Sopchoppy Education Center, Sopchoppy, FL 32358. Co-developed and implemented mental health services program for underserved youth in grades 6 – 12 and GED students. Program specifically addressed the unique mental health and behavioral needs of involuntarily enrolled youth, most of whom were mandated to the “second chance” academic setting following county expulsions and/or juvenile justice recommendations. Most youth were supervised by juvenile justice and attended the rurally located school following involvement with the court system. The counseling services program provided Individual and Group counseling services incorporating CBT, Art Therapy, Sand Tray, and Music Therapy methodologies in the rural homogenous community. SEC is an academic program of the Wakulla County School District and the counseling program was facilitated under the guise of the Florida State University Multidisciplinary Center. Simultaneously provided student intern Task Supervision. Dates: August, 2005 – May, 2006.

Co-developed database management system for gathering, tracking, and reporting data on 4,000 clients per year (using Access) and developed numerous grant specific reports, tables, queries and forms (see ECHO under Employment). This program improvement work require a developing and comprehensive working knowledge of MS Office Programs.

Developed, implemented and codified program evaluation practices into the School Counseling Services Program(s) onsite services delivery with outcomes spanning multiple programs, multiple years, and various providers. School Counseling program evaluations are rare in the US. The program evaluation mechanism was modified after the 2013 year, limited to data gathering beginning in the 2014 academic year (due to three main factors: prior efficacy findings, incremental funding limitations and non-funded report production time constraints). It is believed to be one of the few school counseling programs in the US with evaluation data for multiple years in multiple schools/districts, under varying administrations and providers.

Codified the financial assistance guidelines for a non-profit agency (501C3), as well as created a comprehensive policy and procedure manual standardizing regulations and procedures for client service provisions and personnel training guidelines. Applied for funding, and was awarded funding, from various grant authorities (Private, City and County) and managed the disbursement of various funds (in excess of

CONTINUING EDUCATION TRANSCRIPT

Licensing requirements by the District of Columbia Department of Health mandate security clearance verification at each biennial, including criminal background checks that cover licensee history from all juvenile and adult records/reporting. The DC DOH Board of Clinical Social Work does not (as of 2018) utilize an electronic reporting and verification mechanism for continuing education requirements and records storage/retrieval. All board audits of providers require the production of CEU compliance and certificates in accordance with DC DOH CEU standards, for each biennial where LICSW credentialing occurs. Licensing biennial CEU requirements are in the State of Florida via electronic reporting and verification mechanisms. CEBroker (the electronic reporting and verification mechanism utilized by the DOH licensing board in FL) requires a paid subscription by providers to record and post all CEU’s. In FL it is possible that a non-required CEU will not be logged with the broker gateway without the paid subscription. CEBroker maintains a non-subscription data base of CEU’s, containing only the required board approved courses. In FL a provider is limited in making sure the CEU transcript via CEbroker is entirely accurate due to the subscription limitation and requirement. It is, therefore, possible for a provider to have completed required CEU’s and for those to appear in the CEBroker portal and for a provider who does NOT subscribe to have additional CEUs that DO NOT APPEAR. All training, in addition to other educational coursework for LaRose is listed below involving CEU requirements in both jurisdictions. CEU’s recorded here are those taken, with the dates of completion, and how obtained; coursework in CEU’s often exceed the minimum requirements of licensing as a matter of improved patient care and as a matter of exceeding licensing standards. All CEU’s recorded here are documented with certificates of completion, course syllabi (when provided during presentations), presenter information (with license/educational titles), location, and number of approved hours. The respective records are maintained both digitally and traditionally in paper formats by the office of LaRose. All documentation can be provided upon request, and where required by licensing boards is done so upon demand. All courses, at the time of completion, were board approved, to meet the licensing requirement for continuing education training. It is possible for board approved CEU providers to offer other courses that are “non-board required.” In certain scenarios, it is possible for one CEU approved provider, to be approved in both jurisdictions, thus the coursework may meet the CEU requirements simultaneously. Because both jurisdictions have similar and different CEU requirements it is necessary to have added coursework (even in certain scenarios where the coursework may overlap). CE Transcript Reporting Errors: Where errors in either the CEBroker transcript exist or where the CE provider reported differently than the certificates indicate, contrary to the course work or hours
