Additional Requirements for Application

ExperienceO Observation Experience: Receive points for observing direct

Occupational Therapy services in one or more of the following settings. Must have been completed within last 2 years (since April 2012). No more than 4 hours from any one site will be considered for points. 2-3 hrs = 1 pt, 4 hrs = 2 pts; 6 points max Setting Options: — Pediatrics — Physical Disabilities — Geriatrics

O Volunteer/Work Experience: Receive points for any combination of paid work and/or volunteer experience involving direct patient contact in a healthcare setting. Must have been completed within last five years (since April 2009). 24 hrs = 1 pt, 48 hours = 2 pts, 72 hours = 3 pts, 96 hours = 4 pts; 4 points max

O Computerized Placement Test (CPT) for Reading or Petition to waive
