TASKS CLARIFICATION:
Events of one’s life are explored through activities, listening, support, and conversation.
INTEGRATION: Relationships are identified and recognized
for their ongoing place in one’s life. Meaning is given to each important relationship.
ACTUALIZATION: Permanency in relationship(s) is visualized

KNOWN TRENDS
Ability to build relations. More ready to participate in therapy. Clinical social workers feel better equipped and
motivated to engage youth. Biological families active and willing to
participate in 3-5-7 Model© activities by providing history and filling in the gaps.
County is requesting this service for their youth. Youth are more willing to address feelings with
their family.
KNOWN TRENDS
Intensive Treatment Foster Care => Stable Placement => Permanency.
Stability and Improvement in CANS scores reflecting improvement in overall wellbeing.
Youth take a more active role in their case plan. Decreased altercations in school. Increase in foster parents ability to empathize,
understand, and address behaviors. Foster parents’ increased participation with goal
setting.
BARRIERS
Barriers
Length of client stay. Readiness of client Resources/funding. Community knowledge. Lack of history. Therapy versus therapeutic. Unable to truly have one CANS.
