note down the User Name & Password to access the application. 1.2 Change Password Login into the application using UserName and Password created by the applicant as shown in Fig 1.4.

    Click on Entrepreneurs button under Login

    Enter User Name and Password under Entrepreneur Login and Click on Submit button to enter into the application.

    In the application home page, Click on Home Change Password to access the Change password form as shown in Fig 1.5.

    To Change Password: Enter the Old password, New Password (Desired) and again Enter New Password against Confirm New Password and Click on Change Password button as shown in Fig 1.6.

    2.0 How to file Entrepreneur Memorandum Online The Following points may be noted for filing of EM application.

    1. For convenience of the applicant the EM application is divided into four (4) sections (Section1, Section2, Section3 and Section4). After filling of each section, the applicant has to Click on Next>> button to go to the Next Section (Page).

    2. Before going to the next section, the system alerts the applicant Are you sure to confirm? as shown in Fig 2.2 whether to save or Edit the current Section.

    If the User clicks on OK, the current section is saved and the next section is opened and if the User clicks on Edit the current page will in edit mode and the user can edit/modify the data in the current section.

    3. The Red Asterisk (*) marked fields in the EM application are mandatory fields and the user must

    enter data in those fields.

    4. In the 2nd Section search option is provided to select the Main Activity (NIC codes) and Products (ASICC Codes) of manufacture/service.

    In the main menu, NIC Codes link is provided to search for Line of Activity/ Manufacturing Product/Service to be provided.

    5. In the fourth (4th) section, Submit button is provided to submit the application to the department online.

    While submitting the application, the system alerts the user whether to review or submit the application. Once the Submit button is clicked it is not possible to edit the application. Therefore, the user is requested to review the application section wise before Submitting the application.

    It is advised to print all the Four Sections and verify its correctness manually and modify

    the application if any data entry errors are found . 6. If there are any data entry errors are found in the Review, modify the data accordingly and Click

    on Save link provided on that section. Each Section is to be saved separately if any modifications are done in that section(s).

    7. After verification of all the four section, Click on Submit link to submit the application to the department.

    8. Copy of application filed and status of application is made available in the applicant login under Application Status link in the main menu.

    9. The online filed application will be processed only after receipt of Signed copy of Online filed application along with necessary enclosures/attachments by the respective District Industries Centre (DIC) located at the District Head quarters within 30 days after filing of the application.

    If the signed copy of application is not submitted in the department even after 30 days of online filing date, then the application is deemed to be rejected and the applicant has to file a new application.

    10. The applicant is advised to check the application status frequently to know the status of the application.

    If the status shows Application Shortfalls then the applicant has to revise the online filed application/enclosures and has to submit the signed copy of application or enclosures as the case may to the department.

    11. Filing type in the application status shows whether the application is F or M1 (First filing or Modified Filing1).

    If the First filing (F) has any shortfalls, then the applicant has to file application again (i.e., Modified Filing M1).

    If there any shortfalls in the Modified filing (M1) also, then the application is rejected and the applicant has to file a new application.

    3.0 Status of Online filed application The online filed application will be processed only after receipt of Signed copy of the application with necessary enclosures/ attachments by the concerned DIC. The status of the application is displayed in the applicant login account under Application Status. The EM Application Status will have the following values.

    4.0 Entrepreneur Memorandum Part-I To file EM Part-I, take the Mouse Pointer to Ent. Memorandum in the main menu and Click on Part-1 as shown in Fig 4.1

    The EM Part-I application will be displayed as shown in Fig 4.2.

    4.1 (EM Part-I)

    4.1 Section -1 of EM Part-I

    Item 1- Enter Surname, Middle Name (Optional) and Name of the applicant

    Item 2- Address for communication & Permanent address. If both the addresses are

    Different, Click Check box () provided above item (2) to activate the

    Permanent address fields to enter data.

    Item 3- Enter Name of the Proposed Unit/Industry Name

    Item 4- Select District, Mandal, Village where the Unit/Industry is proposed to be located,

    Area (Rural/Urban) and enter PIN code of the village/town.

    Item 5- Select nature of activity (Manufacture / Service)

    Item 6- Select category of enterprise (Micro/Small/ Medium)

    Item 7- Select Nature of operation (Casual/Perennial/Seasonal)

    Item 8- Select whether the unit is ancillary (Yes/No)

    Item 9- Select proposed plant and machinery installation date (Month & Year).

    Item 10- Select type of organization (Proprietary/ Hindu Undivided Family/ Partnership/

    Co-operative/ Private Limited Company/Public Limited Company/ Self Help

    4.2 Section -2 of EM Part-I

    In this section the user has to select the Products to be manufactured/ Services to be provided by the proposed Unit/Industry as shown in Fig 4.3 to 4.4.

    1. Search option is provided to select Main Activity (NIC Code).

    Enter appropriate word in Name of Activity (like Steel, Welding, Bakery, Pharma etc) and Click on Search button to filter the corresponding activities in Main Activity combo.

    2. Select appropriate Main Activity in the Main activity combo.

    3. Based on the selection, the Products to be manufactures/Services to be provided are displayed in a grid (table).

    4. Select the appropriate product names by checking the Check boxes () and click on Add Products link.

    5. The list of Products will be displayed in a table below the selection grid. These are the products of manufacture/ services to be provided by unit/industry. If desired, the user can delete the products if it found that the products are not correctly selected.

    6. Click on Next>> to go to Section3.

    The system alerts the user Are you sure to confirm, Click on OK to Confirm and Click on Edit if the data is to be edited.

    4.3 Section -3 of EM Part-I

    Item 12- Proposed Investment in Fixed Assets i) Land Select Land Type (Owned/Rented/Leased) and enter Land Cost in Rupees Lakhs if

    the land type is Owned only. ii) Building Select Building Type (Owned/Rented/Leased) and enter Building Cost in Rs

    Lakhs if the building type of Owned only. iii) Plant & Machinery Applicable to Manufacture sector only and Enter value P&M in Rs

    Lakhs.

    iv) Equipment Applicable to Service sector only and Enter Equipment value in Rs Lakhs. v) Foreign Equity- Enter Foreign Equity if any in Rs Lakhs .

    The Total Investment is calculated as the sum of Land Cost(Owned), Building Cost(Owned), Plant & Machinery ( If Sector is Manufacture ) and Equipment ( If Sector is Service)

    The limits of Total Investment for Manufacture and Service Sector are as follows.

    Item 13- Installed Capacity Per Annum- Enter quantity and Units. The products may be either in Plant A or Plant B or in both. Units may be (Metre, Thousand Metre, Kilometer, Square Meter, Litre, Cubic Metre, Tonne, Kilogram, Number, Thousand Number,Pairs, Thousand Pairs, Gross, Thousand Gross, Ream, Thousand Boxes, Lakh Numbers, Dozen, Grams, Kilowatt Hour, Million Cubic Centimetre, Carat, others) Item 14- Power Load- Select Horse Power (HP) or Kilowatt (KW) and Load required. If HP is selected, and 50 is entered Then the Power Load is 50HP

    If KW is selected, and 50 is entered Then the Power Load is 50KW. Item 15- Other Source of Energy/Power Required

    Select Source of Energy/Power, Enter Quantity of Energy/Power Required and their Units (HP/KW/Tons etc) and Click on Add link to save the source of energy selected. The user can add more than one source of energy. Click on Add link to create one more row for selection of Source of Energy.

    Item 16- Expected Employment Enter Management/Office Staff personnel (Nos.), Supervisory Personnel (Nos.) & Workers (Nos.) and the Total Employment will be displayed as the sum of the above three. Click on Next>> to go to Section 4.

    4.4 Section -4 of EM Part-I

    Enter Name of the Entrepreneur, Select Gender (Male/Female), Select Social Status/category (SC/ST/OBC/Others/Physically Challenged), Knowledge Level(Technical Graduate, Management Graduate, Post Graduate, Other Graduate, Under Graduate, Any other Lower), Equity Participation (%), Percentage of Total Equity, Stake in other enterprises (Yes/No), Remarks if any.

    If the type of organization selected in Section 1 is Proprietary then applicant name will be displayed in Name.

    Item 18- Expected Schedule of Commencement of Production/Activity

    Select Month & Year of commencement. Click on Submit link to submit the application to the department. After Submitting the application, the system alerts the user Do you want to Review or Submit the Application? (Once Submitted it is not possible to edit the application) as shown in Fig 4.7

    The user has to review the application filled and after, review the Submit button will be enabled as shown in Fig 4.7a.

    2. Click on save link if any changes are made in any section. 3. After review of all sections, Click on Submit button to submit the application.

    4.5 Review & Submission of online Application

    After entry of Section-4 information, Click on Submit link, then the system alerts the user whether to Review or Submit the application. Click on Review button, then the application will be displayed with links to all the four sections (Section-1, Section-2, Section-3, Section-4) as shown in Fig 4.8.

    Fig 4.9 (Review of Section 2) In the same way Review the four sections before submitting the application. If there is any modification to be done in any section, do the necessary modifications and Click on Save link provided in that section as shown in Fig 4.9 to save the modified data. After review of all the four sections, go to Section-4, and Click on Submit link and again the system alerts the user Do you want to Review or Submit the application? Since the Review of application is completed, the user can click on Submit button to submit the application to the department as shown in Fig 4.10.

    After submission of application, the system will display message showing that Copy of application files is available in the user account as shown in Fig 4.11 and also the copy of the Application filed along with a letter as shown in Fig 4.12 to Fig 4.16.

    A copy of the application filed is made available in the login account of the user. To view/ print copy of the application,

    Take the mouse pointer on to the Application Status and Click on Ent.Memorandumas shown in Fig 4.17.

    An application status window is opened as shown in Fig 4.18 and Click on symbol under Application PDF.

    5.0 Entrepreneur Memorandum Part-II Filing of EM Part-II application is similar to Part-I application as described in Chapter 4.0. If the EM Part-I is already submitted, then the data entered in EM Part-I is reflected in EM Part-II. The additional fields required to be filled in the EM Part II are marked with yellow colour. However the existing EM Part-I data can be modified if required. To file EM Part-II, take the Mouse Pointer to Ent. Memorandum in the main menu and Click on Part-2 as shown in Fig 5.1

    5.1 (EM Part-2)

    5.1 Section -1 of EM Part-2

    Item 1- Enter Surname, Middle Name (Optional) and Name of the applicant

    Item 2- Address for communication & Permanent address. If both the addresses are

    5.2 Section -2 of EM Part-I

    In this section the user has to select the Products to be manufactured/ Services to be provided by the proposed Unit/Industry as shown in Fig 5.3 to 5.4.

    5.3 Section -3 of EM Part-II

    vi) Date of Investment: Enter date of investment (Select from the Calendar) The limits of Total Investment for Manufacture and Service Sector are as follows.

    Item 18- Date of Commencement of Production/Activity

    Select Month & Year of commencement.

    Click on Submit link to submit the application to the department. The system alerts the user Do you want to Review or Submit the Application? and (Once Submitted it is not possible to edit the application) as shown in Fig 5.7

    5.5 Review & Submission of online Application

    After entry of Section-4 information, Click on Submit link, then the system alerts the user whether to Review or Submit the application. Click on Review button, then the application will be displayed with links to all the four sections (Section-1, Section-2, Section-3, and Section-4) as shown in Fig 5.7.

If there is any modification to be done in any section, do the necessary modifications and Click on Save link provided in that section as shown in Fig 5.8 to save the modified data. After review of all the four sections, go to Section-4, and Click on Submit link and again the system alerts the user Do you want to Review or Submit the application? Since the Review of application is completed, the user can click on Submit button to submit the application to the department as shown in Fig 5.9.

    6.5 Issue of Acknowledgement

    7.0 Submission of modified application (M1) If the application is identified with shortfalls by the department, then the Applicant has to revise the

    online filed application or submit the shortfall documents or both. The applicant has to submit the

    revised online filed application along with shortfall information if any to the department within 30 days

    from the date notification of shortfalls.

    After online filing of modified application (M1), the application status in the applicant login will be as

    shown in Fig 6.1. Print online filed application & affix signature and submit three copies in the concerned

    District Industries Centre (DIC) to initiate the application processing.
