By signing up now you can save time, money and hassle should your phone be lost, stolen or accidently damaged. How do I Report a Claim?

Step 1 Call Chat Mobility Customer ServiceIf your handset was lost or stolen, call Chat Mobility Customer Service at (800) 944-5526 to suspend your service and protect yourself against unauthorized use of your account. If your handset was stolen, you may need to file a police report where the theft occurred. This report number may be required to process your claim.

Step 2 Call eSecuritelIn the case of accidental damage, loss, theft or malfunction including mechanical/electrical failure after the manufacturer’s warranty expires, we can provide simple and fast help. Call eSecuritel’s Customer Care Center at (888) 914-3478. Claims must be reported within 60 days of the incident and proof of loss and/or ownership must be provided, if requested. Only the account holder may file the claim.

Please have the following information ready:• Your wireless phone number• The account holder’s billing address• Wireless phone manufacturer/model and unique serial

number also called an ESN or IMEI

• Date, time, location and detailed description of the incident• Police report number (if theft occurred)

Step 3 Pay Deductible & Receive ReplacementIf your claim is approved, you will be directed to a participating Chat Mobility location to pick up your replacement handset within 30 days. A deductible will be collected based on the make and model of the handset ranging from $50 to $150 (please visit chatmobility.esecuritel.com for handset model specific information). Please keep the receipt, original box, manual and all packaging materials for warranty purposes.

How much will be added to my monthly bill?A monthly fee of $5.95 will be added to your monthly bill.

What are the claim limits?$2,000.00 per Named Insured or two (2) occurrences within a 12 month period, whichever comes first.

What is the deductible?Your deductible will be based on the make and model of the handset for which you make a claim, ranging from $50 to $150. Please consult the Deductible Schedule by visiting: chatmobility.esecuritel.com for handset model specific information. The deductible must be paid before you receive your replacement phone.

*Based on the non-contract, non-subsidized retail price paid at the point of purchase.**Savings based on placing only one approved claim (allotted two) in a 12-month period.

How do I enroll?You can enroll in the program at anytime you purchase a new handset from Chat Mobility. A fee of $5.95 will be added to your monthly bill.

When am I covered?To be eligible for coverage immediately, purchase the program at the same time you purchase your new handset from Chat Mobility (Point of Sale). If you elect to activate coverage after the Point of Sale, no claims may be made for thirty (30) days following activation. Please note, your phone must have been purchased from Chat Mobility and must be active and operational at the time of Program activation in order to enroll.

What’s covered? The program covers the handset (device, standard battery & wall charger) you purchased from Chat Mobility from accidental damage, loss, theft and malfunction (after the manufacturer’s warranty expires). Normal wear and tear and cosmetic damage to equipment is not covered.
