Which Beautyrest products are included in the event?
Consumers qualify for a Theme Park Ticket Rebates Booklet by purchase of the following items during the promotional period:
Beautyrest ® mattress and matching foundation or RENEW ® adjustable foundation
(minimum purchase of $799*)
*not to include taxes, delivery fees, warranty agreements, non-Beautyrest ® products or other charges.Note: Consumer must be 21 years of age to
receive the Theme Park Ticket Rebates Booklet.
What has to happen for the customer to get the Theme Park Ticket Rebates Booklet ?
Here are the steps(continued):
1. Customer makes purchase. Store provides Customer a flyer explaining how to go about getting their Theme Park Ticket Rebates Booklet. Stores will be sent flyers to be given customers at time of sale. If, for any reason, flyers are not available, it is possible to go to the Current News page at www.fmgbuyinggroup.com/currentnews.aspx to download and print a copy of the flyer which can be given to the customer. (No password is required to access this page.)
Consumers may qualify for more than one by purchasing more than one complete mattress and foundation set. One per complete set. There is no limit on the number of per customer.

2. Record of qualifying sale.At the conclusion of the event, each participating retailer must provide to FMG a listing of all Customers that have made a qualifying purchase during the event.This simple spreadsheet requires a modest amount of information--name, address, phone, retailer, branch, date and items purchased.
Go to http://www.fmgbuyinggroup.com/Currentnews.aspx to view and download the “Beautyrest® Theme Park promo” spreadsheet.
Only customers with a DELIVERY COMPLETION DATE indicated are eligible to receive a gift card. The last date to send qualifying customer lists is August 31, 2015. The sale must be completed and FMG must be notified prior to that date.
What has to happen for the customer to get the Theme Park Ticket Rebates Booklet?

3. Purchased items are delivered to the Customer. Store notifies FMG using the “Beautyrest® Theme Park promo” spreadsheet (see prior page) that the Customer should be sent a Theme Park Ticket Rebates Booklet. FMG notifies Capitol Marketing that the Theme Park Ticket Rebates Booklet is to be released to the Customer.
Note: Stores will not receive Theme Park Ticket Rebates Booklets. Booklets will be sent directly to consumers.

How to Use Your Theme Park Ticket Rebates Checks Read All Terms and Conditions found in your Theme Park Ticket Rebates
Booklet to best understand how to take full advantage of your savings. Visit most major theme parks within the 48 contiguous United States and
purchase your one-day, single-park tickets for you and your family members. (Multi-park tickets or Season Passes do not qualify).
IMPORTANT: Remember to save your receipt and original tickets! Submit your dated receipt and original tickets along with an enclosed
rebate check that provides you with the biggest rebate value within 30 days of the date you purchased your tickets.
You will receive your rebate in the form of a prepaid single-use Debit Card. You will be given the option to print the card virtually on demand or you can select to have the actual plastic card mailed to the home address provided.
Important: Before mailing in your Theme Park Ticket Rebates Checks, please fill in the Official Book Registration Number (located on the back cover) on each check, in addition to the other required information.
NOTE: Only one rebate check may be submitted at a time and only one rebate check may be submitted within a 30-day period. All terms and conditions must be followed, if not this offer is considered null and void and has no cash value.

Here are the steps (continued):4. Capitol Marketing provides a Theme Park Ticket Rebates Booklet by mail directly to the Customer. FMG and Retailer are notified by update on the “Beautyrest® Theme Park promo” spreadsheet that the Theme Park Ticket Rebates Booklet has been sent.
• There is no charge to the consumer or to FMG member retailers for the Theme Park Ticket Rebates Booklet .
• FMG member retailers do not have to handle the Theme Park Ticket Rebates Booklets.
• Stores are only required to provide the list of qualifying customers. Theme Park Ticket Rebates Booklet will be sent to the customer.

More DetailsCustomers will receive a Theme Park Ticket Rebates Booklet from Capitol Marketing/Meridian upon completion of the sale. Stores will submit lists of qualifying customers to FMG/Capitol Marketing. Offer valid for purchases made between May 14, 2015 and June 2, 2015 only. Prior sales excluded. There is no limit on the number of Theme Park Ticket Rebates Booklet for which a customer may qualify. One Theme Park Ticket Rebates Booklet per each complete qualifying Beautyrest® mattress and foundation set purchased. Upon notification by the participating store via FMG Capitol Marketing/Meridian will process the request and mail the Theme Park Ticket Rebates Booklet directly to the consumer in 2 to 4 weeks. Once issued a Theme Park Ticket Rebates Booklet can not be withdrawn. In the case of a return or cancellation where a Theme Park Ticket Rebates Booklet has been issued, the value of the Theme Park Ticket Rebates Booklet issued should be deducted from any refund by the retailer. Theme Park Ticket Rebates Booklet certificates are completely transferable, but may not be resold under any circumstances.
Theme Park Ticket Rebates Booklet requests must be submitted to FMG/Capitol Marketing by stores by August 31, 2015. All taxes are the responsibility of the person receiving the redemption.
Theme Park Ticket Rebates Terms & Conditions• This Rebate offer is good for most major theme parks in the 48 contiguous states within the continental United
States. Offer is not valid for Alaska and Hawaii.• A Rebate check may only be used for each particular park once during the 12 month redemption period of your
booklet. Rebate checks may not be combined and used for the same day.• Rebates checks are good for single-day, single-park tickets only. No multi-park tickets or season passes are
allowed for this rebate offer. Only one rebate check may be submitted at a time and only one rebate check may be submitted in a 30 day period.
• You must submit your rebate check along with a dated receipt and original tickets within 30 days of the date of the receipt/tickets to qualify for the rebate.
• You must be over 21 years of age to submit for this rebate offer.• The rebate amount is up to the value listed on the rebate check for the specified amount of tickets. The rebate
amount will not exceed the amount of the rebate check, or the amount of the actual cost of the tickets, whichever is the lesser amount. (Examples: If 2 people spend $100 on 2 tickets, they are eligible for the full value of the rebate check of $48 for 2 people purchasing 2 tickets. If 2 people spend $40 on 2 tickets, then their rebate will be the full amount of the tickets which is $40, instead of the full potential rebate of $48.)
• This offer is not valid for tickets purchased on any major holiday.• This rebate offer excludes dinner shows, dinner theaters, and circus type shows. It is valid only for major theme
parks consisting of rides and amusements.• This rebate offer is not endorsed or sponsored by any theme park or related ownership. Nor does this program
specifically endorse the use of any specific theme park. The choice of theme park visited is solely up to the discretion of the rebate booklet recipient with any limitations specified in the terms and conditions of this offer.

How is the event being supported?
Vinyl 24 x 62.5" banner signs have been ordered for and sent to all FMG stores displaying at least one Beautyrest® mattress and foundation set.
The banners have grommets so that they can be used on X-stands, mounted to a wall, or hung from the ceiling.
The banners are being provided to FMG member retailers at no charge.
