Ms. New received a Bachelor of Science in Nursing from Wichita State University in Wichita, Kansas and a JD in Law from the Washburn University School of Law in Topeka, Kansas. She is a specialist in controlled substance security and DEA regulatory compliance and has made a career of assisting facilities starting and refining their drug diversion programs, with a goal of protecting patients from the harm that is frequently associated with diversion.

She is the co-founder and Executive Director of the International Health Facility Diversion Association, and a member of the Executive Board of the Partnership for Safe Medicines. Kim is a frequent speaker and author on the topic of drug diversion by healthcare personnel.

Brian H. Enochs, JD Executive Vice President, Business Development and Marketing, The Joint Commission

Brian Enochs is the executive vice president for Business Development and Marketing at The Joint Commission. In this role, Mr. Enochs oversees all business development and marketing efforts for The Joint Commission and its strategic business units, as well as External and State Relations. Prior to his current position, Mr. Enochs held executive positions with Intarcia Therapeutics, Abbvie Pharmaceuticals, and Eli Lilly & Company. In addition to his

work in domestic pharmaceuticals, Mr. Enochs led and grew business units in Malaysia and Japan including establishing Eli Lilly Malaysia as the fastest growing pharmaceutical company in the country for three consecutive years. Mr. Enochs has more than 20 years of extensive experience in the pharmaceutical industry including general management, marketing and sales, business development, strategic planning, alliance management, new product planning, and commercial analysis. Mr. Enochs received his bachelor’s degree and law degree from the University of Texas at Austin. He is a member of the State Bar of Texas.

Evelyn McKnight, AuD and Thomas McKnight, MD President of HONOReform (Hepatitis Outbreaks’ National Organization for Reform and Family Practitioner, Prairie Fields Family Medicine, Fremont, Nebraska

Dr. Evelyn McKnight is a patient safety advocate and Dr. Tom McKnight is a practicing family physician in Fremont, Nebraska. Evelyn is one of ninety-nine cancer survivors who contracted hepatitis C through unsafe injection practices while receiving chemotherapy for breast cancer in 2000-2001. Tom helped

uncover the outbreak when he diagnosed the disease in several of his patients. Evelyn and Tom were instrumental in the development and securing of funds for the One and Only Campaign, through partnerships with the CDC Foundation, the CDC, and the Safe Injection Practices Coalition. They founded the Hepatitis Outbreaks’ National Organization for Reform (HONOReform) in 2007, which educates healthcare providers and consumers about injection safety. The McKnights have been featured on a number of national news outlets including CBS News, CNN, USA Today, the Wall Street Journal and Newsday and are renowned patient safety advocates and speakers. Evelyn is also the co-author of the award-winning book A Never Event: Exposing the Largest Outbreak of Hepatitis C in American Healthcare History, which chronicles the story of the Nebraska hepatitis C outbreak. She co-authors a weekly blog (www.HONOReform.org/blog) entitled “Survivor Stories” which recounts the personal stories of individuals affected by unsafe injections and current policy and engineering efforts to prevent disease transmission due to unsafe injection practices. Evelyn received a Doctor of Audiology from Central Michigan University, Mt. Pleasant, MI, and Thomas received a Doctor of Medicine from the University of Nebraska Medical Center.

Dr. J. Hudson Garrett, Jr., is the Co-Founder and President of Infection Prevention Institute, Inc. and Assistant Professor of Medicine in the Division of Infectious Disease at the University of Louisville School of Medicine. He is also the Global Chief Clinical Officer for Pentax Medical Hoya Corporation.

Dr. Garrett holds a Doctorate of Philosophy in Healthcare Administration and Policy, Master of Science in Public Health and Nursing, and a MBA in Healthcare Management. He has attained a Post Master’s Certificate in Infection Control along with certificates or advanced diplomas in over thirty other professional endeavors. He has provided over 100 presentations across the nation and internationally and is widely published in books and peer-reviewed journals.

Dr. Garrett is a member of a large number of professional organizations including the Organization for Safety, Asepsis, and Prevention (OSAP) for which he is the Corporate Member Task Force Chairperson. Patricia Quigley, PhD, MPH, ARNP, CRRN, FAAN, FAANP Consultant, LLC

Dr. Quigley has a Doctorate in Nursing from the University of Florida and a Master’s degree in Public Health from the University of South Florida. She is both a Clinical Nurse Specialist and a Nurse Practitioner in Rehabilitation. Dr. Quigley was the Associate Chief of Nursing/Research and the Associate Director for VISN 8 Patient Safety Center of Inquiry at the James A. Haley Veterans’ Hospital in Tampa, Florida. Her contributions to patient safety, nursing and rehabilitation are evident at a national level – with emphasis on

clinical practice innovations designed to promote elders’ independence and safety. She is nationally known for her program of research in patient safety, particularly in fall prevention. The falls program research agenda continues to drive research efforts across health services and rehabilitation researchers. Dr. Quigley has published extensively in peer-reviewed manuscripts, books and media, has given over 300 presentations, and serves on a number of professional boards and committees.

Kevin Garey, PharmD, MS, FASHP Professor and Chair, Department of Pharmacy Practice, Department of Pharmacy Practice and Translational Research, University of Houston College of Pharmacy

Kevin Garey, PharmD, MS, FASHP is a Professor at the University of Houston College of Pharmacy and Chair of the Department of Pharmacy Practice and Translational Research. He is an Adjunct Professor at the University of Texas School of Public Health and a Clinical Specialist and Researcher at Baylor St. Luke’s Medical Center, Houston, Texas. He received a Bachelor of Science in Pharmacy degree from Dalhousie University in Halifax, Nova

Scotia, Canada, a Doctor of Pharmacy from SUNY Buffalo in Buffalo, NY, and a Master of Science in Biometry from the University of Texas School of Public Health. Postdoctoral training includes a pharmacy practice residency at Bassett Healthcare, Cooperstown, NY and infectious disease specialty residency and fellowship training at the University of Illinois at Chicago, Chicago, Illinois.

Dr. Garey is a member of the Infectious Diseases Society of American (IDSA) Standards and Practice Guidelines Committee and is a member of the IDSA-Society of Healthcare Epidemiology of America (SHEA) practice guidelines for C. difficile infection. He is an active member of the Society of Infectious Diseases Pharmacists (SIDP) and the American Society of Health-system Pharmacists (ASHP). He has been awarded several national awards including the ASHP Best Practice Award in Health-system Pharmacy Administration (2010), the ASHP Drug Therapy Research Award (2007), and the SIDP Impact Paper in Infectious Diseases Pharmacotherapy Research Award (2007, 2012). He received the University of Houston PLS leadership award in 2013.

Dr. Garey’s research, supported by the National Institute of Health, the Centers for Disease Control and Prevention, and the pharmaceutical industry involves clinical and translational research in healthcare associated infections including post-surgical infections, candidemia, and Clostridium difficile infection. He directs an anaerobic microbiology research laboratory that provides epidemic surveillance monitoring for C. difficile outbreaks supported by the Texas Department of State Health Services. He has published over 150 peer-reviewed articles including identification that a delay in therapy for patients with candidemia impacts mortality and identification of a genetic risk for C. difficile infection.

Vickie Gillespie, BSN, RN, CIC Preventable Adverse Events Clinical Specialist, Texas Department of State Health Services

Vickie is the Clinical Specialist for Preventable Adverse Event (PAE) Reporting at the Texas Department of State Health Services, a position she has held since January 2014. She participated in the development of the TxHSN Preventable Adverse Event Reporting Model and provides consultation, education and training to reporting facilities in Texas. She has presented extensively in Texas on the PAE program. Ms. Gillespie has worked in a number of roles in direct care, QI, patient

safety and management. She received a BSN degree from Methodist College of Nurse and Allied Health in Omaha, Nebraska, and has extensive experience in multiple hospital clinical areas, community and public health settings. For 18 years, Vickie was the Director of Community Health at Myrtue Medical Center in Harlan, Iowa, which included Home Health, Hospice, Public Health and Behavioral Health. She is certified in Infection Control and Prevention from the Certification Board of Infection Control and Epidemiology, Inc.

Bobbiejean Garcia, MPH, CIC, FAPIC Regional HAI Epidemiologist, Texas Department of State Health Services

Bobbiejean is a subject matter expert on multidrug-resistant organisms (MDROs) and infection prevention and control practices for a 16 county region, including all local health departments and healthcare facilities within those counties. She provides education and assists with public policy related to infection prevention, conducts surveillance of reportable MDROs and conducts and assists with investigations of public health significance. Ms. Garcia received

a Bachelor of Arts in Psychology from the University of Texas at San Antonio and a Master of Public Health in Health Promotion and Behavioral Science from the University of Texas Health Science Center at Houston School of Public Health. She has a Certification in Infection Control from the Certification Board of Infection Control and Epidemiology, Inc. and is also a 2018 APIC Fellow. Bobbiejean serves as the Public Health Region 6/5 South Healthcare-Associated Epidemiologist for DSHS.

Thi Dang, MPH, CHES, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Ms. Dang serves as the Healthcare-Associated Infection Epidemiologist and subject matter expert on multidrug resistant organisms (MDROs) for Public Health Region 2/3 in Texas. She received a Bachelor of Science in Health Studies from the Texas Woman’s University and a Master in Public Health degree in Epidemiology from the University of North Texas Health Science Center in Fort Worth, Texas. Thi is certified in infection control by the Certification

Board of Infection Control and Epidemiology, Inc. She also is the co-chair for the Professional Advancement Committee of her local chapter of the Association for Professionals in Infection Control and Epidemiology (APIC). Her role is specifically to assist members with preparing for their Certification in Infection Control exam. This is her 4th year in this role. Ms. Dang has provided multiple oral and poster presentations at various conferences and venues during her career including the Texas High Consequences Infectious Disease Response Conference and the Dallas Fort Worth Patient Safety Summit.

Gillian Blackwell, BSN, RN, CIC Regional HAI Epidemiologist, Texas Department of State Health Ser-vices

Mrs. Blackwell earned a Bachelor of Science in Nursing from Lubbock Christian University and an RN Diploma from Covenant School of Nursing. Gillian also earned a certification in infection control from the Certification Board of Infection Control and Epidemiology, Inc. She worked as a pediatric ICU nurse and at the Joe Arrington Cancer Center in Lubbock before joining the Texas Department of State Health Services in June 2017 as Texas Public Health Region 1

Healthcare-Associated Epidemiologist. Gillian also has experience in disaster response both in the 2010 Haiti earthquake and Hurricane Harvey. Recently, Gillian presented a poster entitled “Antibiotic Resistance Laboratory Network in Texas: Preventing, Detecting, and Responding to Emerging Threats” at the National Network of Public Health Institutes (NNPHI) Annual Conference in New Orleans.

Anna Nutt, MPH, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Anna has served as the Public Health Region 4/5 North Healthcare-Associated Infection Epidemiologist since 2016. Prior to coming to the Texas Department of State Health Services, Anna coordinated infection prevention in acute care hospitals, long-term acute care hospitals, and surgical hospitals. She has a Bachelor of Science in Biology from the University of Texas at Austin and a Master in Public Health in Epidemiology from the University of Texas Health Science

She has earned a certification in infection control from the Certification Board of Infection Control and Epidemiology, Inc. Anna has presented at several conferences and recently published a poster on “Antibiotic Stewardship Programs in Healthcare Facilities across Texas” at the first annual Houston Antimicrobial Resistance and Stewardship Conference.

Sandi Arnold, RN, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Ms. Arnold has worked in the field of infection prevention in healthcare settings for more than 20 years. She received Board Certification in Infection Prevention initially in 1986 and is currently certified. She has provided consultation and educational presentations to multiple organizations in infection prevention to assist them in improving their programs. Sandi is currently a Healthcare-Associated Infection Epidemiologist for the State of Texas with the Department

of State Health Services for Public Health Region 7. As such, she provides consultation and expertise to facilities experiencing outbreaks and management of multidrug resistant organisms.

Michael Leonard, MD Adjunct Professor of Medicine, Duke University School of Medicine; Managing Partner of Safe & Reliable Health Care, LLC

Dr. Leonard received his medical degree from the University of Missouri School of Medicine and completed a medical residency in anesthesiology at Beth Israel Hospital in Boston, Massachusetts. He is the co-founder of Safe and Reliable Healthcare and spent 20 years with Kaiser Permanente, both as a practicing clinician and leader, and 10 years as the National Physician Leader for Patient Safety. In 1999, he helped Kaiser forge a collaborative relationship

with Dr. Robert Helmreich’s Human Factors Research Project, to work on the application of human factors teamwork and communication training into healthcare.

Dr. Leonard is an Adjunct Professor of Medicine at Duke University School of Medicine. Dr. Leonard has a deep interest in culture, leadership, teamwork and reliability in diverse areas of clinical practice. He has taught extensively in high-risk areas such as surgery, obstetrics, critical care and others to enhance safety. He is a faculty member of the Institute for Healthcare Improvement, and has helped train over 2000 patient safety officers. Dr. Leonard recently collaborated on a third book on patient safety, The Essential Guide for Patient Safety Officers that was published by the Institute of Healthcare Improvement and the Joint Commission. He is also known for co-developing TeamSTEPPs; healthcare CRM; the Safety Attitudes Questionnaire [SAQ], which was the first culture assessment tool for healthcare; and its successor, the SCORE integrated survey; as well as The Mayo Clinic’s Team-based Engagement Model [TEM].

Susana Baumann, MPH, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Susana Baumann is a Healthcare-Associated Infection (HAI) Epidemiologist for the Texas Department of State Health Services in Public Health Region 9/10. Susana was a Certified Sterile Processing and Distribution Technician for 2 years, and an Infection Preventionist in Austin area healthcare facilities for 3 years before joining DSHS as a healthcare data analyst, where she became Certified in Infection Control in 2017. As an HAI epidemiologist, she works with facilities

to address issues of MDROs, outbreaks, antibiotic stewardship and reduction of device-associated infections. Susana has a Bachelor of Science degree in Zoology from North Carolina State University and a Master of Public Health in Epidemiology from the University of Texas in Austin.

Gretchen Rodriguez, MPH, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Ms. Rodriguez is the Healthcare-Associated Infection Epidemiologist for DSHS Public Health Region 8. She received a Master in Public Health in Epidemiology from the University of Texas, School of Public Health in San Antonio, and a Bachelor in Science in Industrial Biotechnology from the University of Puerto Rico in Mayaguez. She is also Certified in Infection Control from the Certification Board of Infection Control and Epidemiology, Inc. and is currently pursuing a

Doctorate in Public Health in Environmental and Occupational Health Sciences. Gretchen provides consultation, education and expertise in infection control and MDROs for healthcare facilities and organizations. Prior to employment with DSHS, she supervised the Infection Control Programs at multiple hospital settings and worked as an Infection Preventionist for Acuity Healthcare. Gretchen has provided a number of oral and poster presentations for state and national conferences.

Jennifer Vinyard, MPH, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Jennifer is the Healthcare-Associated Infections Data Team Lead for the Texas Department of State Health Services. She conducts and coordinates data validation activities and authors the Texas Annual Healthcare Safety Report summarizing the Healthcare Safety Data reported to the state. She has a Bachelor of Science in Biology from the University of Texas, Austin and a Master of Public Health in Disease Control from the University of Texas, Houston. She is

certified in Infection Control and Prevention from the Certification Board of Infection Control and Epidemiology, Inc. Jennifer played a key role in healthcare safety reporting and the implementation of the Texas Healthcare Safety Network. Prior to joining DSHS, she worked as an Infection Preventionist at various healthcare facilities around Texas and in Colorado.

Melba Zambrano, MSN-IC, CIC Regional HAI Epidemiologist, Texas Department of State Health Services

Mrs. Zambrano completed graduate school with American Sentinel in Aurora, Colorado where she earned her Master of Nursing in Infection Control. She is certified in Infection Control and Prevention from the Certification Board of Infection Control and Epidemiology, Inc. Mrs. Zambrano has served as the Texas DSHS Public Health Region 11 Healthcare-Associated Epidemiologist since 2016. Her work concentrates on collaborative efforts with healthcare facilities. She conducts ICAR

assessments, Provides TAP gap analysis, conducts MDRO investigations and assists in managing outbreaks in the healthcare setting. She is a subject matter expert providing consultation, education and training in infection control and prevention and MDROs.

Susan D. Scott, PhD, RN, CPPS, FAAN Manager of Patient Safety and Risk Management, University of Missouri Health Care System

Dr. Susan Scott received a Doctorate of Philosophy – Nursing Studies and both a Master and Bachelor of Science in Nursing from the University of Missouri-Columbia. She has worked clinically in NICU, and has experience as a legal nurse consultant and coordinator of quality care for the Department of Internal Medicine at the University of Missouri-Columbia School of Medicine. For the past 12 years she has been a leader at the University of Missouri Health Care Office of

Clinical Effectiveness, in the areas of quality, patient safety and risk management. She currently holds a Certification in Professional Patient Safety from the National Patient Safety Foundation. She has authored numerous publications in peer-reviewed journals and books on the second victim phenomenon and her research findings and application. Dr. Scott has presented across the US and internationally. She is active member of a number of professional organizations including the Agency for Healthcare Research and Quality (AHRQ) Medical Liability Communication and Resolution (MLCR) Project, Technical Expert Panel (TEP) and the Medically Induced Trauma Support Services (MITSS), Board of Directors.

Kristi Kuper, PharmD, BCPS Sr. Clinical Manager, Infectious Diseases, Center for Pharmacy Practice Excellence, Vizient

Dr. Kuper has over a decade of experience as an infectious diseases/ antimicrobial stewardship pharmacist. To date, she has worked in various capacities with over 500 hospitals nationally, ranging from 10 to over 1000 beds. She has served on a number of committees related to antimicrobial stewardship, including past CDC and Pew Trust workgroups on antibiotic stewardship, the IHI Antimicrobial Stewardship Driver Diagram development committee, the National

Quality Forum’s Antibiotic Stewardship “Playbook” and was an invited attendee at the White House Forum on Antibiotic Stewardship in June 2015. She currently serves on the Society of Healthcare Epidemiology of America (SHEA) National Antibiotic Stewardship Subcommittee and as a technical advisor for the AHRQ Safety Program for Improving Antibiotic Use. In addition, she was a co-founder of the University of Houston Infectious Diseases specialty pharmacy residency and co-founder of the Houston Citywide Antibiotic Stewardship symposium. Dr. Kuper is an active member of several pharmacy and infectious diseases related societies, including ASHP, IDSA, SHEA and SIDP. She has published over 40 articles, posters, and book chapters related to her expertise and developed a national training course in antibiotic stewardship which was launched in 2005.

Blake Hanson, PhD, MS Assistant Professor, Epidemiology, Human Genetics & Environmental Sciences, School of Public Health, University of Texas Health Science Center at Houston

Dr. Hanson received an MS and PhD in Epidemiology from the College of Public Health at the University of Iowa and completed a Postdoctoral Fellowship at The Jackson Laboratory for Genomic Medicine with Dr. George Weinstock, receiving the prestigious JAX Scholar fellowship.Dr. Hanson is currently an Assistant Professor in the Department of Epidemiology, Human Genetics and Environmental Sciences in the

School of Public Health, and the Department of Internal Medicine, Division of Infectious Diseases, and serves as the Associate Director of Microbial Genomics in CARMiG.

Dr. Hanson’s research interests are in infectious disease transmission and colonization, how microbial communities impact the development of disease, and how antimicrobial resistance develops and transmits through society. He uses a combination of existing and innovative laboratory techniques, and cutting-edge sequencing and bioinformatics within his lab’s research.
