• Selection is limited to the items listed and is subject to availability. Items may change.

• Unused amounts do not roll over to the next quarter.

Your Leon Medicare plan includes an over-the-counter (OTC) benefit that allows you to choose medications and health products on this list. You may order items for a total of $150 each calendar quarter. Unless indicated, there is a limit of 5 units of the same item per order. Unused benefits may not be transferred to the next quarter.

Unless indicated on the specific item, you may order up to five (5) units of any single item. Blood Pressure Monitors are limited to one per year.

WHEN ORDERING

All orders must be placed by 5:00 p.m. on the last day of the quarter.
