The following information is to assist staff joining LJWB to understand and appreciate the work content of their post and the role they are to play in the organisation.

1. Prime objectives of the post To support people living with dementia by delivering provision of appropriate support services to enable them

to live well and remain independent for as long as possible, whilst equipping clients and their families/carers to feel confident about their ability to live well with dementia.

To support the Dementia Coordinator to develop and implement a dementia friendly environment at the MAZCC.

To support the Living Project in delivery against its service outcomes.

To support with undertaking the start-up, implementation and completion of assigned projects relating to the Living Project.

To support the Dementia Coordinator to undertake comprehensive, tailored, holistic assessment of need and co-ordination of services in order to collaboratively identify support goals.

To support carers in their roles, providing supporting advice, information and respite support, including the development of self-sustaining carers group and social activities.

To ensure the provision of effective services to meet Leeds Jewish Welfare Board’s (LJWB) charitable objectives and meet community’s needs.

2. Key responsibilities and range of duties: The post holder will have to be flexible and able to adapt to the pace of a fast changing environment,

supporting the Dementia Coordinator in reacting to this changing environment.

The post holder will undertake case management in line with organisational policies and procedures.

To demonstrate knowledge and understanding of issues affecting people with dementia.

To support the Living Project to promote the delivery of excellence in dementia practice for individuals within the community living with dementia, memory and/or communication problems.

Working with the Dementia Coordinator to develop and implement creative projects for clients with dementia and their family/carers.

To support the Living Project in providing a comprehensive and dynamic range of activities and support which successfully integrate clients, and their family/carers and facilitates the reduction of their social isolation.

To promote a person-centred approach to all activities with clients in their own homes, to meet their needs.

Working with the Dementia Coordinator to triage referrals and signpost quickly where necessary to specialist support services.

To ensure that client records and other information systems are completed accurately and timely, enabling the monitoring and recording of outcomes on all clients. Completed using accessing database systems, the use of Outcome Star software and Person Centred Support Plans.

To seek, record and collate client feedback to understand whether needs are being met, and work with the Dementia Coordinator and Quality Assurance Manager to plan any necessary adjustments.

To support the Dementia Coordinator in raising awareness in the community about dementia and the way it affects individuals and families.

To ensure that day to day delivery of service is underpinned by a constant focus on outcomes, with regular goal planning and review ensuring that outcomes are achieved within service timescales.

To identify any barriers to individuals accessing services and activities and work with individuals to overcome these.

To support with statistical and other information and data as required for the Dementia Coordinator to report activity and outcomes and ensure effective qualitative and quantative monitoring and evaluation of the service

To engage effectively and participate in team meetings, practice development, supervision and forums.

To ensure compliance with all organisational policies and operational procedures including full compliance with statutory regulations, best practice and professional standards.

To ensure that excellent customer service is being delivered by the service on a day to day basis in line with service standards.

To act in accordance with the principles set out in the Employee Code of Conduct and LJWB Values.

To promote and support LJWB’s Vision: Positively Changing Lives, including our aims and objectives.
