Vertical Transportation Equipment

Scope

This specification is for the annual code inspection of elevators for Florida State University The inspections shall be performed as required by Florida Statutes Chapter 399061 and shall conform to the ASME A172 Inspectors Manual Inspections shall be reported to the State Bureau of Elevator Safety using direct electronic submission The ASME Elevator Inspection checklist shall be utilized during the performance of the inspections

Qualifications

The Elevator Inspection Service is to have a minimum of three elevator inspectors residing within 30 miles from Florida State University to ensure that there will be an inspector readily available for accident investigation or elevator emergencies All inspectors shall hold national QEI certification a Florida Elevator Certificate of Competency and a Florida Certified Elevator Inspector number Copies of the current valid certificates listed above shall be submitted for all inspectors performing work specified herein

The principal or Elevator Inspector supervising the Elevator Inspection Service shall have a minimum of twenty (20) years elevator experience and ten (10) years of elevator code inspection experience All inspectors working on the covered equipment shall have a minimum of ten (10) years elevator experience and three (3) years of elevator code inspection experience

There shall be at least two employees qualified and capable of performing elevator consulting services as may be required These services are to include code research equipment evaluations maintenance audits specification writing and project management for repairmodernization work

Having employees with previous experience in code inspection for a jurisdictional authority membership in code writing organizations andor participation on national code writing committees would be a factor considered in awarding this work

All inspectors and supervisors shall be directly employed by the Elevator Inspection Service no subcontracting of any portion of this work will be permitted

The Elevator Inspection Service shall have and document their possession of a library which contains all editions of the A171 Safety Code for Elevators and Escalators necessary for the elevators to be inspected under this agreement including the 1971 1978 1981 1984 1987 1990 1993 1996 2000 and 2004 editions including all supplements and addenda for each edition and interpretation books 1 through 31 It shall also include copies of ASME A172 Inspectorrsquos Manual ASME A173 Elevator Code for Existing Elevators and Escalators ASME A175 Electrical Requirements for Elevators NFPA-70 - National Electrical Code NFPA-72 - National Fire Alarm Code NFPA-13 - National Sprinkler Code NFPA-101 - National Life Safety Code and the Florida Unified Building Code

Each inspector shall have in hisher possession the following specialty equipment

Hydraulic pressure gauge calibrated within the last 12 months Door pressure gauge Light meter Rope gauge for determining maximum reduction in wire rope diameter A copy of ldquoInspection Handbookrdquo published by Elevator World A copy of the ldquoElevator Industry Field Employeersquos Safety Handbookrdquo

Documentation

All inspections shall be reported to the Florida Bureau of Elevator Safety with a process for confirming delivery of the electronic submission of inspection reports to the Bureau office

The inspector shall install in the elevator machine space documentation of any tests witnessed listing the elevator serial number and whether the periodic test witnessed is a Category 1or Category 5 test

Schedule

2 A copy of the lsquoQEIrsquo Certification Card of each person(s) which will be performing the inspections and witnessing of each test shall be submitted for approval by the University Renewal Cards shall be submitted annually

RIGHTS OF TERMINATION Either party for any reason or without cause damages or penalty can cancel this bid contract with a 30 day written notice to the other However during the (30) day period after such notice is given the rights obligation and liability of each party to this agreement will remain full force and affect Such cancellation will end the obligations of both parties with regard to the purchase order

FAILURE OF VENDOR TO PERFORM In the event that the vendor violates any of the provisions of the agreement the University may serve written notice upon such vendor of its intention to terminate the agreement Such notice is to the state the reasons of such intention to terminate the agreement and unless within (10) ten days after serving such notice upon the vendor Such violation has ceased and satisfactory arrangements for correction is made the agreement will upon expiration of said ten (10) days cease and terminate But the liability of such vendor and his surety for any and all such violation(s) will not be affected by any such termination

TAX EXEMPTION Florida State University does not pay Federal Excise or State Sales Tax on direct purchase of tangible personal property

Scope

This specification is for the annual code inspection of elevators for Florida State University The inspections shall be performed as required by Florida Statutes Chapter 399061 and shall conform to the ASME A172 Inspectors Manual Inspections shall be reported to the State Bureau of Elevator Safety using direct electronic submission The ASME Elevator Inspection checklist shall be utilized during the performance of the inspections

Qualifications

The Elevator Inspection Service is to have a minimum of three elevator inspectors residing within 30 miles from Florida State University to ensure that there will be an inspector readily available for accident investigation or elevator emergencies All inspectors shall hold national QEI certification a Florida Elevator Certificate of Competency and a Florida Certified Elevator Inspector number Copies of the current valid certificates listed above shall be submitted for all inspectors performing work specified herein

The principal or Elevator Inspector supervising the Elevator Inspection Service shall have a minimum of twenty (20) years elevator experience and ten (10) years of elevator code inspection experience All inspectors working on the covered equipment shall have a minimum of ten (10) years elevator experience and three (3) years of elevator code inspection experience

There shall be at least two employees qualified and capable of performing elevator consulting services as may be required These services are to include code research equipment evaluations maintenance audits specification writing and project management for repairmodernization work

Having employees with previous experience in code inspection for a jurisdictional authority membership in code writing organizations andor participation on national code writing committees would be a factor considered in awarding this work

All inspectors and supervisors shall be directly employed by the Elevator Inspection Service no subcontracting of any portion of this work will be permitted

The Elevator Inspection Service shall have and document their possession of a library which contains all editions of the A171 Safety Code for Elevators and Escalators necessary for the elevators to be inspected under this agreement including the 1971 1978 1981 1984 1987 1990 1993 1996 2000 and 2004 editions including all supplements and addenda for each edition and interpretation books 1 through 31 It shall also include copies of ASME A172 Inspectorrsquos Manual ASME A173 Elevator Code for Existing Elevators and Escalators ASME A175 Electrical Requirements for Elevators NFPA-70 - National Electrical Code NFPA-72 - National Fire Alarm Code NFPA-13 - National Sprinkler Code NFPA-101 - National Life Safety Code and the Florida Unified Building Code

Each inspector shall have in hisher possession the following specialty equipment

Hydraulic pressure gauge calibrated within the last 12 months Door pressure gauge Light meter Rope gauge for determining maximum reduction in wire rope diameter A copy of ldquoInspection Handbookrdquo published by Elevator World A copy of the ldquoElevator Industry Field Employeersquos Safety Handbookrdquo

Documentation

All inspections shall be reported to the Florida Bureau of Elevator Safety with a process for confirming delivery of the electronic submission of inspection reports to the Bureau office

The inspector shall install in the elevator machine space documentation of any tests witnessed listing the elevator serial number and whether the periodic test witnessed is a Category 1or Category 5 test

Vertical Transportation Equipment

Scope

This specification is for the annual code inspection of elevators for Florida State University The inspections shall be performed as required by Florida Statutes Chapter 399061 and shall conform to the ASME A172 Inspectors Manual Inspections shall be reported to the State Bureau of Elevator Safety using direct electronic submission The ASME Elevator Inspection checklist shall be utilized during the performance of the inspections

Qualifications

The Elevator Inspection Service is to have a minimum of three elevator inspectors residing within 30 miles from Florida State University to ensure that there will be an inspector readily available for accident investigation or elevator emergencies All inspectors shall hold national QEI certification a Florida Elevator Certificate of Competency and a Florida Certified Elevator Inspector number Copies of the current valid certificates listed above shall be submitted for all inspectors performing work specified herein

The principal or Elevator Inspector supervising the Elevator Inspection Service shall have a minimum of twenty (20) years elevator experience and ten (10) years of elevator code inspection experience All inspectors working on the covered equipment shall have a minimum of ten (10) years elevator experience and three (3) years of elevator code inspection experience

There shall be at least two employees qualified and capable of performing elevator consulting services as may be required These services are to include code research equipment evaluations maintenance audits specification writing and project management for repairmodernization work

Having employees with previous experience in code inspection for a jurisdictional authority membership in code writing organizations andor participation on national code writing committees would be a factor considered in awarding this work

All inspectors and supervisors shall be directly employed by the Elevator Inspection Service no subcontracting of any portion of this work will be permitted

The Elevator Inspection Service shall have and document their possession of a library which contains all editions of the A171 Safety Code for Elevators and Escalators necessary for the elevators to be inspected under this agreement including the 1971 1978 1981 1984 1987 1990 1993 1996 2000 and 2004 editions including all supplements and addenda for each edition and interpretation books 1 through 31 It shall also include copies of ASME A172 Inspectorrsquos Manual ASME A173 Elevator Code for Existing Elevators and Escalators ASME A175 Electrical Requirements for Elevators NFPA-70 - National Electrical Code NFPA-72 - National Fire Alarm Code NFPA-13 - National Sprinkler Code NFPA-101 - National Life Safety Code and the Florida Unified Building Code

Each inspector shall have in hisher possession the following specialty equipment

Hydraulic pressure gauge calibrated within the last 12 months Door pressure gauge Light meter Rope gauge for determining maximum reduction in wire rope diameter A copy of ldquoInspection Handbookrdquo published by Elevator World A copy of the ldquoElevator Industry Field Employeersquos Safety Handbookrdquo

Documentation

All inspections shall be reported to the Florida Bureau of Elevator Safety with a process for confirming delivery of the electronic submission of inspection reports to the Bureau office

The inspector shall install in the elevator machine space documentation of any tests witnessed listing the elevator serial number and whether the periodic test witnessed is a Category 1or Category 5 test
