A Healthy PartnershipSome people are blessed with good health, but for most of us, getting and staying healthy takes the right health care coverage and a commitment to our well-being . At Phillips 66, we offer a great start with our coverage options, but to make the most of that coverage, you need to do your part as well .

In this issue of BenefitsNews, we’ll look at easy steps to maximize your health care coverage . First, we’ll examine the importance of having a primary care physician (PCP) to help you make sound health care decisions and coordinate all your care .

In addition, we’ll tell you about the information and tools available through the CVS/caremark website to help you manage and save on prescription drug expenses, whether it’s finding a network pharmacy, using generic medications or accessing the comparison shopping tool to make smart consumer choices .

And don’t miss our announcement about the new Lean in ’15 wellness challenge from WebMD that focuses on the role of good nutrition in supporting your optimal health .

Last but not least, we’ll preview changes to retiree medical coming in 2016 that will give retirees even more flexibility and savings when it comes to coverage options .

A 2011 Dartmouth study published in the Journal of the American Medical Association found that patients under the care of a primary care doctor are more likely to live longer and stay healthier .

Studying the medical claims of 5 million Medicare patients, researchers found that patients with the greatest access to primary care physicians were 9 percent less likely to be admitted to a hospital for preventable conditions and had a 5 percent lower death rate .

Choosing a PCPAs with any important relationship, you’ll want to make sure that you and your PCP are a good fit . Make sure your PCP is a network provider to obtain the highest benefit coverage . Here are some areas to consider when selecting a PCP:

1. Medical qualifications and experience: In addition to looking at education, you’ll want to ask about board certification and special areas of expertise .

facilities are conveniently located to you . Check to see that they are accredited and meet high standards for quality and safety .

3. Communication style: Are you happy with a “just the facts” approach or do you prefer knowing all the details? Do you like to discuss other treatment options and their pros and cons? Are you comfortable with someone who’s very direct or do you prefer a nurturing personality?

4. After-hours policy: What happens if you have a sudden illness or injury and need to reach your PCP after office hours? Who is on-call to cover for your PCP if he or she is unavailable?

5. Protocol for handling patient calls: Ask about procedures for handling patient calls . Does he or she take calls during office hours? Is a nurse or assistant designated as a liaison to handle questions for your PCP?

Most of us do more research when buying a new smartphone than when seeking health care . The good news is that your primary care physician (PCP) can help make that process easier, by providing valuable guidance on health care decisions .

Managing Your Health CareYour PCP plays a central role in managing your health care . They:

• Understand your health history and habits

• Perform routine check-ups, preventive screenings and immunizations

• Diagnose and treat you for minor, acute and chronic conditions

• Provide referrals when needed, and coordinate your treatments and medications

• Advise on lifestyle changes to help you prevent or manage chronic illness

These days, it’s becoming more common for physicians to ask for patient Social Security numbers (SSNs) for billing and collection purposes . Please be aware that Phillips 66 does NOT require you to provide your SSN to your doctor’s or dentist’s office.

Providing your SSN is entirely voluntary, unless you are enrolled in a government health care plan such as Medicare, Medicaid, a Veterans Health Administration plan or TriCare, which require an SSN for reimbursement purposes .

According to the Social Security Administration (SSA), you should be cautious whenever you are asked for your SSN, due to risk of identity theft . However, there are times when a doctor’s office may ask you to confirm your SSN in instances where patients may have the same name and/or date of birth . In such instances, you should ask:

• Why do you need it?

• How will you use it?

• What law requires me to provide it?

• What are the consequences if I refuse?

If the doctor’s office is insistent, try asking if they will accept just the last four digits . Bottom line, if you’re not comfortable sharing your SSN with a health care provider, you have the right to decline the request .

Protecting Your Data at the Doctor’s Office

File Claims for 2014 FSA Expenses by June 30Remember, claims for the Health Care Flexible Spending Account (HCFSA) and/or Dependent Care Flexible Spending Account (DCFSA) for services rendered from Jan . 1, 2014 through Dec . 31, 2014 must be postmarked by June 30, 2015 .

Make the Most of Your Prescription Drug CoverageStudies show that prescription drug expenses are one of the biggest factors behind rising health care costs . With CVS/caremark, you can take advantage of savings on a wide range of generic and brand-name medications . Here are some tips on making the most of your coverage:

• Register on the CVS/caremark website: When you register as a user at www.caremark.com, you have access to a wide range of services for managing your prescription coverage, including:– Locating a pharmacy– Printing your ID card– Ordering and renewing prescriptions– Reviewing price comparisons

Just log onto www.caremark.com, and click “Register Now” to get started .

• Download the CVS/caremark mobile app: You can download the CVS/caremark mobile app either from www.caremark.com or from the HR Connect Mobile App and have all of your prescription drug info at your fingertips .

• Comparison shop: You can also use the website to track your Rx spend and learn about ways to save on prescription drugs (for example, by switching to an alternate drug or opting for a 90-day supply at mail order or through Maintenance Choice for long-term medications) . Additionally, for retail generic drugs, you can find locations that offer great savings (for example, you can obtain $4 generics on certain medications at Walmart) .

• Review the 2015 Formulary List: CVS/caremark periodically reviews the latest information on drugs, products and prices to ensure that covered medications work well and are cost-effective . Please review the 2015 Formulary List available in the Medical section on hr.phillips66.com to see the current generic and brand-name drugs covered under the Phillips 66 plan . Switching from a non-preferred brand to a preferred brand can save you significant money, and if available, moving to a generic can save you even more!

Sign Up for a Biometric ScreeningRemember, you need to complete a biometric screening and health assessment to participate in the 2016 Wellness incentive Program . Getting a biometric screening is an important step for preventive care . Your results must be submitted on or before November 30, 2015 .

HSA Bank to Become Our New HSA AdministratorLast year, JPMorgan Chase sold its health savings account (HSA) business to HSA Bank . As a result of this sale, HSA accounts for employees with Phillips 66 affiliated Chase HSA accounts will be transferred to HSA Bank . We anticipate the change to happen this fall and we’ll provide more information as the details become available .

Consider New Options for Retiree Medical

More Information Is Coming!We’re excited about the upcoming changes to the RMA and the greater flexibility and choice Phillips 66 retirees will have when choosing retiree medical coverage . Additional details will be available in the coming months prior to this year’s Annual Enrollment .

A Quick Primer on Exchange OptionsMost Phillips 66 retirees will find that coverage options on the Exchange not only meet their health care needs, but also cost less than options available through Phillips 66 because they pool the insurance risk across more lives than just former Phillips 66 employees .

Exchange options are available in four tiers (bronze, silver, gold and platinum), to suit a range of coverage and budget needs . As a point of reference, the Phillips 66 Consumer Plan and PPO Plan are comparable to “gold tier” options .

While options in a tier may differ in insurance carrier and plan design, the actuarial value (the portion of the cost of services that would be paid by insurance) is essentially the same from option to option within each tier .

When Phillips 66 transitioned to the Retiree Medical Account (RMA) in 2013, the health insurance marketplace was preparing for change driven by health care reform . Since then, more medical coverage options have become available to retirees . In response to this changing health care landscape, we’re making enhancements to the RMA effective January 1, 2016:

• Added flexibility for RMA credits: You can use RMA credits, if eligible, not only to help pay for coverage options available through Phillips 66, but also to reimburse yourself for the cost of any retiree medical coverage purchased through the health insurance exchange (Exchange) .

• New consumer resources for choosing Exchange coverage: With so many additional options available to retirees on the Exchange, Phillips 66 is partnering with an outside vendor to provide resources for evaluating these new options . Licensed representatives will be available to provide information and answer questions . In addition, retirees will have access to online tools and resources to shop for and compare coverage options .

The Cash Balance Account — A Key Component of Your Retirement SavingsFinancial security is important at any age, but especially so during retirement . Your benefit from the Cash Balance Account is one of the key components of your retirement savings — working with Social Security, your benefit under the Phillips 66 Savings Plan and your own personal savings, to provide valuable retirement income .

The chart below provides the basics about the Cash Balance Account . Remember, some employees may have access to other retirement benefits instead of the Cash Balance Account .

Cash Balance Account at a GlanceDo I need to enroll? No . If you’re eligible, you’re automatically enrolled .

Who pays? The company pays all costs . You can’t contribute .

What’s my benefit? Unlike a bank account, in which you deposit and accumulate money, the Cash Balance Account is a nominal (in name only) account, which means that instead of accumulating money, it accumulates credits .

Each month, your account increases with pay credits and interest credits . Your benefit is based on the total amount of those credits in your account when you leave the company, your age when your benefit payments begin, and the form of payment you elect .

Your benefit is viewable on YBR or HR Express .

When am I vested? Generally, you’re vested after three years of service . That means the value of your account is yours to keep when you leave the company .

When do I get my benefit? You can start taking your vested benefit as early as the first day of the month after you leave the company . However, you must take your benefits when you reach your normal retirement date, unless you’re still employed by the company at that time . Don’t forget to keep in mind that tax penalties may be associated with early withdrawal .

What is my normal retirement date? It’s the first day of the month closest to your 65th birthday .

How is my benefit paid? You have a choice of annuities (monthly payments for life) . You can choose payments for your lifetime only, or for the combined lifetimes of you and your spouse . Or you can choose to take it in a lump sum . If you’re married, you’ll need your spouse’s consent for some of the options .

Even though Phillips 66 contributes 100% to your Cash Balance Account, it’s valuable to understand how the plan works and how your benefit is accrued . Check Out Your Social Security EstimateTo get an idea of your estimated Social Security benefit, based on your actual earnings record, check out the Retirement Estimator available on the Social Security Administration web site at http://www.ssa.gov/retire2/estimator.htm .

Like the Cash Balance Account, your Social Security benefit helps you build a foundation for retirement . So be sure to keep track of your earnings and verify this information every year .

Body Mass Index (BMI) is one of the factors measured when you get a biometric screening . Your BMI tells you if you’re underweight, overweight or the appropriate weight for your height .

Because BMI focuses on body fat, an unhealthy BMI can be a red flag for serious health problems such as cardiovascular disease, hypertension, high blood pressure, diabetes, and cancer .

Understanding BMI RangesThe chart below shows BMI ranges for adults . Although these ranges apply to most people, there are exceptions . For example, women who are pregnant or lactating may not have a BMI within the normal range . In addition, some competitive athletes and body builders will have a much higher BMI because they have a larger amount of muscle .

Different Factors for Children and TeensBMI calculations for children and teenagers also take into account age and gender, since their level of body fat changes with age and the amount of body fat differs between boys and girls . To reflect these differences, the BMI chart for children and teenagers includes age- and gender-specific ranges .

• Nearly 7,000 employees are earning a discount on their medical premiums in 2015 because they participated in the Wellness Incentive Program in 2014 . Of those, 60% are receiving the full $75 per month discount!

We’re on the right track, but we could do even more . Let’s stay focused by eating right, staying active and making the most of the wellness programs and resources available to boost our health and well-being!

While working out on a regular basis is a good thing, repeating the same exercises day after day could actually prevent you from making progress . Research shows that your muscles get accustomed to doing the same fitness routine, so your rate of progress may slow down or even plateau over time . To breathe new life into your workout, try the following tips:

• Try new equipment: If you work out with barbells or dumbbells, try elastic bands or cables . If you usually do the treadmill, try the stair stepper instead .

• Vary your speed: Instead of fast reps with light weights, try slower reps with heavier weights . If you’re a jogger, try some intervals of speed walking or sprinting .

• Check out a new class: If you’re an aerobics fanatic, try a kickboxing class . Swap out your regular Zumba class for a cycling or step class .

• Alter your sequence: Change the order of your exercises . If you typically do upper body work Mondays and Wednesdays, work your lower body instead . If you always do cardio before weights, try swapping the order and do weights first .

• Change your scenery: Do some hill work instead of jogging on a flat surface . Check out a new hiking trail or bike trail .

• Use the buddy system: If you always work out by yourself, try exercising with a friend . You might find that the two of you will push each other in a positive way for an even better workout .

So if your exercise regimen has become a chore, try adding some variety to your routine . By making just a few changes, you’ll stay motivated, work new muscles and see better results in the long run .
