ANNEX "A"

GUIDELINES FOR SPECIAL SITE VISIT FOR THE LIGHT RAIL TRANSIT (LRT) LINE 2 OPERATIONS AND MAINTENANCE PROJECT

I. COVERAGE AND SCHEDULE

This Special Site Visit covers certain areas of the LRT Line 2 system which are not included in the Guidelines for the Site Visit for the LR T Line 2 Operations and Maintenance (O&M) Project issued under GBB No. 10-2015 and Guidelines for the LRT Line 2 O&M Project Library Visit issued under GBB No. 18-2015 ("General Guidelines"). However, Bidders are still encouraged to continue with said Site Visits subject to the requirements stipulated in the aforementioned GBBs.

The Special Site Visit shall include nine (9) modules which cover the fo llowing allowable schedule (a detailed itinerary per module is herein attached as Annex " A"):

Each Bidder can choose up to nine (9) modules following the allowable schedule stated above for the Special Site Visit. Further, each Bidder is required to propose their own schedules for their Special Site Visit which should not exceed three (3) days. This is to ensure that due diligence and sound evaluation are considered before making the tender. Likewise, Bidders are advised to schedule the modules according to their highest priority of interest.

ll. OBJECTIVES The objectives of this Special Site Visit are the following:

1. To allow Bidders to conduct a more thorough inspection of the LRT Line 2 System for them to be able to effectively understand the concession and competitively price their bid; and

2. To enable Bidders to assess the existing conditions of the LRT Line 2 system, areas, premises and objects ofthe contract.

III. REQUEST FOR AUTHORITY TO CONDUCT A SPECIAL SITE VISIT

1. Only pre-qualified Bidders for the above-mentioned project are allowed to

participate in the site inspection, namely:

a. Aboitiz Equity Ventures, Inc. and SMRT Transport Solutions Consorti urn;

b. D.M. Consunji, Inc. and Tokyo Metro Co., Ltd. Consortium; c. Light Rail Manila Holdings 2 Consortium; and

d. San Miguel Holdings Corporation and Korea Railroad Corporation Cons01tium.

2. Bidders intending to participate in the Special Site Visit shall address their

confirmation to conduct a site visit to the Pre-qualification, Bids and Awards Committee (PBAC) through its Chairman. Requests should be accompanied by a duly accomplished Confirmation Form (Form A). Both requirements shall be submitted to the Light Rail Transit Authority (LRTA) copy furnish the Department of Transportation and Communications (DOTC) PBAC Secretariat through fax and email (details are indicated below), at least seven (7) days before the proposed schedules for the Special Site Visit to the following address/contact numbers:

3. Only formal requests duly received by the LRTA shall be given due course.

4. The LRTA, through the point person ofLRTA 2 Operations Department, shall

confirm with the Bidder its request and schedule to conduct the Special Site

Visit.

5. Bidders wishing to change their proposed schedule may do so by submitting a

formal letter fo llowing the same instructions in item 2 of this Section.

6. A maximum of fifteen (15) representatives per Bidder will be allowed to join

the Special Site Visit. In the absence of any of the representatives indicated in

Form A, alternate representatives may be allowed to join the Special Site Visit

in place of the absent representatives.

7. Bidders will choose which modules to participate in and will decide as to how

to maximize their time and indicate the same in Form A.

IV. ATTIRE

Bidders are responsible in providing the required attire during the Special Site

Visit in the following areas:

1. Required Attire for Activities in the Viaduct (Line Side), Maintenance Building and Stabling Yard: Personal Protective Equipment (PPE) safety

shoes and high-visibil ity vest;

2. Required Attire for Conducting Inspection on the Underfloor Parts of the Downed Trains Inside the Pit Area of the Maintenance Building: Safety

shoes, high-visibility vest and hard hat; and

3. Required Attire when Visiting Offices within the LRT Line 2 System: Business attire.

V. CONDUCT OF SPECIAL SITE VISIT/INSPECTION

1. Duly designated LRTA personnel will conduct/oversee the Special Site Visit

in accordance with the itinerary stipulated in Form A.

2. Right before the conduct of the inspection, all Bidders are advised to proceed first to the office of the LR T A Operations Department and coordinate with

Mr. Laigo.

3. Personnel of LRT A, including those of its contractors and service providers,

who are on-site working and operating the current system, as well as the

members of the PBAC, the Technical Working Group (TWG) and/or PBAC

Secretariat will not answer questions from Bidders. Bidders are advised to send their queries in writing and address the same to the PBAC.

4. The requested acti vity as well as the Bidder' s representatives involved therein

must not in any way impede or interfere with the normal operations,

maintenance and/or business activities of the LRT Line_ 2 System, the

activities of LRTA personnel and its contractors and service providers, and/or the normal movements of LRT passengers.

5. For lineside (viaduct) inspection, LRTA will ensure that due assistance and

protection will be provided when doing walkthrough assessment/inspection at the viaduct.

6. [n order to fac ilitate the orderly conduct of the Special Site Visit and for easy mobi li ty, Bidders are encouraged not to bring heavy or bulky materials, equipment or gadgets. The materials, equipment or gadgets in the list submitted by the Bidder will be subject to the inspection and approval of the

LR TA prior to entry in the LR T Line 2 premises.

7. For the inspection of downed and running trains, Bidder's representatives shall not tamper, handle and/or operate any train control at any time.

8. All representatives involved in the activity must log-in/log-out with the designated security personnel and wear proper identification tags at all times while inside the LRT Line 2 premises.

9. Taking of videos and photographs during the Special Site Visit is allowed, provided that these shall only be used by the Bidder and only for purposes of

the subject procurement process.

10. The no-smoking (except in designated Smoking Areas of Santolan Depot), no­eating/drinking (except in the Santolan Depot Cafeteria, and Designated Unpaid Areas of the Stations) and no-littering policy inside the LRT Line 2 premises shall be strictly observed. The Bidder shall ensure that any activity

area is clean and in order upon leaving the premises. All waste materials used

by the Bidder must be completely cleaned up and brought out of the LRT Line

2 premises by the said Bidder.

11 . The Bidder shall be responsible for the conduct of its representatives at all times. Bidders shall not hold LRTA liable for any injury or loss suffered by them or their personnel while inside the LRT Line 2 premises. However, any damage or loss suffered by LRTA as a result of the actions of the Bidder's representatives shall be charged against the said Bidder.

12. Bidders are not allowed to request for documents, records, manuals and the

like during the Special Site Visit. All documents, record manuals and the like

must be requested in writing and addressed to the PBAC, and they shall be issued through a supplemental/bid bulletin or posted in the Data Room for the

subject project.

13. Minutes of the Special Site Visit will not be prepared. Any statement made by any official, employee, personnel and/or contractor of the DOTC and/or

LRT A shall not be binding nor render any effect with respect to the subject procurement process or project unless the same is addressed or contained in a

duly promulgated supplemental/bid bulletin.
