Since Mid January, has had hygiene issues. wears the same clothes for 2-3 days at a time. feet have an odor. struggles with self hygiene due to only having use of one arm. has a strong pungent odor when she has a menstrual cycle. hair is half braided and half unbraided. There are no negative implications based on signs of present danger. Mom has yet to be interviewed, but her live inparamour and house were seen and interviewed. She std that mom treats all the children the same, and never spanksthem. Home was free from hazards and it was clean. Adequate food was in the home as well as adequate sleepingarrangements. The incident did not happen in the home, it happened at the mgm's home. All the children std that it was anaccident, and mgm took the child to the hospital immediately and cared for the burns. All the children were observed to beclean and dressed appropriately. Mom's paramour and mgm serve as babysitters, so the children are always supervised.

V. Overall Safety AssessmentOverall risk is Intermediate to Low. All the children do not fear anyone in the home and care for their mom. Live in paramour stdthat mom is a good mom and that all the childrens needs are being met. She std that mom treats all the children the same, andnever spanks them. Home was free from hazards and it was clean. Adequate food was in the home as well as adequatesleeping arrangements. Paramour spoke approriately about the children and the mom. CPI interviewed the children and theystd that the incident did not happen in the home, it happened at the mgm's home. All the children std that it was an accident,and mgm took the child to the hospital immediately and cared for the burns. Mom has appropriate support w/ paramour andmgm. and the children are verbal and visible in theschool and community.

Overall risk is low. Long term safety plan is to monitor the children when they are w/ MGM, mom or friends. Mom is veryprotective of all the children, All std that the burn was an accident caused by her brother running w/ hot soup in his arm (he wascurrentlly in a cast w/ a broken arm). Mom's safety plan: "My kids are being watched; they keep clean clothes, they showerevery night. clothes fit her, she just has one arm that she can use and when I button her pants at home, she pops herbuttons at school when she goes to the restroom b/c she can't unbutton them herself. My mother doesn't keep them amy moreafter school. My friend Anna Coonors do".

Dr. Issac has denied that the burns were consistant w/ abuse. They are futher reviewing medical records and if the CPT thinksthe case needs to be recalled, CPS will open the case again immediately. The case is currenty being closed as No Indicators.

VI. Summary/Findings ImplicationsAll denied allegations; Dr. Issac did not find the burn to be consistant w/ abuse. Home was very clean, adequate sleepingarrangements, food and all the children were b/m/w free (exception for injuries from the accidental burns). cpt isreviewing med records to ensure mom was compliant with treatment, if an issue arises another rpt will be called in

There are currently no known dangers for child safety based on the following. The mother stated that she was attacked byher friend but she never spoke to the police about the matter. The mother said that was not living in the home and she is going to get an injunction against her to not return. She stated that they were in the room arguing and she asked her to leave. She stated that was refusing so she tried to call the police and took the phone and swung on her hitting her. She stated that she was able to get the phone away from her and she was able to push her out the door. She stated that she did not hit back. She stated that the children could have heard it but did not see anything because they were in their rooms.

UPDATE: Attempts have been made to see but have been unsuccessful. The mother stated that attacked her. According to the police report, the mother attacked Keishanna stated that she has not seen or heard from since the incident and she does not thinkt hat they will continue to be friends. Mom stated that she has not filed for the injunction because she does not qualify based on the report. She stated that she will not allow to return to the house. No further updates.

Mom signed a safety plan, ICWA Denied, R&R and a community resource pamphlet were received.

UPDATE: The mother said that she was a victim and according to the police report is the victim. The mother cannot get an injunction unless she pays because she is not a vicitm. has not been located. Attempts have been made at her residence and phone messages have been left with no return phone call. The mother said that will not be allowed back in the house. The children's basic needs are being met and there are no hazards in the house. No further updates.

V. Overall Safety AssessmentCPI and LE went to the complex managers office and met with the victim. The child stated that his mom spanked him with anextension cord. The child was seen with two welts on his arm and several more on his butt. The mom attempted to report thechild as a runaway. CPI and LE met with the mother who at first denied that she spanked the child. She stated that the welts onhis arm may be from her grabbing him to keep him from running out of the house. Det. Maltreatment/Nature of Maltreatment (Surrounding Circumstances): On 09/24/15, CPS received an immediate callout where the child, had run away from his home after the mother, Keishanna, hit his head into the mirror in her bedroom. had stopped at Walmart on his way home where he stole a few pieces of candy. The mother works at the same Walmart, so they called her instead of calling the police. Keishanna reportedly picked up from Walmart and stopped her car somewhere in the parking lot where alleges his mother hit him repeatedly. The mother then continued home where she took out of the car by the shirt collar. She walked him inside where and the paramour, Trayvon, already were inside. Keishanna walked into her bedroom where it is alleged she pushed him into the mirror. CPS saw a small cut on his head and what appeared to be a dark mark on his head. The paramour sent too his room, and he stopped

the mother from pursuing the child further. then opened his bedroom window, and he ran down to the clubhouse to get help. Susan, the clubhouse manager, has seen this child with similar allegations and injuries before within the last year. The mother denied the allegations and blames the child for his injuries.

The mother stated she wanted out of the home, but she did not have any family in the area that could take him. CPS made arrangements for to go to the Family Resources shelter for the night, but once there, the mother refused to sign the consent forms. When CPS returned to the home, there was loud, violent yelling coming from the families apartment. The fighting could be heard from the parking lot while the parents were behind closed doors in the apartment. The mother denied any fighting or yelling taking place even after being confronted about it by CPS and LE. The mother refused to allow CPS to interview the children, but CPS was able to interview them at school. They both love their mother, but they sometimes feel unsafe in the home when the mother and the paramour are fighting. They are used to sitting in their bedroom with the lights off listening to the adults fight. When they are done fighting, everyone just laughs and everything goes back to normal and they go to sleep.

There has been ongoing department involvement with Keishanna and her family dating back to 2000. There have been several reports of alleging physical injury and family violence with one verified report in 2004. The mother is consistently uncooperative with CPS, and she does not follow through with services when they have been offered and put in place. The children have become increasingly fearful of their mother and frequently feel unsafe in their home environment. Admittedly, the mother is sometimes overwhelmed with being a single mother to five children, and she is currently pregnant. does act out and has become more rebellious, however the mother incorrectly handles disciplining him. All of the children have become normalized to the mother and the paramour fighting, and they speak in very quiet tones like they are afraid to speak too loudly.

Summary of all priors: The mother has been involved with the Department of Children and Families for physical injury and family violence threatens child ongoing since 2003. All of the cases have been with Manatee County. There was a verified physical injury with the child's older sister, , being sheltered from the mother in 2003. Since the mother has been involved with the Department of Children and Families, there have been six cases alleging physical injury to the children in the home, and there have been two cases of family violence threatens child with the mother being the alleged perpetrator in both.

Analysis: There are many concerns for the behaviors and violence in the home as to the mother, Keishanna. She does not take responsibility for anything that happens, and she always blames for the issues in the home. Keishanna was unwilling to speak with CPS and refused to allow her daughters to speak with CPS. The mother was seen to be quick tempered and very combative with both CPS and LE. It seems the mother would benefit from counseling and anger management, however she denied needing any help. If the mother does not engage with any services herself, it seems unlikely anything will change in their household or family dynamics.

Safety Analysis SummaryThe children have been determined unsafe due to the danger threat presented by the mother due to her inability to properlydiscipline her children, control her violent tendencies, or cooperate with investigation. The mother has a fifteen year historywith the department with several reports of physical injury and family violence. The mother does not take responsibility for anyof the issues or violent behaviors even when being confronted by CPS and LE, but she instead blames one child, Thechildren in the home have become normalized to the mother and the paramour fighting, but they still become easily upsetwhen asked about their mother and her paramour. When confronted about services to help her manage and better herhousehold, the mother denied needing any help and instead stated was the only one that had any issues or needed

Her mother Keishanna Thomas refuses to provide any information regarding her current

whereabouts. Ms. Thomas has been taken into custody until she provides information on her

daughter's whereabouts as Judge Brownell with the Manatee County Court had previously
