Accidents and Incidents

During June there was 1 accident and 1 incident reported:

1. Accident Details

Worker tripped over a pallet at the top of the compound in Alker lane, mentioned to supervisor and recorded for information– no injury was sustained by the worker.

Investigation

Pallets in area were behind a cordon, no obvious hazard when checked. The IP may have been distracted, however no injurywas sustained.

Recommendations

No recommendations required.

1. Incident Details

On June 17th, a member of the telecoms team working at Milner Royd Junction witnessed what he believed to be a near miss and reported it to the SRA Construction Manager. Babcock Civils Team had to move clear of an open line upon the approach of a passenger train. The Agency team were pushing a skate, containing plant / equipment, up the MVN2 down line and upon hearing the approaching train sounding its horn removed the skate and moved clear of the track. The train did not sound an emergency horn, or undertake an emergency brake and to date the driver has not reported the issue as a near miss or close call. The CCTV footage has been requested from the train. Upon receiving the report the Construction Manager immediately stood the team in question down and asked them to return to the Huddersfield depot. After assessing the incident and the initial evidence provided the SRA Construction Manager believed that the Telecoms installation team had accessed in the same manner. He then requested that the telecoms installation team, comprising of a COSS and Engineer, return to the Huddersfield depot for further witness statements and drug and alcohol testing. All results provided a negative alcohol reading, subsequent drugs test results were returned as negative.

Safety Related Reminders:

Since the last brief we have had more close calls raised on our staff for failing to sign in with SACS with adequate & correct PPE. Please ensure that when you attempt to sign in for site that you are wearing full orange PPE including cut level 5 gloves, safety glasses, cap lamps, safety boots & FFP3 mask (when required).

Please ensure that when you arrive on site that you have your sentinel card in order for the COSS & SAC to complete a Sentinel check, and that you sign IN & OUT with SAC.

Reminder of NWEP Site Safety Rules

1) Gloves and glasses MUST be worn by ALL2) Paperwork – all personnel must receive a task brief by the PIC and sign the paperwork3) SSOW – all personnel to be signed in and M/C’s to scan card to ensure all competencies are correct4) Exclusions zones – ensure that all M/C’s are enforcing the exclusion zone. The exclusion zone distance is not necessarily 10 metres and depends on works going on. It is there to protect persons from unplanned movements and things falling so in reality could be less or more than 10metres but the M/C should be able to identify the zone. If you are not sure what the exclusion zone is ASK5) Persons clipped in when working in the basket

Everyone has a personal responsibility to ensure compliance with basic site rules. It is mandatory that the correct access road to Farnworth Compound is utilised and under no circumstances is anyone to use Church Rd:

Update to Sentinel Scheme Rules

With the update to the Sentinel Scheme Rules in June we would like to remind every individual that you have a personal responsibility to comply with health & safety rules including, but not limited to:

1) No individual shall undertake or attempt to report for duty, if they have worked on NRMI within the preceeding 12 hours, unless a risk assessment has been conducted by the Primary Sponsor and suitable controls implemented

2) No individual shall exceed the maximum working hours determined by law or the, managed infrastructure requirements (or Sponsor) whichever is shortest

3) Every individual has a responsibility to be fit for work, not fatigued by excessive travel or work, and not under the influence of drugs or alcohol. Individuals shall report anything that may affect their ability to work safely including medication, lack of equipment or personal circumstances

4) An individual shall report for duty with the appropriate Personal Protective Equipment (PPE) to enable them to undertake their duties

5) No individual shall undertake a task for which they are not competent, do not have the right equipment, or the relevant information or the knowledge to complete safely

6) Individuals shall act in a safe manner at all times when on the managed infrastructure, and report any incident, close call or breach of the Sentinel Scheme Rules they are aware of

7) Individuals shall always carry their Sentinel card when on managed infrastructure and present their card for checking upon request

Arriving “Fit for Work”

Every individual has a responsibility to arrive ‘fit for work’ to complete their work in safe manner under the working conditions. It is your obligation to ensure that you are well rested prior to your shift, to not be under the influence of alcohol or drugs and to report any changes in personal or medical circumstances

which may affect your ability to complete your job safely. This includes reporting any over the counter or prescribed medication to your line manager to be checked through the chemist on-call system.

Sentinel app improvements. Release date: August 2018

What is happening?

Four improvements will be made to the Sentinel app/ MySentinel.

Why? In the last 12-months you have told us what we can do to make the Sentinel app/ MySentinel better for you and easier to use, we listened and we are making some improvements.

What are the improvements?

1. Swipe Out Reminders on Mobile Apps To provide card checkers with the ability to set swipe out reminders for themselves and/or their team members. This will initially be defaulted to 15 minutes prior to swipe out however, extra fields will be added to enable you to set your own reminders.

2. Surfacing Medical Expiry when checking a card The most recent medical date will be displayed underneath the medical restrictions section on the swipe in/check a card and show own card pages of the app and the PC Card reader.

3. Show reasons for 'No Core Competence' and update existing ATW denial reasons

In order to make it clearer why people are being denied access to site we will be changing some of the alerts to, Check Core Competence, Check Primary Sponsor, Check Medical and Check D & A. Alerts that will stay the same are, Cardholder Suspended, Card Cancelled, No Sponsor (in relation to a sub sponsor) and Pending Sponsor Risk Assessment.

4. Display 'Valid Until Date' in MySentinel This will provide visibility of the 'Valid Until date' for competences held by an individual in addition to the Expiry Date for each competence. This will help the individual identify when their competences will become invalid as this may be sooner than the Expiry Date, for example, post-mentorship or interim assessments are required to maintain the competence validity.

Environmental

Dust – Nuisance & Disturbance

Dust is commonly responsible for causing eye irritation and airborne dust presents serious risk for human health, alongside the impact it can have to the environment & society. It can have many negative impacts on agriculture, cause issues with irrigation (which affects river & stream water quality) and can cause reduction in visibility as well as causing a nuisance or disturbance to the local community.

This is why it is important when working on site to take into consideration your work activity and take into account the following mitigation measures:

1) Ensure you comply at all times with site speed restrictions to reduce the risk of dust clouds on roads or accesses

2) Wash or clean vehicles on site prior to leaving to reduce the dust exposure in the local communities in which we work

3) Spray water when cutting building materials takes place to minimise the dust 4) Ensure that when using cutters or saws that adequate equipment or techniques are adopted to reduce the

amount of dust exposure (this can include dust extractors or the use of bowsers)5) Ensure that dry materials are stored appropriately on site and that stock levels are maintained at an

appropriate level. Where materials are stored close to neighbours, ensure that control measures are in place to prevent the escape of materials

6) Always remove waste dusty materials from site once they are no longer of any use

Ballast Dust

When working with ballast ensure that the control measures in place to mitigate the dust emissions are adhered to; the movement of ballast can have the potential to cause serious health & environmental impacts. Processes to minimise the ballast dust should be considered & briefed to all those involved in the works.
