Dear Selection Committee Members:
We would like to thank you for this opportunity to express interest in providing professional
services for the Oklahoma Department of Transportation. We are pleased to submit the attached
Statement of Qualifications and CAP 255. In these pages, you will find that CGA Engineers
meets the pre-qualification provide county engineering.
Our current staff has the qualifications, capacity and experience to provide the requested
engineering and consulting services for County Engineering projects. Our professional staff has
previous experience with county, municipal and state agencies. We have recently completed
numerous transportation projects, many of which have brief descriptions included in this
submittal. CGA staff does not only provide design of projects but also provide services during
construction. This experience insures our designs are constructed under state/county standard
construction practices. Additionally, all of our engineers have experience in working on
Oklahoma Department of Transportation projects, county projects and municipal projects.
CGA Engineers understands that a project is more than design plans and specs on paper. Much of
our design work involves streets and roadways, projects that bring their own challenges such as
maintaining traffic flow during construction and working with limited rights of way. With this
kind of projects, CGA works with other entities such as utility providers and permitting agencies
to assist in developing a successful project. Whatever services may be required to see a project to
completion, you can rest assured that we will be able to provide them.

EXPERIENCE AND FAMILIARITY WITH DEPARTMENT PROCEDURES
CGA Engineers, Inc. has extensive experience in transportation design for both urban and rural
highways, including grading, drainage and surfacing. All our project engineers have experience
in working with the Oklahoma Department of Transportation (ODOT) in either a consulting or
sub consulting basis. The engineers in our transportation section have also worked for ODOT
and other National Transportation and highway departments.
Currently as DBE subcontractors, CGA Engineers has provided both survey and design services
on Department projects for other firms. This has given CGA Engineers insight and familiarity
with current Department policies and procedures in contracting, planning, design, and
preparation of construction plans and specifications.
ORGANIZATION
Mr. Dwain Garner, P.E., will head up the project development. His responsibilities will include
design team task assignments, schedule, and budget oversight, along with technical expertise. In
this capacity, he will be directly responsible for the project development and function as the
project manager.
Mr. Gary Gill, PLS, will coordinate all field investigations. He will head up the survey team
responsible for collecting survey data if needed for design. Additionally, Mr. Gill will be
responsible for right of way documentation which will include investigating the existing right of
way and easements on the projects. Having our own in-house survey crew offers many
advantages. These advantages include flexibility in scheduling and customization of our field
data collection methods. Survey schedules are coordinated with our design schedules.
Furthermore, as soon as our surveys are downloaded, our engineers an begin the design work.
This is because we have set up field codes, cell libraries, and symbol libraries to match our
design standards as well as ODOT standards.
The rest of the engineering and technical staff will assist Mr. Garner in the design and plan
production for projects. Following the organization chart, we have included resumes for the
professional staff. From the experience listed in these resumes, we hope you will see that we
have the technical and project management experience to deliver the preconstruction deliverables
for a successful project.

PRESIDENT
In 2005, Mr. Garner took ownership of CGA Engineers, Inc. With more
than twenty years experience, primarily in the areas of transportation,
municipal, and airport engineering for municipalities across northeastern
Oklahoma. Dwain has been involved in projects for the Oklahoma,
Kansas, Alabama and Mississippi Departments of Transportation and the
Arkansas Highway and Transportation Department. Mr. Garner has been
expanding his resume during the past several years to include land
development projects, primarily for our Native American clients.
EXPERIENCE CGA Engineers, Inc. - President – Responsibilities include managing all aspects of the firm, from office
administration to marketing to project management
City of Tulsa - Professional Design Engineer - Management of transportation infrastructure and
rehabilitation projects. His responsibilities included coordination on consultant designs, budgets,
schedules, construction management, and miscellaneous in-house design projects.
Garver Engineers - Office/Project Manager - Responsible for marketing, project management and
general office administration while being manager of eleven employees in the Tulsa office.
Holloway, Updike and Bellen, Inc. - Design Engineer – Was responsible for the design of streets,
highways, and general civil projects.
Oklahoma Department of Transportation - Student Engineer – Worked with the engineering staff on the
design and board drafting of rural highways while studying civil engineering. The Design Squad was
located on campus in the Civil Engineering Building at Oklahoma State University (OSU).
M.J. Lee Construction Company – Carpenter/Operator – Worked as Carpenter and paving machine
operator on numerous bridge projects throughout northeast Oklahoma while obtaining his engineering
degree.

Sara Arzu, P.E., CFM
CGA’s Project Manager, Ms Arzu, has 20 years of civil engineering
experience with an emphasis in Transportation and Storm water. She
oversees the production of street and storm water design projects. She
has worked for numerous municipalities and private engineering firms
designing and overseeing all types of public projects. Ms. Arzu is a
Certified Floodplain Manager in addition to being a Professional
Engineer.
EXPERIENCE
CGA Engineers, Inc. - Project Engineer – Ms. Arzu is responsible for
project site design and plan production with a focus on transportation
projects.
Breisch & Associates E&A - Project Engineer – Independent and
supervised level of engineering design for numerous municipal and private projects. Project examples
include street reconstruction, crack/joint seal, water and sanitary sewer design, retaining wall and
drainage system design, and permanent traffic signage plan design.
City of Owasso Public Works Department/Engineering Division - Senior Civil Engineer – Engineering
Management and independent, advanced level of engineering work in design, project management,
planning, construction and inspection of public works and private development projects. She served as the
Development Services Manager, Deputy Floodplain Administrator, and MS4 Storm water Phase II
Coordinator during her time with the City of Owasso Public Works Department
City of Tulsa Public Works Department/Engineering Services - Civil Engineer III – Sara demonstrated
an advanced level of engineering work in design, project management, contract negotiation and
administration, and construction and inspection of public works projects while with the City of Tulsa
Public Works Department. Sara worked on a multitude of projects encompassing the areas of storm water,
infrastructure rehabilitation, waterline, sanitary sewer, and transportation.
Burns & McDonnell E&A - Project Engineer – Involved in roadway and traffic design projects in which
she designed and evaluated alternatives for such projects as an Arterial Street Widening project in Broken
Arrow and the widening of US Highway 169 for ODOT. She prepared construction cost estimates using
ODOT standards and specifications along with specialized cost estimating software.
Port Authority of NY/NJ – Engineer intern for aviation design division. Worked with the aviation
engineering staff on design/drafting analysis and vision 2025 conceptual alternatives for all three major
airports in the state of New York.

Patrick Wilson, P.E.
Mr. Wilson is a licensed engineer with more than ten years of experience in all
phases of engineering for transportation and civil site projects, particularly in the area
of hydrology and roadway design. His experience includes roadway and site design;
horizontal and vertical roadway geometry; preparation of final plans, specifications
and cost estimates; hydraulic analysis; drainage design for culverts, ditches, storm
sewers, water mains, and sanitary sewers; permits; and construction observation and

A land surveyor with close to 30 years of experience performing boundary surveys,
American Land Title Association property surveys, cross country utility surveys,
topographic surveys for engineering design, hazard mitigation and remediation,
subdivision and cemetery designs and lot layouts, engineering surveys for various
types of civil engineering projects, accretion surveys, hydraulic feature surveys and

CGA Engineers was contracted by EST, Inc. to provide topographical survey information for bridge
replacement project over Red River on Highway 75 Bryan County line between Oklahoma and Texas.
The scope of work for this project included providing cross sections upstream and downstream of the
existing bridge for approximately 1,000 feet both ways. The topographic collection information provided
sections on 50 ft intervals for the first 250 ft and 100 ft intervals for the remainder of the project limits.
The survey data was developed from digital terrain model and direct sections. Investigation and collection
was done both on land and with the assistance of a boat. Low flow conditions less than 700 cfs collected
at the Arthur Gauge Station was used as maximum flow to perform the work.

CGA Engineers, Inc. was subcontracted by Holloway, Updike & Bellen, (HUB), Engineering to design an
incidental construction detour shoofly. Approximately 2,200 linear feet of detour shoofly with minimum
offset of 70’-75’ from the centerline of the proposed mainline road. The best design involved designing
the shoofly as short as possible within the specified limits. Project also involved design of 12 ft TBSC
driveways for local access; extending temporary storm cross drain 30” RCP pipes. The project also
involved providing Cut and Fill quantities for shoofly area. The most current edition (2011) of ASSHTO
standards design parameters were used for the Shoofly. The following parameters were used:
Design speed of 45 mph
2% normal crown/reverse crown
Meet proposed mainline design elevation at both ends of project
Use typical section as provided by HUB 2” wearing surface over 8” sub-grade
Use 4:1 to 3:1 cut/fill slope for the shoofly road
Check design for maximum radii
Use 2009 ODOT Construction specifications
CGA prepared P&P and X-sections and HUB incorporated the Shoofly Design into the main road P&P

The Cherokee Nation Roads and Bridges Department contracted CGA Engineers, Inc. to design
improvements for the intersection of US Highway 62 and Coffee Hollow Road/Cherokee Street. The
project includes the design of a deceleration/acceleration lane. A 12 ft with 8 ft shoulder right turn
auxiliary land for a 45 mph corridor was designed. The new cross section of 2% and pavement thickness
of 9 inches Portland cement concrete over a 4 inch oen graded bituminous base. Also included as part of
the improvement was the rehabilitation of Cherokee Str, main entrance to Sequoyah High School which
included widening of the school access with a combined open and closed cross section. The project also
included the installation of new signalization eight (8) phasing cycle with loop detection system. Existing
markings and traffic signs were replaced with new MUTCD standard traffic control marking and signage.
Also part of the improvements included milling and overlaying of Coffee Hollow Rd. and new striping of
the US 62 intersections.

SUMMARY
DBE Goal
CGA Engineers, Inc. is an ODOT Certified DBE. Being and ODOT Certified DBE firm, CGA will meet
the project goals by performing all the required services with the exception of geotechnical services,
which we typically use the services of the consultants Terracon.
Far Audit Status
CGA Engineers, Inc. has an audit for 2010 accepted by the Department. This audit was performed after
the award of a Pedestrian Improvements Project that was placed on hold indefinitely by the department.

CGA Engineers, Inc. is committed to providing engineering services tailored to fit the needs of our
clients, and to deliver projects on time and within budget. We are familiar with ODOT construction
specifications and standards and have previously worked with the members of the Department staff.
In order to maximize the efficiency of various projects, CGA uses the latest version of MicroStation and
InRoads civil engineering software, the same used by the Department. All design software is kept up-to-
date to ensure compatibility. CGA updates its computer hardware to provide accurate, timely designs.
Additionally, CGA Engineers has its own in-house survey crew to provide timely and accurate collection
of field data.
At CGA Engineers, we understand that there are many design and technical items that go into the
development of a transportation project. Typically, one thinks of this including surveying, right of way,
geometric design, grading and drainage, and bidding. But there are many other items that must be
included. These items include stormwater management both during construction and for final
construction. It includes traffic control, not just the traffic control of the completed project, but control
during construction. This may include detours or temporary construction. Likewise, the design of
pedestrian facilities will include access during construction. Our engineers are aware of all the
complexities involved in the design of these projects and they are experienced in all these areas.
With our staff, we are able to perform and manage all the necessary planning and design functions of a
grade, drain and surfacing project in-house. We have professional engineers, a registered surveyor, civil
technicians, and construction observers on staff. The current staff has worked together on similar projects
and understands the complexities. We look forward to the opportunity to working with the Department.

DCAM-CAP Form 255 is the companion form to CAP Form 254 which permits Architects, Landscape Architects, Engineers and Land Surveyors to respond to invitations to be considered for design projects from the State of Oklahoma. It permits consultants to tailor their response to the specific project being considered by an agency. This form is used in conjunction with DCAM-CAP Form 254 in the same manner as Federal Standard Forms 254 and 255 are used for Federal selections. DCAM-CAP Form 255 is to be used for a specific project and DCAM-CAP Form 254 is used to be registered for consideration. These forms have been designed to be as similar as possible to the Federal forms but SF254 and SF255 MAY NOT be used for State registration and selection. This form is divided into (6) break sections. To easily view where these sections occur, use the “View/Normal” command. The first section contains a table, is unprotected, and allows for the first sheet/instructions to be deleted. The table can be deleted by selecting the whole table and using the “Edit/Cut” command. The second section (Items 1 thru 5a) are protected and contains fields that can be filled in with the appropriate information. The third and fourth sections (Item 6.) are unprotected and don’t contain any fields. This allows for the whole table to be copied and pasted so that additional “Brief resumes…” can be input. This project included an existing two-lane rural highway being upgraded to a super two-lane on four-lane right of way alignment. The 4.6 mile project included two multi-span bridge structures for streams feeding Oologah Lake. The project incorporated design, right of way plans, hydraulics and hydrology studies, utility relocations, and railroad coordination. Maintenance Zone 5041 – Tulsa, Oklahoma – Rehabilitation of 1.8 miles of 2-lan asphalt streets with concrete curb and gutters.
