Progress Towards Goal
Based on the 91 groups currently reporting data through the campaign portal:• Average prevalence rate: 27.3%• Average control rate: 69.7%* (increase of 1.2% since initial reporting period of Q1 2013 to
Q1 2015)• Number of patients with improved hypertension detection or control since start of the
campaign (Q1 2013 to Q1 2015): 447,764 (adjusted for seasonality)

Rooming Process for Nurses• We Use Standard Sphygmomanometers
• Patient to be Seated in a Chair with Both Feet on the Floor
• Not Talking While Blood Pressure is Taken
• If Initial Blood Pressure is High, Repeat after Patient Has Been Sitting Quietly for 5 minutes
• Nurses Have had In Service Education and BP Skills Checked Out by Nursing Supervisors and new Hires are Trained and Checked Out

• When a patient is seen and the patient’s blood pressure is > 140 systolic or > 90 diastolic a BPA (best practices advisory) fires

New Pilot Program – The Steps We Will be Taking
1. Patient Recalls for a Free Nurse Visit for a Blood Pressure Check for those whose blood pressure was elevated at the most recent visit
2. If well received by the Patients consider adding a dedicated nurse vital sign station to perform this function
3. Depending upon the number of patients that are found to have elevated blood pressure on the recheck, start Nurse managed protocols

New Pilot Program Patient Recall for Free Nurse Visit For a Blood Pressure Recheck
• Under age 65 systolic >140 or diastolic >90• 65 and older with a systolic >150 or diastolic >90• Reports run to identify those whose BP exceeded
the above parameters• MD to review list for those whose blood pressure
should not be lowered further• Patient contacted and instructed to take usual
medications when coming for their blood pressure recheck with a nurse

• 146-160 – patient will be appointed with MD within 3-4 weeks for further medication management and reassessment of BP management plan
• 160-179 – Patient will be scheduled to see MD within 1-2 weeks for further medication management and assessment of BP Management plan
• 180 and greater - Patient will be referred for same day assessment with primary care MD or if primary MD not available then first available MD with opening
