How Aruma will help you make

a plan from the goals in your

NDIS Plan.

We will get to know you using lots

of different ways.

We will work out if there are any

risks to your safety.

We will get to know you using

focussed conversations.

all about you.

What is the About Me Tool?

The About Me Tool helps us

find out:

• What is important to you.

• What you want.

• What your hopes and

dreams are.

• What you like and do not

like.

information that you tell us when

we are getting to know you.

It helps us make a plan about

which goals from your NDIS Plan

we can help you with.

Every time we find out something

new about you we will write it

down in your About Me tool.

You can also use the About Me

tool to help when you go to your

NDIS planning meeting and

reviews.

We will tell you what supports

we can give to you.

If we can not give you

the supports you want we will tell

you where to go to get them.

We will look at the goals in your

NDIS Plan.

all your goals into a plan.

Every plan must have

• What steps we will take

to reach each goal.

• What supports you will

be given.

• How long we think it will

take to finish each step.

• Which person will be

helping you to finish each

step.

When your staff work with you

to reach your goals they will write

down what happens in your file

in the computer.

A review is when we talk to you to

see if the supports that we are

giving you are the right supports

for you to be able to reach

your goals.

We will work out with you

how often we do this.

Every year just before your NDIS

Plan Review meeting we will

look over your About Me tool

to see if anything has changed.

You can ask at any time to look

at your plan or About Me Tool.

You can change your plan

if you want to.

You can stop and think about

the support we provide you

at any time.
