King-Devick Test Online System (KDTOS) : Quick Start & Tips

The King-Devick Test Online System (KDTOS) allows for storage of athletes’ K-D Baselines, post-injury screening and monitoring assessments.

Login to KDTOS via the web at: www.kingdevicktestapp.com

The King-Devick Test has been validated for a standardized test size therefore calibration of the display is required before use. See Calibrate your screen.

KDTOS allows you to organize your athletes by team. Quickly and easily import subject details and team rosters to your KDTOS account. See Import Data.

Use the King-Devick Test in association with Mayo Clinic companion tablet app (compatible with 10.1” Android and 9.7” iPad) for testing or when internet is not available. The app synchronizes with your KDTOS account when internet access is available. Visit: www.kingdevicktest.com/tutorials for instructions and tutorials for the King-Devick Test in association with Mayo Clinic tablet app.

Team Details Page The Team Details Page displays the team details of the selected Team from the left pane Teams list.

View All Unassigned Subjects Click on "All Unassigned" to view subjects that are not currently assigned to any teams. As the KDTOS account Administrator you can assign these subjects to teams. See Assigning a Subject to a Team.

View All Subjects Click on "All Subjects" to view All Subjects in your KDTOS account.

Search Team Search for a Team by name by simply typing a Team Name in the search team field.

Print List Click on "Print List" to print (or export as a spreadsheet or PDF) the team list to quickly view the numbers of subjects on a team, subjects that need baselines, subjects that have exceeded their baseline.

Add a Team The K-D Test App allows you to add teams to help organize subjects. This function is available to KDTOS account Administrators. See Adding a Team.

My Account Click on "My Account" to view your KDTOS Administrator account details. Here you can Edit Your Account, Add/Edit Users, and access Maintenance of your KDTOS Account.

Scoresheets Click on "Scoresheets" to download or email scoresheets.

Calibrate Before King-Devick Test administration, the computer screen requires calibration to standardize the test card size. Click on "Calibration" and use a ruler or standard letter page (8.5 x 11 inches) to calibrate your display.

Log Out Securely log out of your KDTOS account.

View Team Click on "View Team" to view all the subjects in the selected Team.

Edit Team Click on "Edit Team" to make changes to the team name, add or remove subjects from the team, or delete the

team.

Change Image Click on "Change Image" to display a team image (i.e. team logo, etc.)

Subject Details Page The Subject Details Page displays the subject details of the selected subject from the left pane subject list.

Print Details Click on "Print Details" to print (or export as a spreadsheet or PDF) all Subject Details of the selected subject:

Baseline, Concussion History, Teams, Post-Injury Test History.

Edit Subject Edit a subject's details such as Name, Subject ID, Date of Birth or Delete the Subject.

Change Image Click on "Change Image" to display a subject image (i.e. athlete head shot, etc.)

Search Subject Search for a Subject by name by simply typing a Subject Name in the search Subject field. Alternatively, check “Use Subject ID” to display the Subject IDs or check “Sort by Status” to sort athletes by status: Baseline determined, Baseline Needed, or Failed Status.

Add Subject Add additional subjects to your KDTOS and assign them to a team. See Adding a Subject.

Print List Click on "Print List" to print the current subject list or team to quickly view each subject's details: Date of Birth, Status, Baseline, when the baseline is valid until and the number of test cards used in baseline testing.

View Teams Click on "Teams" to view Teams that have been created in your KDTOS account. See Adding a Team.

My Account Click on "My Account" to view your KDTOS Administrator account details. Here you can Edit Your Account, Add/Edit Users, and access Maintenance of your KDTOS Account.

What is the Test Set? There are 3 test sets available through KDTOS. Test sets are the same King-Devick task with different number sets to minimize memorization. As the test administrator, be sure to have the corresponding Test Set Scoresheet to follow along for errors.

What is a Post-Injury Test & What is a Baseline Test? A Post-Injury Test is any test administration following suspected head trauma to screen for injury and/or following confirmed head trauma to monitor recovery. See Administering a Post-Injury Test. A Baseline Test is a pre-injury baseline test score determined by following the Baseline Testing protocol which establishes a baseline test score as the faster of two error free trials. See Administering a Baseline Test.

Check Baseline Test Check Baseline Test is available for any subjects who have previously established a Baseline. See Administering a Baseline Test.

Recording a Test Allows you to add prior test results into your KDTOS account. See Recording a Test Result.

Subject Details Tabs The Subject Details Tabs allow you to toggle between displays of the selected Subject's details.

Post-Injury Test History displays details of all Post-Injury Tests completed. Concussion History displays the subject's Concussion History. Add Concussion History by tapping on this

tab, and then tap "Add…" You may include the date of any previous concussive injuries and comments. Tap "Continue" to save the result.

Teams tab displays all the teams that the subject has been added to. See Assigning a Subject to a Team. Baseline Test History displays details of all Baseline Test trials completed.

Log into KDTOS 1. Access the King-Devick Test Online System (KDTOS) via the web at: www.kingdevicktestapp.com. Enter your

KDTOS User name and Password to access your KDTOS account.

Forgotten Password? If you’ve forgotten your password, you may reset it by clicking on “Forgot your Password?” Enter your KDTOS

account User name email. Click “Send Email” and you will receive a time sensitive link to reset your password.

Calibrate Your Screen The King-Devick Test has been validated for a standardized test size therefore calibration of the display is required

before use. You can choose to calibrate your screen with either a ruler or a Letter sized (8.5” x 11”) sheet of paper.

1. Select to “Use Ruler” or “Use Letter Page”.

2. Hold up a ruler or letter page to your computer screen to match the display on the left corner.

3. Drag the displayed ruler or letter page image from the right to match the size of your ruler or letter page.

4. When the horizontal distance of the letter image matches 5 inches on the ruler image matches 5 inches on your

ruler or matches your letter page size, click “Done” to save.

Add New User As the administrator of your KDTOS account, you may add Users to your KDTOS account with separate logins and full or limited visibility of subjects by team. Edit User login details and team access here.

1. In My Account, click “Add New User”. 2. Enter the User details: Name, Login (preferably an email), Telephone number (if necessary) and password. To

allow the User full access to ALL subjects, check the box “Can view all subjects”. Click “Save & Exit”.

Edit User Details As the administrator of your KDTOS account, you may add Users to your KDTOS account with separate logins and full or limited visibility of subjects by team. Edit User login details and team access here. See Add New User.

1. In My Account, select the User from the left pane User list.

2. Click “Edit Details”.

3. Make any necessary changes to the User details.

o Check the box “Can View all Subjects” to give the User access to all subjects.

o Check the box “Suggest Team Selection...” to ensure that new subjects added via tablet are assigned to a

team and visible to the User.

Change User Password As the account KDTOS administrator you may change User passwords. Prior to changing the password be sure to synchronize all tablets associated with that login to ensure that no offline data is lost.

1. In My Account, select the User from the left pane User list.

2. Click “Change Password”.

3. Prior to changing the password be sure to synchronize all tablets associated with that login to ensure that no

offline data is lost. A list of tablets associated with the selected User and the last date that those tablets were

synchronized is displayed.

4. If all tablets have been recently synchronized, proceed with changing the password. Enter the new password in

the corresponding fields. Click “Save & Exit”.

5. All tablets associated with that User log in account will need to log out and log in with the newly established

password to continue to synchronize correctly.

Edit User Team Access Control visibility of Users to one or all teams.

1. In My Account, select the User from the left User list. 2. To limit Users’ visibility by team, click “Team Access”. 3. Select which teams to allow User access to. Then click “Save & Exit”.

Import Data Quickly and easily import subject details and team rosters into your KDTOS account.

1. Under My Account, click “Maintenance” then “Import” 2. Use the sample CSV file available in the instructions to organize your subjects and team rosters. 3. Follow the data import process instructions.

Export Data Export Data allows you to quickly download and export your current account data.

Click “Export Details” to view a summary report and individual subject detail reports. Click “Export Single Sheet” to view a single sheet summary of individual subjects organized by row and test administration.

The summary will automatically begin downloading as an Excel file.

Bulk Delete Bulk Delete allows you to quickly and easily remove subjects you no longer want to include in your account (i.e. student-

athletes that have graduated). Click the column headers to sort by column, drag columns to the white area to group by

columns. Select the subjects to be deleted. Highlight multiple by using SHIFT or CTRL key, then click “Delete Subjects”.

Renew Your Account If your KDTOS account is expired or expiring soon, you may renew your account within KDTOS.

1. In My Account, under Maintenance, click “Renewals” to renew your account. 2. Adjust the subject limit for your account as needed. Make note of your KDTOS Account Administrator email

associated with the account. You’ll use this upon checkout. 3. Click “Order” and proceed to Check Out. Enter your Billing Details and King-Devick Account Administrator Set-up

Information. *Be sure to use the same KDTOS Account Administrator email for account renewal.

Increase Subject Limit If your KDTOS account is still active and not expired however you simply would like to increase your subject limit, you may do so within KDTOS.

1. In My Account, under Maintenance, click “Renewals” to renew your account. 2. Click on the link at the bottom of the Renewals page to increase your subject limit:

and enter contact email(s) for the subject.

As a User Account Add a New Subject’s details to your account. This function is available to KDTOS account Administrators and Users with access to teams and/or all subjects.

1. From the Team Details page, select the team that you would like to add a subject to. Tap "View Team". 2. Tap "Add Subject" to add a subject to the selected team. 3. Enter the subject first name, last name, subject ID, date of birth, check if the subject wears glasses or contacts

and enter a contact email for the subject. The new subject will be assigned to the selected team. TROUBLESHOOTING:

When logged in as a User, if there is no "Add" button available please contact your account Administrator. Users must have access to teams and/or all subjects to be able to add teams and subjects.

1. From the Subject Details page, select the subject that you would like to assign to a team. 2. Click on the “Teams” tab of the Subject Details. 3. Click “Edit Teams”. 4. Select the team(s) that you’d like to add the subject to. 5. Click “Save Changes” when complete.

Administering a Baseline Test KDTOS allows you to administer a Baseline Test directly on a calibrated computer. Be sure to calibrate your screen before testing. A Baseline Test is a pre-injury baseline test score determined by following the Baseline Testing protocol which establishes a baseline test score as the faster of two error free trials.

1. Baseline testing is available for new subjects that need a baseline. Select the subject from the subject list on the left pane that you would like to administer a Baseline Test to. The subject's profile will appear in the subject details on the right. Select the Test Set by clicking on the "Test Set" number. Tap "Start Baseline Test".

What is a Test Set?

There are 3 Test Sets available through KDTOS. Test Sets are the same King-Devick task with different number sets to minimize memorization. As the test administrator be sure to have the corresponding Test Set Scoresheet to follow along for errors.

Check Baseline Test is available for any subjects who have previously established a Baseline. For these subjects, select the test set number. Then click "Check Baseline Test".

2. Read the Instructions for Use carefully. The test administrator should have the corresponding Test Set Scoresheet ready to record any errors. Email Scoresheets to yourself by clicking on the link in the instructions or view Scoresheets online at https://www.kingdevicktestapp.com/phone. Tap "Start Test" to begin the test.

3. The Demonstration Card is displayed and no active, automatic timing is occurring. The test administrator should explain the task to the subject. When both the test administrator and subject are ready to begin, the test administrator should click anywhere on the screen to control the automatic timing and advance to Test Card 1. The subject should immediately begin reading aloud the numbers and the test administrator should follow along on their Scoresheet to mark any errors.

4. When the subject calls out the last number on Test Card 1, the test administrator should click on the screen to pause timing. A break card is displayed and no active, automatic timing is occurring. When both the test

administrator and subject are ready to continue, the test administrator should click anywhere on the screen to control the automatic timing and advance to Test Card 2. Repeat this for Test Card 3.

5. On the Test Results page, enter the Number of Errors, the Tester's Initials and any comments. Click "Continue" to save the result or tap "Cancel" to cancel and discard this result.

6. Baseline testing requires two error free test trials. Follow the on screen prompts to complete Baseline testing. Click “Continue” to administer additional Baseline trials.

7. Click on "Start Test". Repeat testing through Test Cards 1, 2 and 3. 8. On the Test Results page, enter the Number of Errors, the Tester's Initials and any comments. Tap "Continue" to

save the result or tap "Cancel" to cancel and discard this result. 9. When two acceptable baseline trials have been administered, you will receive a notice that the Baseline has

been determined. 10. The Baseline test result is now established and displayed in the subject’s profile with the expiration date.

Baseline trials are saved in the subject's Baseline History tab.

TROUBLESHOOTING:

Baseline testing requires two test administrations. If you've run one baseline test but the subject still has a YELLOW indicator and states "Needs Baseline", please administer another baseline test until you reach the results page that displays "Baseline Determined. Thank you."

Administering a Post-Injury Test KDTOS allows you to administer a Post-Injury Test directly on a calibrated computer. Be sure to calibrate your screen before testing. A Post-Injury Test is any test administration following suspected head trauma to screen for injury and/or following confirmed head trauma to monitor recovery.

1. Select the subject from the subject list on the left pane that you would like to administer a Post-Injury Test to. The subject's profile will appear in the subject details on the right. Select the Test Set by clicking on the "Test Set" number.

7. The test result is now saved in the subject's Post-Injury Test History tab.

Record Test: Adding a Test Result KDTOS allows you to manually enter and save previous test results.

1. Select the subject from the subject list on the left column that you would like to add a test result. The subject's profile will appear in the subject details on the right. Click "Record Test".

2. In the Record Result page, select which type of test you are recording.

What is a Post-Injury Test? A Post-Injury Test is any test administration following suspected head trauma to screen for injury and/or following confirmed head trauma to monitor recovery.

3. Record the total Time, Errors and Number of Test Cards used during testing. Enter the Tester's Initials and the date of the test. In the comments area you may add observations about the suspected injury (i.e. how the injury was sustained, acute signs and symptoms exhibited, etc.). Click "Continue" to save this result.

What is a Baseline Test? A Baseline Test is a pre-injury baseline test score determined by following the Baseline Testing protocol which establishes a baseline test score as the faster of two error free trials.

4. Enter in the final Baseline Time (in seconds), Number of Test Cards used, Tester's Initials, date of the test and any additional comments. Click "Continue" to save this result.
