How to Apply for a Food Business Permit
When Opening a New Business
1. Discussions in Advance of the Opening
(Please contact the person in charge in your municipality in advance, before visiting their office.)
・Before commencing construction of your establishment, please prepare a ground plan detailing the kitchen
or working area. We will check whether your plan meets our facility criteria etc. At this time, we will
hand over a complete set of application documents.
2. Submitting the Application Documents
・The table below lists the application documents. Please submit these documents 7-10 days before the
scheduled completion date of the construction of your establishment (if there is none, then the day you
wish to be inspected).
When submitting the documents, please inform the person in charge in your municipality of the name
and contact details of the person who will be present for the on-site inspection, as well as confirming the
scheduled date of the completion of the construction.

This is necessary if the water used in the kitchen etc. passes
through the building's water tank (only in the case of a small-
scale water tank with an effective capacity of 10 m3 or less), or
is from a well. It is not necessary if the water is directly
connected to the mains supply.

* Please complete these documents with either a ballpoint pen or fountain pen, using blue or black ink. (Do not use
writing materials that can be erased.)
* In principle, any handling fee for a permit application, once paid, will not be refunded according to the rules of
the Minato City Health and Hygiene Office regarding handling fees. Please be aware of this.

3. On-site Inspection of Facilities
・Based on the documents submitted in advance, an inspection of the completed facilities will be carried
out. Should anything be noted during the inspection, please redress this, and arrange another inspection.
4. Permit Issuance
・When receiving the Business Permit(営業許可書), your personal seal is necessary.
・Please ask the person in charge of your inspection when your Business Permit is scheduled to be issued.
Continuing Your Business
1. Submitting the Application Documents
・ The table below lists the application documents. Please ensure that all documents are submitted
approximately one month before your Business Permit expires.

* Please be aware that if you are unable to submit these documents by the time your Permit runs out, you may be
seen as operating your business without a license. In addition, your application fee may be treated as that of a
new application (approximately doubled).
* In principle, any handling fee for a permit application, once paid, will not be refunded according to the rules of
the Minato City Health and Hygiene Office regarding handling fees. Please be aware of this.
2. On-site Inspection of Facilities
・When you renew your permit, your facilities will be inspected in the same way as they were when you
first opened your business. Should anything be noted during the inspection, please redress this, and
arrange another inspection.
You can obtain the Business Permit Application, Outline of Business Facilities, Notification of Change, Notification of Change of Food Hygiene Manager, and Notification of Cessation of Business from the counter at the Minato Public Health Center, or download them from the Minato City Website(http://www.city.minato.tokyo.jp) > Applications > Download Application Forms

It is the responsibility of anyone who runs a food-related business to have a Food
Hygiene Manager for each facility with a Business Permit. (Please display the
Manager's plate inside the facility)
In order to become a Food Hygiene Manager, at least one qualification, such as the
following, is necessary:
(1) Nutritionist, Chef, Ship's Cook, Poultry Processing Hygiene Supervisor or those
who have qualifications of Food Hygiene Supervisor
(2) Those who have taken classes to become a Food Hygiene Manager

Plans and overview of your facilities clearly
showing the changes, two copies of each.
* If the form of your corporation or the facilities of your business are being changed, you may have to apply for a

* For the Certificate of Registered Matters, please prepare a document (Certificate of All Historical Matters,
Certificate of All Removed Matters etc.) that can confirm the record of changes. (Must be from within the last
six months. You must bring the original certificate, as a copy alone will not be accepted.)
Notification of the Change of Food Hygiene Manager(食品衛生責任者変更届)
・Should there be a change in the Food Hygiene Manager in any facility with a Business Permit, please
bring documentation proving the qualifications of the new Food Hygiene Manager, and submit a
Notification of the Change of Food Hygiene Manager.
Notification of Succession
・It is always necessary to obtain a new Business Permit in the case of a change in the business owner.
However, please consult us in advance as in the following cases succession is acknowledged.
1. (For individuals) If the named owner passes away and their heir takes their place.
2. (For corporations) If the named corporation merges and terminates.
3. (For corporations) If the named corporation splits and the newly established corporation takes their
place.

Cessation of Business
Notification of Cessation of Business(廃業届)
・In the following cases a Notification of Cessation of Business is necessary. As well as bringing your
Business Permit(営業許可書), please submit a Notification of Cessation within 10 days of finishing
business.
1. If your business has discontinued.
2. If you have relocated your premises.
3. If the owner (named) has changed.
4. If there are changes to the business facilities such as an extension or reconstruction of the building.

Facility Criteria for Restaurant Business (Excerpt) Regulations for the Enforcement of the Food Sanitation Act, Appended Table 2 (Re. Article 3)
1. Building ・・・・・・・・・・・・・・・・・・・・・・・・・・・・・ The building must be a sufficiently durable structure;
steel-framed, reinforced concrete, stone, wood and
plaster, wood, etc.
2. Partitions ・・・・・・・・・・・・・・・・・・・・・・・・・・・・ There must be suitable partitions, for example walls or
boards, according to the intended use.
3. Floor ・・・・・・・・・・・・・・・・・・・・・・・・・・・・・・・・ The floor must be constructed from water-resistant

and sterilization purposes.

to store any food items.

refrigerator and kitchen.

hygienically store tableware and utensils.

lid.
15. Storage Facilities for Cleaning Equipment ・・ There should be dedicated storage facilities for cleaning
equipment.
16. Changing rooms or lockers for changing ・・・・ These should be installed outside of the kitchen.
17. Toilets for employee use ・・・・・・・・・・・・・・・・・ Toilets must be installed in an area where they will not
affect the kitchen, and have hand-washing facilities and
equipment for disinfecting hands.
