LABORATORY C-14 COVERAGE PLAN EH&S is committed to ensuring safety and compliance during the COVID-19 pandemic. In accordance with New York City Fire Code, C-14 Certificate of Fitness coverage is required at all times when laboratories are in operation. While a C-14 holder is typically required in each laboratory, in consideration of physical distancing requirements, EH&S is extending the option – as allowed under FDNY regulations – for one C-14 holder to provide coverage for two laboratories on the same floor. To ensure adequate C-14 coverage for both research groups, laboratories now have the option to enter into a temporary, formalized agreement with one other laboratory on the same floor of the same building by using this form. By signing below, the PIs as well as their valid C-14 holders agree to the FDNY and EH&S guidelines and requirements, as follows: At least one valid, current C-14 holder from the signatories below must be present on the floor whenever a laboratory is in operation (this applies whenever personnel are physically in the lab, regardless of work being performed, including “break” periods). The C-14 holders must stay on the floor for their respective shifts, in accordance with the schedule on the next page. This agreement is building-specific. The two laboratories must be on the same floor of the same building. Note: For research groups who need coverage for additional floors, please submit separate forms. The PIs and C-14 holders assume responsibility for the hazards present in both laboratories. This requires the two laboratories to share their current LATCHs with all the C-14 holders. Note: For some groups, the assumption of responsibility may not be possible due to potentially unfamiliar hazards. This agreement will become void when the Columbia University research community is permitted to host a personnel density level that allows for laboratory-specific coverage. EH&S will notify enrolled research groups of the effective termination date of the agreement. C-14 Holders: Please provide your UNI below for any shifts that you will be covering. Ensure that this schedule meets social distancing requirements as well as any established shifts set by your PI for your laboratory. Please provide an ‘X’ for any timeframes in which both laboratories will be inactive.

In the example below, both laboratories would be covered by a researcher (UNI AA5145) on Mondays, Wednesdays, and Fridays from 9am to 5pm. The two laboratories would be covered by another researcher (UNI BB4321) on Tuesday and from 9am to 5pm and Thursday and Saturday from 9am to 1pm. The two laboratories would not be covered – and therefore closed with no research activity – on Thursday from 1pm to 5pm and on Saturdays from 1pm to 5pm.
