Artwork must be based on or inspired by this year’s featured book. Please print clearly. Complete the “Agreement & Registration” form in full for each work of art submitted and securely attach to artwork(s).

I agree to display my work(s) in the White Salmon Valley Community Library during the dates indicated. I understand that the work(s) will be displayed in an unlocked and unattended room and that the library does not assume responsibility or liability for materials exhibited in library facilities. I understand that location and placement of work(s) accepted for the exhibition are at the discretion of the Art Exhibition Committee, and that removal of work(s) prior to the ending date will result in ineligibility for future exhibitions. I also understand that prior to the opening of the exhibition, as artworks are submitted, they will be stored in an unlocked, unattended room; and that I am responsible for removing my work(s) promptly after the exhibition, or at another time by arrangement.
