Survey results Providing accountable care The National Association of Primary Care has published Providing accountable care: comparing the delivery of Primary Care in the UK and USA through accountable care systems and organisations (pdf). This report summarises some of the debate by system leaders within the UK and US around the accountable care system. Based on a symposium held in May 2017, it also describes some of the future challenges and possible solutions. Report Delivering a workforce for integrated care Reform has published Getting into shape: delivering a workforce for integrated care. This paper makes recommendations designed to build a workforce for integrated care. It highlights cultural factors that divert trainees away from high-need specialities and describes barriers preventing some staff from utilising all their skills. It also provides details of reform to the funding and training systems. To read more click here Accountability of partnership working The Good Governance Institute has published Scrutiny: the new assurance? A good governance discussion document? As collaboration and partnership working need to become more streamlined, more strategic and more effective, organisations spending public monies should be constantly redefining their roles and responsibilities, searching for constant improvement. This paper looks at scrutiny across a number of organisations, as all four home nations are seeking better outcomes by the alignment of health, social care and other funders and providers. To read more click here CQC consultation The Department of Health has launched a consultation on proposals to amend the Care Quality Commission (Reviews and Performance Assessments) Regulations 2014 so that CQC has a duty to rate all providers of regulated activities, except for some specific registered service providers and regulated activities. The consultation is targeted at providers of services registered with the CQC and closes on 6 November 2017. To read more click here NHS efficiency map: engaging staff The Healthcare Financial Management Association in partnership with NHS Improvement has updated the NHS efficiency map. This latest update includes a case study from Salisbury NHS Foundation Trust’s Save 7 programme. This aims to give all staff the chance to be involved in innovative change projects that simultaneously deliver financial savings, cultural change and service improvement (Open the efficiency map to access the link to the case study – search for “engaging staff” within the map). To read more click here Group B streptococcal disease guideline The Royal College of Obstetricians and Gynaecologists has updated Group B Streptococcal Disease, Early-onset (Green-top Guideline No 36). This guideline guidance for obstetricians, midwives and neonatologists on the prevention of early-onset (less than 7 days of age) neonatal group B streptococcal (EOGBS) disease. Guidance Lack of new antibiotic development The World Health Organisation has published Antibacterial agents in clinical development: an analysis of the antibacterial clinical development pipeline, including tuberculosis. This report focuses on the lack of new antibiotics under development to combat the growing threat of antimicrobial resistance. The report found very few potential treatment options for those antibiotic-resistant infections identified by WHO as posing the greatest threat to health. Report

Specialised commissioning NHS England has published NHS England Specialised Commissioning: service development policy. This policy sets out NHS England’s approach for making decisions about which new treatments and interventions to routinely commission, and its approach for updating existing service specifications or creating new ones. NHS England has also published an infographic providing an overview of the service development process. To read more click here Service development infographic Commissioning biological medicines NHS England has published Commissioning framework for biological medicines (including biosimilar medicines. This framework sets out the importance of taking a collaborative approach to the commissioning of biological medicines from the outset, as well as setting out how this can be achieved. Framework Safer use of mental health medicines NHS England has published Health and justice mental health services: safer use of mental health medicines. This document provides a background to prescribing and mental health medicines in secure environments with some common principles that apply throughout the medicines optimisation pathway, and more detailed information within the pathway to support safer practice. To read more click here Transferring people in and out of hospital The Social Care Institute for Excellence and NICE has published Moving between hospital and home, including care homes: a quick guide for registered managers of care homes and home care. This guide, based on NICE guidelines and quality standards, provides a brief overview of how care home managers can work with hospitals to ensure a good experience of transition for the people in their care. Guidance Housing services and the NHS The National Housing Federation has published Home from hospital: how housing services are relieving pressure on the NHS. The report contains case studies which demonstrate how housing services are successfully reducing delays in discharging people from hospital and preventing unnecessary hospital admissions. It considers the impact and additional savings that could be made by housing providers if this work were to be scaled up. Report Housing needs of ageing populations The Local Government Association has published Housing our aging population: learning from councils meeting the housing need of our aging population. Between 2008 and 2039, 74 per cent of projected household growth will be made up of households with someone aged 65 or older. The suitability of the housing stock is of critical importance to the health of individuals and also impacts on public spending, particularly social care and the NHS. This report sets out in detail what is required to meet the housing needs of the UK’s ageing population and how councils around the country are innovating to support older people to live in their homes for longer and promote positive ageing. Report New technology helping to tackle frailty in older people NHS England is reporting the trialling of new wearable sensors, based on technology used by NASA and the CGI film industry, to help identify older patients at risk of falls as part of a new drive to tackle frailty. One system, designed to analyse the way a patient walks, was developed in partnership with Intel and tested at University College Dublin’s Institute for Sports and Health. In Sheffield, the Perfect Patient Pathway Test Bed is testing Kinesis’ QTUG device for patients over 65 who have no recorded falls and score as ‘moderately frail’. The system sees patients walk with the motion sensor attachments on their legs while an app carries out the analysis. To read more click here Enabling people with dementia to remain at home Housing LIN has published Are we nearly there yet? Enabling people with dementia to remain at home: a housing perspective. This report sets out the key role housing providers, and in particular social housing providers, can play in supporting people living with dementia to stay independent in the home of their choice for as long as possible. Hip fracture database The Royal College of Physicians has published National Hip Fracture Database (NHFD) annual report 2017. This ninth annual report examines trends in performance and outcome over the 10 years since the NHFD was established. It shows that while more patients are receiving early surgery and surviving a hip fracture, two in five are not receiving all of the recommended elements of a hip fracture programme that represent ‘best practice’. Death and dying in the UK The University of Bath Institute for Policy Research has published Death, dying and devolution. This report reviews policy areas associated with death, dying and bereavement within the context of devolution. It focuses on the national and regional delegation of power, resources and authority across the UK and the implications of this for those who are dying, dead, or bereaved as well as those tasked with organising and running the services that support these groups of people. Report

New care models and technology NHS Clinical Commissioners in partnership with NHS Providers, NHS Confederation and the Local Government Association has published New care models: harnessing technology. Rising demand for services, constrained funding and a multitude of workforce challenges requires the NHS think differently about the way services are delivered. This report explores how digital tools are a key part of the answer to this set of challenges, and demonstrates how technology has the potential to revolutionise the way health and care is delivered. The report highlights how five vanguards are implementing innovative digital technology solutions at the heart of a new approach to care. Report

Sustainability and transformation plans in London The Kings Fund has published Sustainability and transformation plans in London: an independent analysis of the October 2016 STPs (completed in March 2017). This report looks at the five STPs in London, their contents and common themes. The authors assess the main issues and risks to be addressed across the plans. They make a small number of recommendations for the future of the STP process in London, focusing specifically on the role of London-wide action in taking forward the plans. The report includes a brief update on progress since March 2017. Report

Partnerships for improvement: ingredients for success This report from the Health Foundation focuses on five different partnering arrangements, as well as interviews with national leaders, and draws learning to help inform and guide policymakers and providers. Report

Specialised services NHS England has published Spotlight on specialised services. This document provides an insight into some of the life-transforming specialised services that are already being delivered and will continue to be rolled out over the coming years. To read more click here

Sepsis action plan NHS England has published Second sepsis action plan. This document identifies further actions which will be undertaken by healthcare organisations to enable continuing improvements in sepsis care across over the next year. Amongst others, this document is aimed at CCG clinical leaders. To read more click here

Clinical negligence costs The National Audit Office has published Managing the costs of clinical negligence in trusts. This report highlights that over the last ten years, spending on the Clinical Negligence Scheme for Trusts has quadrupled from £0.4 billion in 2006-07 to £1.6 billion in 2016-17, while the number of successful clinical negligence claims where damages were awarded has more than doubled, from 2,800 to 7,300. The cost of clinical negligence claims is now rising at a faster rate year-on-year, than NHS funding. Report

Day in the life of a general practice nurse NHS Health Education England has launched a short film showcasing a day in the life of a general practice nurse. The film shows the depth and breadth of activities a general practice nurse can get involved with in a regular day. This has been developed as part of the General Practice Nursing Workforce Development Plan aimed at promoting the importance of expanding and developing the general practice nursing workforce. Reducing health inequalities Public Health England has published Reducing health inequalities: system, scale and sustainability. This document aims to support local action to tackle health inequalities. It helps local partners identify what specific interventions could measurably improve outcomes. It is also accompanied by a slide set. It is aimed at: local authority leaders; chief executives other senior officers and councillors; Directors of Public Health; public health specialists and health service commissioners. Whooping cough vaccination in pregnancy Public Health England has published an information poster to support the whooping cough vaccination in pregnancy programme. The poster has been designed to promote the need for maternal pertussis vaccination which is now recommended from week 16 of each pregnancy. It is suitable for GP surgeries PHE poster

Childhood eczema The latest Highlight from the National Institute for Health Research Studies (NIHR) summarises findings from studies looking at treatments for childhood eczema. It provides evidence for treatment options, examines how NIHR research fits with current NICE guidance, and provides details of where to get advice and support. To read more click here

NHS maternity services The Institute of Fiscal Studies has published Under pressure? NHS maternity services in England. This briefing note discusses the causes and consequences of short-run and long-run pressures on NHS maternity services in England. It finds the number of maternity cases has stabilised since 2010 but the case mix has changed. Briefing

Community paediatric services The Royal College of Paediatrics and Child Health and the British Association for Community Child Health have published a set of resources to support workforce planning for community paediatric services. The main report raises concerns over the growing demand and pressures faced by services; it also provides a number of recommendations about workforce numbers and service planning. To read more click here Large scale general practice In July 2016, the Nuffield Trust published Is bigger better? Lessons for large-scale general practice. This study examined the factors affecting the evolution of general practice and its impact on quality, staff and patient experience. The Nuffield Trust has now release four booklets which sit alongside this main report covering: the leadership and governance of large-scale general practice organisations, sustainability, working with the wider health economy, and improving quality. Electronic referrals Two hospital trusts and their referring GP practices have become the first in the country to adopt a digital process for booking patient hospital appointments. Sherwood Forest Hospitals NHS Foundation Trust and County Durham & Darlington NHS Foundation Trust are now processing all of their hospital referral appointments electronically via the NHS e-Referral Service. Patients are also able to change or cancel their appointments through the system with analysis showing that use of electronic referrals has halved the rate of patients missing appointments from 10% to 5%. Body art guidance In its first clinical report on tattoos and piercings, the American Academy of Pediatrics (AAP) offers guidance for physicians and parents on talking to adolescents about an increasingly common desire for body art. The AAP report Adolescent and Young Adult Tattooing, Piercing and Scarification was developed to address the growing numbers of teens and young adults choosing body art. Report

Preventing heart attacks and strokes The NHS and Public Health England (PHE) have announced a new drive to save lives by preventing heart attacks and strokes brought on by cardiovascular disease. Analysis suggests that there is an opportunity to prevent more than 9,000 heart attacks and at least 14,000 strokes over the next 3 years with better detection and management of: high blood pressure; high cholesterol and atrial fibrillation. PHE and NHS England have written to all 44 STPs, drawing attention to the prevention opportunity in their local areas, and sharing with them the data for their individual STPs. NHS England launches practical guides to help people with disabilities access GP online services Practical guides to help disabled patients to access GP online services have been published by NHS England. Aimed at supporting people with sight loss, hearing loss, autism or learning disabilities, a series of new guides have been developed to help patients use GP services online such as book appointments, repeat their prescriptions, etc., Guidance Health economics evidence Public Health England has published Health economics: evidence resource. This tool shows the key cost-effectiveness and return on investment evidence on activities in the public health grant. It allows users to understand the relevance and apply the evidence to their local setting. To read more click here

MENTAL HEALTH NEWS

Maternal mental health care The Chief Executive of NHS England has announced four new Mother and Baby Units which will allow women to stay with their babies while receiving the specialist care they need. These new units will provide in-patient support for women and their babies with the most complex and severe needs who require hospital care, who are experiencing severe mental health crisis including very serious conditions like post-partum psychosis. Mental health in older people NHS England has published A practice primer on mental health in older people. This document highlights symptoms often attributed to ‘old age’ but where a mental health diagnosis and follow-up is more appropriate. It is aimed at primary care staff, in particular GPs. Blending Face-to-Face and Internet-Based Interventions for the Treatment of Mental Disorders in Adults: Systematic Review Many studies have provided evidence for the effectiveness of Internet-based stand-alone interventions for mental disorders. A newer form of intervention combines the strengths of face-to-face (f2f) and Internet approaches (blended interventions). http://www.jmir.org/2017/9/e306/ Reforming the mental health service workforce The Centre for Mental Health has published The future of the mental health workforce. This report is based on insights from service users, carers and professionals and outlines a list of recommendations for a sustainable mental health workforce. It emphasises the importance of prevention, including the role of GPs in supporting people before they reach crisis point. It describes commissioning of mental health services as in “crisis” with a “shrinking workforce, growing expectations and exhausting demands” putting pressure on staff across the country. The report notes that a third of GP consultations involve mental health but fewer than half of GPs have mental health training placements. Report

Resource to reduce alcohol stigma A new e-learning programme has been developed for NHS healthcare professionals to reduce the stigma faced by patients with alcohol problems. A package of training materials, including facilitator’s notes, are included in the programme, which aims to help health professionals to have a better understanding of alcohol dependency and to improve longer-term outcomes for patients. The resources will be of interest to health and social care staff who come in to contact with patients with alcohol problems in either: hospital, primary care or community settings. This report includes the results of a freedom of information request to providers of specialist child and adolescent mental health services to explore access and waiting times for young people. Mental health support for young people leaving care Barnardo’s the children and young people’s charity has published Neglected Minds: A report on mental health support for younger people leaving care. This report highlights that two thirds of care leavers identified as having mental health needs were not receiving any help from a statutory service. It also found that one in four had faced a mental health crisis since leaving care. Barnardo’s wants clinical commissioning groups to invest more in services specifically aimed at meeting the needs of young people leaving care, such as embedding a mental health worker within leaving care teams. Mental ill-health of children and young people The National Children’s Bureau and UCL Institute of Education have published a briefing Mental ill-health among children of the new century. The briefing summaries the prevalence of mental health problems among children taking part in the Millennium Cohort Study and shows that while the majority of 3-14-year-olds in the UK are not suffering from mental ill-health, a substantial proportion experience significant difficulties.
