Dan Exam Application Instructions Prerequisites

Prior to taking the Dan examination, each student must have the following:

• Permission from your instructor • Proper documentation • Completed Application Form • Application Fee (see your instructor for details)

Documentation includes all of the following:

• Current membership card or JKA passport • Date current rank achieved • Certificate number of current Dan rank (if applicable) • Present Qualifications License (for JKA International Instructor/Judge/Examiner)

Completing the Dan Examination Application Form

• Print in all of the form fields. Be neat and legible. Alternatively, use Adobe Reader XI (version 11) or later to fill in the form on a computer. To save data that you type into the form, you must use Adobe reader XI or later. Earlier versions of Adobe Reader do not allow you to save changes. You can download the latest version from http://get.adobe.com/reader/.

• Enter the applicant’s name in ALL CAPITAL LETTERS. • Use the following syntax for applicant name and dates:

Name: LASTNAME, FIRSTNAME

Dates: YYYY MM DD

Example: 2009 12 12

• Karate Organization: JKA SHOTOKAN KARATE DO INTERNATIONAL

• : JKA Qualifications License Number. Leave blank if you do not have one. Do NOT enter your SKDI Membership number here.

• Height: Centimeters (height in inches x 2.54 = height in cm) • Weight: Kilograms (weight in lbs / 2.2 = weight in kg) • Reference: Students should use their Instructor for their reference • Leave blank any areas of the form you are unsure about • Fees are to be paid at the time of exam registration.
