JobDescriptionHIVMedicalCaseManager

TheHIVMedicalCaseManagerpositionisafull-timepositionrequiring37.5hoursperweek.TheMedicalCaseManagerwillworkcloselywithclientslivingwithHIV/AIDSwhohavemultiplepsychosocialand/orhealthrelatedneeds.MedicalCaseManagementservicesisastrength-basedapproachtoservicethatincludestreatmentadherencecounseling,coordinationandfollow-upofmedicaltreatments,clientadvocacy,andassistanceinobtaininghousing,financialsupport,legalservices,socialsupportandanyotherneededservice.ThegoalsofMedicalCaseManagementistoinsurethatclientswithHIV/AIDShavetimelyaccesstocomprehensivemedicalcareandsocialservices;preventdiseasetransmissionanddelayofHIVprogressionandtopromoteandsupportclientindependenceandself-sufficiency.TheMedicalCaseManagerwillbesupervisedbytheClinicalSupervisor.Minimumeducationrequirementisabachelor’sdegreeinahumanservicefield.PriorworkingexperiencewiththeHIV/AIDScommunityispreferred.KeyPerformanceArea:Screening

1. WorksincollaborationwiththeServiceAccessCoordinatorandClinicalSupervisortoidentifyclientsatintakethatareinneedofMedicalCaseManagement.

2. CollaborateandConsultwithClinicalStafftoidentifycurrentclientswhoareinneedofmedicalcasemanagementservices.

3. WorkswithagenciesintheHIV/AIDSservicesystemtoengageindividualsinfectedwithHIV,increasingthenumberofclientsenrolledincareatPERSADCenter

4. Respondwithin24hourstorequestsforcasemanagementservices.5. Scheduleascreeningtimewithin5daysoftheinitialrequest.6. Completethescreeningtodetermineeligibilityformedicalcasemanagementservices.7. ProvidebackuptoPERSAD’sHIVTestingprogram.

KeyPerformanceArea:OrientationandAssessment

1. Provideanoverviewofcasemanagementservicesincludingtheroleandresponsibilityofthecasemanagerandtheclient;theagency’sgrievanceproceduresandtheCaseManagementAgreement.

2. Completionofthecomprehensiveassessment,within30daysofinitialscreening,todeterminetheclient’sstrengths,resources,needsandproblems.

3. WorkwithclienttodevelopanindividualServiceCoordinationPlanwhichincludesrealistic,measurableandmutuallyacceptablegoalsthatarebasedontheresultsoftheassessment.

4. Identifyactionstepsneedstoachieveeachgoal,includingtargetdatesforaccomplishmentofthestatedgoals.

5. Identifyreferralsmadetootherproviders/servicesinconnectionwithactionsteps.6. Obtainclientandcasemanager’ssignaturesonServiceCoordinationPlan.7. Copyplanandgivetoclientandfileoriginalinclient’sfile.

1. AttendandparticipateinHIVCollaborativeMeetingsasscheduled.2. EnrollMedicalCaseManagementclientsinPERSAD’sCCNPharmacyprogram.3. Identifyandcontactcommunitybasedorganizations,primarycareproviders,housing

servicesandotherneededproviderstoestablishreferralagreementsandtocoordinatecollaborativeeffortswithclients.

4. Completealldocumentationtoshowevidenceofthetime,date,placeanddescriptionofeachcasemanagementservice.

5. DocumentprogressmadetowardgoalsbywritingnotesintheData/Assessment/Planformat(DAP).

6. Completeallrequiredbillingandtrackingforms.KeyPerformanceArea:ContinuedContact

1. Facetofaceinteractionwithclientatleastevery90days.2. CompletereviewofServiceCoordinationPlanevery90days;makinganychanges,

additionsordeletionstocurrentservices.3. ObtainclientssignatureonServiceCoordinationPlanreviewindicatingtheagreement

forcontinuedcontactandcasemanagementservices.4. Developinterventionplantore-engageclientifface-to-facecontactisnotmaintained

andincludetheinterventionintheServiceCoordinationPlan.5. Documenttreatmentadherenceactivitiesinclientsfile;theseincludekeepingmedical

appointments,takingprescribedmedication,refillingprescriptions,etc.6. Meetweeklyutilizationrequirementof24hoursofclientcontact.7. DocumenttheCD4count,resultsofviralloadtest,orprescribedARTmedicationsevery

180days(every6months).8. CompletereassessmentannuallyincludingthedevelopmentofanewService
