IntegrationofMedicalImagingServices-AnIncredibleJourneyThisIntegratedMedicalImagingReportisatestamenttothetremendousefforts,activitiesandachievementsofourdedicatedMedicalImaging(MI)Team.Whileitisan“AnnualReport”ithighlightstheresultsofmorethanthreeyearsofplanningandhardworkandmarksanimportantmilestoneforeveryoneinvolvedwithMedicalImagingservicesacrossFraserHealth,ProvincialHealthServicesAuthority,ProvidenceHealthCareandVancouverCoastalHealth.

TheprocessbeganinAugust2009whenhealthauthorityCEOsidentifiedMedicalImagingasoneofseveralprogramstobeincludedinlowermainlandconsolidation.Thiswaspartofanoverallprocesstoimproveaccessandachieve

efficienciesbycreatingoneorganizationalstructuretosupportservicesacrossallhealthauthorities.Inthefallofthatyearweconductedaservicereviewandproductivityassessment.ByJanuary2010weannouncedanew,streamlinedleadershipteamwithagoaltoreducecostsby10%whilemeetingincreasingdemandsforMedicalImagingservicesacrossallhealthauthorities.

ThegeographicareasupportedbyMedicalImagingextendsfromHopetoBellaCoola,farbeyondtheboundariesofthelowermainland.WemovedforwardasanIntegratedMedicalImagingteamandinitiateddetailedplanningforaconsolidatedMedicalImagingservicemodel.AneworganizationalstructurewasannouncedinOctober2010andwasimplementedbythefollowingApril.Throughoutthistransitionprocessweworkedtosupportstaffandensurepositionswereavailableforeveryonewhowantedarolewithinthenewintegratedmodel.

ThefinalstepintheconsolidationprocesswasthetransferofallstafftoVancouverCoastalHealth(VCH)inMay2011.Thiswasanemotionalprocessformanyindividualswhohadworkedforyearsasemployeesofanotherhealthorganization.Whilewerecognizeandcontinuetosupportthestrongworkingrelationshipsthatexistwithinthesitesandhosthealthorganizations,thestafftransfertoacommonemployerwasessentialtoachievingourgoalsforimprovedefficienciesandstandardization.Withthestafftransfercomplete,aneworganizationalmodelestablishedandafive-yearstrategicplaninplacethejourneycontinues.Wehavemadetremendousprogressandachievedmorethan$7millioninsavingsto-date,whileservingmorepatientsthaneverandimprovingproductivity.

Theseachievementsaresignificant,butwhatimpressesmethemostisthepeoplewhohavemadethispossible.Wehavemorethan2500MedicalImagingstaffworkingat30+sitesacrossthelowermainlandandcoastalcommunities.Eachhasdemonstratedtheirongoingcommitmenttopatientsandfamiliesbycontinuingtodeliverthebestpossiblecareandserviceduringaperiodofconstantchangeandconsiderablechallenge.IamproudtohavebeenpartofthisdedicatedMedicalImagingTeamandthankeveryonefortheirongoingsupportandcommitment.

MI/IMITSStrategicSteeringCommitteeIntegratedMedicalImaging(MI)andVCH/PHSAInformationManagement/InformationTechnologySolutions(IMITS)andFraserHealthAuthority(FHA)IMITSarecollaboratingonanumberofMIInformaticsProjects.TheseinitiativesareessentialtoachievingthebenefitsintendedthroughIntegratedMedicalImaging.KeybusinessdriversfortheseMIInformaticsProjectsinclude:

• ImprovedclinicalcarethroughaccesstorequiredMIdataandinformationwhen/whererequiredtosupporttimelyandappropriatedecisionmakingandimprovedcollaborationamongcliniciansacrossallIntegratedMedicalImagingsites.

• ImprovedbusinessdecisionsandresourcemanagementwithaccesstoMIdataandinformationasrequired.

• Reducedcostsresultingfromimprovedworkflow,systemintegrationandincreasedefficiencyenabledthroughtechnologysolutions.

• Compliancewithrequiredprivacyandsecuritystandardstoensuresafeandsecureaccessto/sharingofMIdataandinformation.

TheMI/IMITSStrategicSteeringCommitteemeetsmonthlytoprovidehighleveloversightandguidancetoallMIInformaticsProjects.Prioritiesforthepastyearincluded:

SpeechRecognitionProject

Thisinitiativewasfocusedonestablishingrequirementsandselectingavendorforatechnologysolutiontoimprovereportturnaroundtimesbyenablingradiologiststodictateandcreatereportsinrealtime.VendorselectionisunderwayandtheSpeechRecognitionSolutionwillberolledoutin2013.

VCH/PHCPACSRFPProject

InFebruary2012itwasdecidedthatanewPACSsystemisrequiredforallVCH/PHCsites.Thiswouldaddressexistingriskassociatedwithoutdatedtechnologyandwouldalsocreateacommonsystemforaccessing/sharingimagesandreports.ARequestforProposals(RFP)wasinitiatedinsummer2012toselectaPACSsolutionforallVCHandPHCsitesandestablishacommonPACSplatform,andcloseralignmentwithalowermainlandandpossibleprovincialPACSstrategy.

AgfaPACSUpgrade

AnupgradeisunderwayoftheAgfaPACSatVGH,UBCH,MSJ,RH,andLGHtoaddresssystemcrashesassociatedwiththesebeyondend-of-lifesystems.ThisupgradeisfocusedonstabilizingtheexistingPACS,anyenhancementswillbeaddressedviatheRFPprocessthatisunderway.ThisupgradewillbecompletedwithinthestandardoperatingbudgetallocatedtoPACSsystemmaintenance.

MembersoftheMI/IMITSStrategicSteeringCommitteeoverlapwiththeMIExecutiveCommitteemembershiptoensurestrategicalignmentandcontinuitybetweenthesetwoleadershipgroups.

ExtendedLeadershipCommitteeTheMedicalImagingExtendedLeadershipCommitteeincludessitecoordinators,regionalpracticeleads,nuclearmedicinesupervisorsandotherMedicalImagingleaderswhosupervisestaffandhaveoperationalresponsibilities.Thisgroupisintendedtomeet4-6timesayearandisstillinthedevelopmentphase.

Giventhegeographicdistancesbetweensites,CommitteemeetingsprovideanopportunityfortheseMedicalImagingleaderstoconnectface-to-faceanddiscusstopicsofcommoninterestorconcern.Itisalsoanopportunitytohearfromseniorleaders,askquestionsandprovidefeedbackwithregardstochangesandnewprograms.Agendasreflectoperationalprioritiesandpresentationsareaimedatensuringmembersareupdatedonnewinitiatives.


RegionalQualityandSafetyCommitteeAProgramQualityCommitteewillbeformingunderDr.PatrickVosandexpectedstartinfall2012.ThecommitteewillhaveprimaryresponsibilitytomonitorandadviseonmedicalortechnicalissuesrelatedtoqualityofcareandpatientsafetywithinIntegratedMI.

RadiationSafetyCommitteeInconjunctionwithOccupationalHealthandSafety(OH&S),aradiationsafetymanualhasbeencreated.ARadiationSafetyCommitteeforNuclearMedicinestartedinfall2011.ARadiationSafetyCommitteeforX-rayemittingdevicesisstartinginfall2012

OtherIntegratedMedicalImagingCommitteesandWorkingGroupsInadditiontothemanystandingcommitteesestablishedtoguidetheplanningandsupporttheprogressofMedicalImagingprogramsandservices,thereareothergroupscreatedasrequiredtosupportspecificprojectsorinitiatives.TheseincludeClinicalAdvisoryGroupswithradiologistsandotherclinicalstakeholdersparticipatingforaspecificperiodoftimetofocusonataskorimportantdecisionsuchasthepurchaseofnewtechnologysystemsorothercapitalequipment.Othercommitteesareestablishedtosupportoperationalinitiativessuchasthenewtimekeepingandpayrollsystemthatwasimplementedoneyearago.Thesecommitteesfunctionforthedurationoftheprojectoraslongastheyareneededandarethendisbanded.

• PeerReviewIntegratedMedicalImaginginitiatedaradiologistPeerReviewPrograminfall2011.Thispilotprojectinvolvesradiologistsatallfacilitiesagreeingtohave2%oftheirComputedTomography(CT)studiesreviewedbycolleaguesasaqualityassessmentinitiative.ThePeerReviewProgramhasbeenimplementedat80%ofsitesto-dateandiscurrentlyfocusedonCTreporting.ThiswillcontinuetoexpandandultimatelyalignwithaProvincialPeerReviewProgramthatisbeingdeveloped.

Access,QualityandSafetyInterventionalRadiologyServiceExpansion

InterventionalRadiology(IR)isaspecializedservicethatusesminimally-invasiveimage-guidedproceduresforpatientswithavarietyofhealthconcerns.Theseproceduresincludevenousaccessfordialysisandchemotherapypatients,angioplastiesforrenalaccessandperipheralarterialdiseaseandembolizationproceduresforactivebleeding,tumorsandfibroids.OtherexamplesofIRproceduresincludechesttubeplacements,percutaneousdrainagesofabscesses,biliarydrainagesandfeedingtubeinsertion.

InterventionalRadiologyserviceshavehigherassociatedcoststhanotherMIservicesduetothespecializednatureoftheproceduresandthemedicalandsurgicalsuppliesrequired.MedicalImagingdevelopedabusinesscasein2011toaddressincreasingdemandforIRservicesinFraserHealthandatBCChildren’sHospital.Inadditiontorisingcosts,bothSurreyMemorialHospitalandBCChildren’sHospitalwereunabletomeetgrowingpatientneedsforIRprocedures.

MedicalImagingreceivedatotalof$1.2millioninadditionalfundingtoaddressIRcostpressures.ThisfundingwillestablisharenalvascularaccessIRserviceatSurreyMemorialHospitalandfundmorethan500IRcasesatBCChildren’sHospital.

BenefitsofInterventionalRadiologyInterventionalRadiologyprovidesanalternativetomorecostly,complexsurgicalproceduresandfreesupORtimeforothersurgicalpriorities.Itisalsoalessinvasiveprocedure,whichiseasieronpatientsandsupportsfasterrecovery.OtherbenefitsofIRincludereducedneedforgeneralanesthesia,shortenedhospitalstays,reducedpatientrisk,lesspainandimprovedhealthoutcomes.

ImprovingTranscriptionReportTurnaroundTimes

OverthepastyearsignificantchallengesaffectedMedicalImagingtranscriptionreportturnaroundtimesatvarioussitesacrossthelowermainland.Thesewereattributedtoanumberoffactors,includingstaffingshortages,unplannedabsences,systemdowntimesandincreasedactivities.Thesefactorscametoacriticalpointinsummer2011whenreportturnaroundtimesaveraged4-7days,wellabovethe24hourstandard.

Anumberofactionsweretakentoaddressthesereportingdelays.Thisincludedsharingstaffacrosssites,overtimeetc.Resourcesweredistributedefficientlytotheareasofgreatestneedandalmostimmediatelythebacklogofreportsbegantodecrease.BytheendofOctober2011,MedicalImagingwascaughtupwiththebacklogofreports.

ThankstostaffcommitmentandimprovedprocessesthemajorityofMedicalImagingsitesareconsistentlymeetingthe24hourturnaroundtime.

PeerReviewProgramforRadiologistsInitiated

AnewPeerReviewProgramisakeyinitiativeunderwaytosupportthequalityandsafetyofourIntegratedMedicalImagingservices.ThisisaninternalradiologistledprocessthatispositionedwithinthecontextofouroverallQualityAssuranceProgram.Itwilllinktotheprovincialpeerreviewprogramwhenitisrolledout.

TheradiologistPeerReviewProgramwasphasedinoverthepastyear.ItstartedwithUBCHospitalandPeaceArchHospitalandnowincludesallProvidenceHealthCaresites,NorthShoresites,andallFraserHealthAuthoritysites.TheremainderofthesitesatVancouverCoastalHealthandProvincialHealthServicesAuthorityarescheduledtogoliveinfall2012.

MRI–ImprovingAccessandReducingCosts

IntegratedMedicalImagingperformed27%moremagneticresonanceimaging(MRI)examsin2011thantheydidin2009whiledecreasingtheoverallcostperexam.Thisachievementwaspossiblewithincreasedfundingandconsiderablestaffcommitmenttoimprovepatientaccessandoperationalefficiency.

In2010theHealthServicesPurchasingOrganization(HSPO)ofBCprovidedpatientfocusedfundinginordertodecreasethewaitassociatedwithelectiveMRIexams.Atthattime,thelongestwaitlistforMRIexamswasover160days.IntegratedMedicalImagingreceived$5.4millioninfundingfromHSPOin2011toprovideanadditional18,000exams.Asaresult,averagewaittimesdecreasedbetween20-30%.

Theoverarchinggoalfor2012istohaveanexcessof50eventswithintheregionandtoincreasethegeneralknowledgeofLeanmethodologywithinMedicalImagingstaffincluding:thetoolsandmethodsweuseandthewaysthattheseeventsarebenefittingpatientsandstaff.TheseeventspresentanopportunityforMedicalImagingstafftoparticipateinprocessimprovementrelevanttotheirworkflowandexperience.

IntegratedMedicalImagingexpandedsupportforpost-secondarystudentsinB.C.ThisincludesBCIT’srestructuredmedicalradiologyprogramandsonographyprogram.ItalsosupportstheupcomingmedicalradiographyprogramexpansionsattheCollegeofNewCaledoniaandCamosunCollege.

UnderthenewBCITprogramclinicalpracticumrotationswillrequireadditionaltrainingandorientation.TheBCITsonographyprogramhasrecentlychangedtoa2yearstructure,necessitatingthatlowermainlandteachingsitesaccommodatestudentsforalongerdurationthroughouttheyear.TheCollegeofNewCaledoniaandCamosunCollegewillbeimplementingtheirnewmedicalradiographyclassesin2012and2013.Anadditional12lowermainlandpositionswereaddedfor2013.

In-HouseMetaiodobenzylguanidineMIBG

Iodine-123-MIBGisusedtoimageneuroenndocrinetumourssuchasphaeochromocytoma,paragangliomaandneuroblastoma.ThisinitiativestartedinMarch2011toimplementtheinhouseproductionof~100dosesperyearatacostsavingsof~$1500perdose(thisdoesnotincludenetnewproductioncosts).For2011/2012,over$46,000insavingshavebeenrealizedthroughtheproductionofMIBGin-houseforintegratedmedicalimaginguse.ThemajorityofsavingswiththisinitiativearethroughBCChildren’sHospital,astheyarethelargestuserofMIBG.Anaddedvaluetothisproductcomparedtotheproductpreviouslyusedisthattherearefewerreactions/sideeffects.Inadditiontotheaccessandsafetybenefits,thequalityofMIBGscansareimprovedbybetterimagedistribution.

Overview

In2009,theAHRA(TheAssociationforMedicalImagingManagement)releasedtheresultsofastaffutilizationsurveythatincludeddatafrom328hospitalsacrossNorthAmerica.ThesehospitalsincludedrepresentativesfromeveryStateandProvince,withequaldistributionsofurban,suburban,andruralfacilitiesthathadcapacitiesrangingfrom<100to>500beds.

OneofthekeymetricsproducedbytheAHRAsurveywasthe“ProcedureVolumeByTechnologistFTE”ormoresimply:Exams/FTE.IntegratedMedicalImagingmadeitagoalforeverydepartmentateverysitetoreachthe75thpercentileforthismetric.Thismeantthateachdepartmentwouldstrivetobebetter(intermsofExams/FTE)than75%ofthefacilitiesthatrespondedintheAHRAsurvey.

Thereasonthatthe75thpercentilewaschosenasatargetis:

• Wewanttocontinuallyimproveaccessforourpatients.

• ProductivityintheUnitedStatesishistoricallylowerthaninCanada,sostrivingforalowerpercentilewouldhaveprovidedafalselydeflatedtarget,whichwouldhavebeenunfairtoourpatients.
