Enter this year’s golf tournament on the 71-par Hurdzan-Fry course at the Keystone Resort and Conference Center. With magnificent views of snow-capped peaks and wildlife, it’s easy to see this is a golfing experience unlike any other. The par-35 front nine is oriented around the path of the Snake River and the par-36 back nine winds through a lodgepole pine forest. Impressive elevation changes, variable

bunkers, water hazards, and five sets of tees on each hole combine to challenge golfers of all abilities. The 16th hole features a 194-foot elevation drop from tee to green and the scenic 18th hole offers 12 bunkers and spectacular views of Lake Dillon. The fee of $125 includes greens and cart fees for a 10:00 am shotgun start on Wednesday.

ExHIb ItoR RAtESCorporate Exhibitor: Each corporate exhibitor will receive a 10’ x 10’ booth. For membership information go to www.physicianhospitals.org. All contracts must be accompanied with payment. There is a $300 up charge for a corner booth on the Silver and bronze Corporate Membership level if space is available. If you wish to upgrade your booth size, it is $2000 per 10’ x 10’ booth for Silver and bronze Corporate Members; $1000 per 10’ x 10’ booth for Gold and Platinum Corporate Members if space is available.

Cancellation Policy: All booth and sponsorship cancelations must be made in writing to PHA on or before July 25, 2011. All cancellation requests received before July 25, 2011 are eligible for a 50% refund of the amount that has been paid. There will be no refund given for any cancellation request received after July 25, 2011.
