LTA (Lost Time Accident): below industry levels “0” Medical Aids (MA): “0” Reported Recordable Injuries to Regulatory Agency “#” HSE Awareness Index: 90% or greater Departmental HSE Scorecard: safety meetings, safety
inspections, recordable injuries & lost time accidents (Site performance with no deficiencies (i.e. green status) in all four categories)

per each census headcount per year (Example: 80)
GOAL FOR COMPANY: Approx. 160 observations

ENVIRO-ECO-EFFICIENCY PROJECT (E3) COMPLETION
The company should identify and carry out waste minimization and ecological conservation (E3) projects annually.

% SAFETY COMMITTEE PARTICIPATION:
-The company should have a safety committee team established;- Meeting facilitator should be one of the site’s leadership;- Team members - rotation is optional;-Current events, safety performance, corrective actions, initiatives, etc.. Are all discussed during these meetings; meeting minutes recorded;

# SAFETY AUDITS PERFORMED
The company should have audit zones identified where safety behaviors and/or compliance audits are performed monthly (e.g. Plant: process lines (extruder, injection, moulding, maintenance, compressor rooms, blending, mixing rooms, distribution, chemical storage, QC, etc..)

# 6 S HOUSEKEEPING AUDITS PERFORMED
Each department (or site) should have monthly housekeeping audits performed in each zone area.
HOW ?Answer: Integrating the 5 S program inspection to
the existing Safety Inspection.
