IRS Verify IRS Verify is available through the LexisNexis® Insurance Solutions portal or via interactive XML delivery.

IRS Verify: Place a New Order

There are two different ways to use this service:

• Upload an existing 4506-T/4506T-EZ that has already been signed and dated by the consumer. • Have LexisNexis generate the 4506-T/4506T-EZ to then have signed and dated by the consumer.

You can upload forms for an individual consumer or a business entity. The steps specific to these processes differ slightly. Please be sure to follow the steps for your specific consumer type.

Uploading an existing 4506-T/4506T-EZ for an Individual If you already have a signed and dated 4506-T/4506T-EZ in .pdf, .tif or .tiff format, you can choose to upload it to IRS Verify. Follow the steps below to complete the process.

Step 1 1. In the Step 1 area, click on the radio button next to Upload a Signed 4506-T/4506T-EZ Form.

Fields with an asterisk (*) denote information is required for the form to be submitted.

2. Select whether or not your existing form is signed, then enter the date the form was signed in

the field provided, or click the calendar icon to choose the date. If you select No, the form must be signed and dated before submittal.

3. Choose the type of form you want to upload.

a. If you select Individual 1040 or 1041 you must also select whether the form is a Return Transcript, Account Transcript, Record of Account, or Verification of Nonfiling.

b. If you select W2, the Form W2, Form 1099 series, Form 1098 series, or Form 5498 series transcript option is already selected for you.

c. If the signed form has both 1040 and W2 selected, LexisNexis will automatically create two orders for the customer.

4. Click Browse, navigate to the existing form on your computer, and click Open.

5. Verify before submittal that the form is signed by the tax payer and that Line 5 contains LexisNexis’ name and address.

Step 2 1. In the Step 2 area, enter the consumer’s SSN, last name, and first initial.

2. Enter the tax years requested in the Tax Year Requested field(s). a. If you are uploading a form 1040 or 1041, you must include the current year in the first

YYYY field. You can choose to also include up to the past three years. b. If you are uploading a form W-2, 1099, 1098 or 5498, the required field must include the

current year, plus up to the prior three years. IRS returns only the four most recent years. Ensure the number of years requested here match the number of years requested on your form as they will determine the amount you are charged.

Step 3 1. In the Step 3 area, include your email address and phone number. You can choose to use the

Reference ID field to tag this form with a distinct identifier.

2. Click Place Order to complete the process, or Reset Form to empty all fields and start over.

Uploading an existing 4506-T/4506T-EZ for a Business If you already have a signed and dated 4506-T/4506T-EZ in .pdf, .tif or .tiff format, you can choose to upload it to IRS Verify. Follow the steps below to complete the process.

Step 1 1. In the Step 1 area, click the radio button next to Upload a Signed 4506-T/4506T-EZ Form.

Fields with an asterisk (*) denote information is required for the form to be submitted.

2. Select whether or not your existing form is signed, then enter the date the form was signed in the field provided, or click the calendar icon to choose the date.

3. Choose business form 941, 1065 or 1120.

4. Choose Return Transcript, Account Transcript, Record of Account or Verification of Nonfiling.

5. Click the Yes radio button next to Title to indicate that the consumer who signed the form is an officer of the company and that his title appears on the form. Click No if these parameters are not met. The Title field will be required upon upload.

6. Click Browse, navigate to the existing form on your computer, and click Open.

Step 2 1. In the Step 2 area, enter the company’s EIN and name.

2. Enter the tax period(s) requested in the Tax Period Requested field(s). a. Period(s) can be the current year plus the prior three years. b. The number of years requested should match the number of years requested on your

form as they will determine the amount you are charged.

Step 3 1. In the Step 3 area, include your email address and phone number. You can choose to use the

Reference ID field to tag this form with a distinct identifier.

2. Click Place Order to complete the process, or Reset Form to empty all fields and start over.

IRS Verify: Place a New Order (Generating a 4506T/T-EZ) To have LexisNexis generate a 4506T/T-EZ form for you, follow the steps below:

1. In the Step 1 area, click the radio button next to Create a 4506-T/T-EZ.

2. Select a Transcript type from the drop down menu.

3. In the Step 2 area, do as many of the following as apply: a. Complete the taxpayer

information. The fields with an asterisk are required.

b. If there is a spouse and the transcripts are for a joint return, enter the spouse information.

c. If the address on the last tax return is different from the taxpayer’s current address, enter it in the previous address section.

d. Enter the Tax Period requested.

4. In the Step 3 area, enter your contact information.

5. Click Place Order.

Line 1a Name shown on tax return; if a joint return, enter the name shown first.

Line 1b Enter the Social Security Number for the person listed on Line 1a. If your request relates to a business return (e.g., 1120), enter your Employer Identification Number (EIN).

Line 2a If a joint return, enter spouse’s name shown on tax return.

Line 2b If a joint return was filed, enter the spouse’s SSN.

Line 3 Current name, address (including apt., room, or suite no.), city, state, and ZIP code.

Line 4 Previous address shown on the last return filed if different from Line 3.

NOTE: the address must match the address for the tax return year being requested. If the address was different in a previous year, please make sure to list that address on Line 4.

Line 5 This section MUST be completed at the time the consumer signs the form. LexisNexis and the IRS require that Line 5 contain the following information:

NOTE: the UID data MUST be on Line 5 for the request to be processed.

Line 6 Enter only one tax form number per request.

Signature and Date Form 4506-T must be signed and dated by the taxpayer listed on Line 1a or 2a. Note: LexisNexis must receive Form 4506-T within 115 days of the date signed by the taxpayer or it will be rejected.

Tip

Signatures of Taxpayers The 4506-T must be signed by either the taxpayer whose name is shown on Line 1a or 2a, or a person authorized to obtain the tax information requested. If the request applies to a joint return, either husband or wife must sign. If signed by a corporate officer, partner, guardian, tax matters partner, executor, receiver, administrator, trustee, or party other than the taxpayer, that person must have the authority to execute Form 4506-T on behalf of the taxpayer. It is critical that the date not only be legible, but also must appear unaltered. This is a strict IRS requirement.

Individuals Transcripts of jointly filed tax returns may be furnished to either spouse. Only one signature is required. Sign Form 4506-T exactly as your name appeared on the original return. If your name has changed, sign your current name as well.

Corporations Generally, Form 4506-T can be signed by:

I. An officer having legal authority to bind the corporation II. Any person designated by the board of directors or other governing body

III. Any officer or employee on written request by any principal officer and attested to by the secretary or other officer

The type of data supplied to the business is determined by their needs. There are five (5) types of tax return/account information transcripts:

• Tax Return Transcript – (used most by our clients) • Tax Account Transcript • Record of Account Transcript • Verification of Nonfiling • Wage and Income Information Documents

IRS Verify – Current and Completed Orders Note: Current orders are separated from Completed orders to make it easier to review and manage pending inventory.

Columns (sort by clicking on the column heading) will include:

• Subject – the first name, middle initial and last name • SSN or EIN – last four digits • Order Number – displays the order number and allows the user to “Edit,” “Delete” or “View”

the order form • Upload – provides the ability to “Add” a signed 4506-T or 4506T-EZ that you have scanned

into your system in .pdf or .tiff format • Status – provides the status and sub-status of each order • Date Created – date and time order was created (by customer) • Last Updated At – date and time the status or sub-status last changed

Tips

Refresh Screen The Current Orders screen does NOT automatically refresh itself. To see the current status of a newly updated order, refresh the screen.

Upload When you upload your 4506-T form, it cannot be edited or saved again. Once you upload the form, view it to make sure it displays a barcode in the upper right-hand corner. If the barcode is missing, please try rotating your image and upload it again. The barcode is a critical component of the submission to the IRS. If the barcode is covering important information, such as the SSN, do not worry—LexisNexis can move the barcode up and down for you.

Current Order Status and Sub-Status New – Action Required – A live link directing the user to upload a signed 4506-T or fax to the toll free number provided.

Pending – QA Review – A live link stating the order is “Being Reviewed by CUSTOMER SERVICE/QA.”

Pending – Action Required – A live link displaying all errors supplied by CUSTOMER SERVICE/QA.

Tip

An order with an Action Required status means that the user must correct the errors and then resubmit the form to LexisNexis. Click on the link to review the form, make the necessary corrections and resubmit.

Pending – Sent to IRS – A live link stating the order was “Submitted to the IRS” for processing. 1. Upload column states “Document Received” 2. Order Number column will display a link stating “View” Complete – View Report – A live link displaying the final report for that order. Fatal Error – A live link that indicates there was a fatal transmission problem with the submission of the form to the IRS. You must resubmit your form for processing as this order is now cancelled. Expired – A live link stating the authorization period has elapsed, a new signed 4506-T form is required for processing, or the request has been inactive for more than 30 days. 1. Upload column states “Expired” 2. Order Number column will display a link stating “View.”
