Health Care Budgets Specific budgets for individual services

Services provide care to specific patient groups

Most services have a dedicated team of nurses to deliver delegated aspects of care

Total Service DemandComponents of Work:

Clinical Care (Medical, Nursing, Allied Health)• Technical • ADL’s• Nutritional • Emotional Support / Education • Functional Environment

Management work Administrative work Environmental work Professional development work Maintenance work

Each department will have a unique service demand

Determining the Required Nursing ResourcesThis involves matching demand for nursing hours and the supply of nursing hours by:

Identifying ALL the work that will be delegated to the nursing team

Designing the nursing team profile that will match the identified work demand

o Skill Mix

o Nursing hours for inpatient care

o Nursing hours for other clinical activities

o Roster pattern that matches peaks and

troughs in acuity and activity

Nursing – Providing the Largest Care Component

Inpatient Services

Nursing Operates 24hrs/day caring for all patients Wide scope of clinical activities Overlapping disciplines – delegated, transferred and defaulted

work to nursing services Assigned and/or defaulted environmental work

Work Completed by the Nursing Service

Delegated and

defaulted work from the allied

health team

Delegated and

defaulted work from

the medical team

Defaulted administrative/clerical tasks

Defaulted environmental tasks

• Work that is delegated by, or defaulted from allied health staff

• Work that is defaulted to nurses when the ward clerk is unavailable

• Environmental tasks are generally defaulted to nurses after hours

Using Information Technology to Measure Nursing Work

• Must be integrated with or seamlessly interfaced with:

PMI

Roster

HRM

• Captures ALL work completed by nurses and nursing support services

• Requires minimal data entry

• Automatically collates the data

• Produces reports on all components of nursing work

Data Analysis

Provides evidence of:

• Volume and type of work done by nursing in a service

• Funded and non funded nursing work

• Inadequate or over resourcing of nursing services

• Inadequate or over resourcing of nursing support services

Cler, Other, MED, AH, Unpaid Student

RN, EN, AIN

Using Data to Generate Change

Enables Nurse Managers to:

• Identify work that should be moved to another service

• Identify work that should be delegated to another discipline / support service

• Develop realistic budgets

• Improve productivity and efficiency

• Maintain fair and equitable nursing workloads

• Achieve improved patient outcomes
