Diagnostic Services include: • Humphrey Visual Field tests • Ocular Coherence Tomography • Nerve Fibre Analysis • Fluorescein angiography/Isocyanine green angiography • Orthoptic assessment of ocular motility conditions, binocular vision, diplopia and

symptoms of eye strain • Colour vision assessment • Pentacam • Endothelial cell count • Keratometry (measurement of the curvature and diameter of the cornea for

diagnostic purposes) • Refraction (measurement of the refractive power of the eye for diagnostic

How to refer

Routine Outpatient Clinic Referrals

All referrals require triage prior to appointment allocation. The Eye Clinic recommends the use of our endorsed Referral Template (attached on following page) which ensures all minimum referral requirements are met. Email the completed referral form to [email protected] alternatively Fax to (02) 8890 6117 Mail to PO BOX 533, WENTWORTHVILLE 2145 In order to triage accurately to the most appropriate specialist clinic, within a clinically suitable timeframe, it is critical that we receive accurate and details referral information. In some cases this may require diagnostic support from local ophthalmologists or optometrists. The referring GP or optometrist must include: • Clear statement of symptoms • Duration of problem – sudden onset / long standing • Risk factors • Date of last eye examination – including refraction and BCVA • Current diagnostic report if available

For Urgent Referrals, please follow the above process to submit the referral to the Eye Clinic. Practitioners and Health Care Professionals, please contact the clinic via (02) 8890 6668 to speak to our Nurse Team Leader to progress the referral. Emergency Referrals For Emergency Referrals please phone the Westmead Hospital switchboard on (02) 8890 5555 and ask for the Ophthalmology Registrar On Call or ask the patient to present to our Emergency Department:

Full Orthoptic Services and Diagnostic Screening ANTI -VGEF SERVICES AVAILABLE IN PRIVATE ROOMS Westmead Hospital’s Community Eye Care (C-Eye-C) Program has been established for the management of patients with low-risk chronic eye diseases, such as diabetic eye disease or glaucoma. To refer to C-Eye-C please follow the above referral process sending your referral to allow the referral to be triaged appropriately.
