1 Authority 1.1 Issuance of Certificates and Documents

The Maritime Administrator and Deputy Maritime Administrators are authorized to issue certificates and

documents to Dominica merchant marine personnel.

(Dominica International Maritime Act 2000 (IMA 2000) Section 11; Dominica Maritime

Regulations 1.4 and 7.30).

1.2 Establishment of Standards

The Maritime Administrator is authorized to establish such standards as are necessary and proper for

certificating and upgrading of merchant marine personnel and for the maintenance of high standards in

the Commonwealth of Dominica Merchant Marine.

(See IMA 2000, Section 11; Dominica Maritime Regulation 1.4). 1.3 Compliance with the STCW Convention, as amended in 1995

The standards established pursuant to Item 1.2 comply with the provisions of the International Convention

on Standards of Training, Certification and Watchkeeping for Seafarers, 1978, as amended in 1995

(hereinafter referred to as STCW 95). Note The Dominica International Maritime Act 2000, Maritime Regulations, and Marine Safety Circulars referred to in this document may

2 General 2.1 Background

In 1993, the International Maritime Organization (IMO) embarked on a comprehensive revision of the

Standards for Training Certification and Watchkeeping - 1978 (STCW 78) to establish the highest

practical standards of competence for mariners and to reduce human error as a major cause of marine

casualties. On 7 July 1995, a conference of Parties to STCW adopted a package of amendments to the

Convention, which establishes requirements for qualification of masters, officers, watchkeeping and other

crew personnel on seagoing merchant vessels operating outside the boundary line and the

responsibilities of companies. Dominica acceded to the Convention on 21 June 2000.

2.2 Maritime Training and Assessment Program

2.2.1 The Commonwealth of Dominica has a comprehensive marine personnel program that implements

STCW 95 and is embodied in the Commonwealth of Dominica International Maritime Act 2000 (IMA 2000)

and the Commonwealth of Dominica Maritime Regulations.

2.2.2 The Commonwealth of Dominica’s program has a dual commitment to maintain the high standards

of the international treaties that affect the mariner and a commitment to “Honor the Mariner.” Its program

has been developed to ensure that all Dominica certificated masters, officers and ratings will be educated

and trained to the levels of knowledge and competence equal to or better than those agreed

internationally.

2.2.3. All maritime academies, marine education and training facilities and training providers approved by

the Commonwealth of Dominica must meet or exceed all standards prescribing the achieved learning

objectives, training outcomes, ability, skill proficiency and competency assessed as defined by the STCW

Code.

2.2.4 In keeping with its commitment to “Honor the Mariner” and to fully implement the treaty, Dominica

uses a number of mariner assessment and certification options contemplated in the Convention for

mariner training, certification, and advancement. It is Dominica’s belief that the program will benefit the

mariner because it provides various options for training, assessment certification and licensing.

2.2.2.5. The Maritime Administrator and Deputy Maritime Administrator are authorized to issue certificates

and documents to all merchant marine personnel. (

2.2.6 Establishment of Standards - The Maritime Administrator is authorized to establish such standards as

are necessary and proper for certificating and upgrading of merchant marine personnel and for the

maintenance of high standards in the Dominica Merchant Marine.

2.2.7 Suspension an Revocation of Certificates - Any certificate may be suspended or revoked at any time

by the Maritime Administrator or Deputy Maritime Administrator on the grounds set forth in Dominica

Maritime Regulations and in accordance with the procedure set forth in the Procedures for Marine

Investigations and Hearings.

2.2.9 Dominica STCW 1995 certificates specify function, level, and limitation and give the appropriate

STCW 95 reference cite with respect to the regulation(s) under which the mariner has been found

qualified. The function term is not included when the function regulation(s) cite is given on the certificate.

This is the preference that will be used.

2.3 Dominica Mariner Assessment and Certification Options The Commonwealth of Dominica uses the following approaches for STCW certification:

2.3.1 Certificate of Endorsement - in accordance with regulation I/10 of the STCW Convention; a person

holding a valid certificate that has been verified, issued by a Party fully applying the provisions of STCW

95, and who is not otherwise disqualified in the opinion of the Maritime Administrator or Deputy Maritime

Administrator to whom application is made shall be eligible for an equivalent Dominica STCW 95

Certificate, subject to proof of physical fitness and adequate communications skills. The Maritime

Administration will reserve the right to require examinations if it determines, for whatever reason, that

further assessment is necessary. Selective use of examinations is part of the Registry quality standards

program.

2.3.2 Certificate of Competence – in accordance with regulation I/6 (Approved Training and Assessment) of

the STCW Convention. Approved Training and Assessment Programs – Candidates using the approved

training and assessment approach to obtain certificates as deck officer, engineer officer and radio

operator are required to have completed approved education and training in the traditional classroom

setting, training institution, or maritime academy which is structured in accordance with written programs,

including such methods of media delivery, procedures, and course materials as are necessary to achieve

the prescribed standard of competence; and conducted, monitored, evaluated and supported by qualified

persons. The Administration will require a written examination of all such candidates. Use of examinations

is part of the Registry training institution quality assessment program. Mariners entering into the work

force must begin by successfully completing the Basic Safety Training program. Varying courses must be

completed for different upgrades (see Marine Safety Circulars for reference).

2.3.3. Candidates may participate in the following approved programs:

In-Service Training- Candidates using this approach to obtain STCW certificates as deck officer, engineer

officer and radio operator are required to have completed an approved “in-service” program of training,

assessment of competence as prescribed in Section A-I/6 of the STCW Code while either on-board ship

or ashore. The Maritime Administration will allow in-service training and assessment programs if

organizations use the Administration provided Training Record Book and receive approval of their

program through the Administration or a “Recognized Organization.” The Administration will require a

written examination of all such candidates. Use of examinations is part of the Registry quality assessment

program.

Training and Assessment within an Institution- Candidates using this approach to obtain STCW certificates

as deck officer, engineer officer and radio operator are required to have completed an approved program

within an Institution that has been recognized to deliver approved courses of training. The Maritime

Administration will approve certificates of training from institutions who have been approved or accepted

by the Administration and have been found to conduct training and assessment with approved instructors

and assessors and that have incorporated the quality standard provision of Section I/8 of the Code. Such

qualification, experience and application of the quality standards shall incorporate appropriate training in

instruction techniques and training and assessment methods and practice. The Administration will require

a written examination of all

such candidates. Use of examinations is part of the Registry quality assessment program.

2.3.4 Recognizing that some mariners and companies may select the more traditional maritime

training avenue through a recognized training institution, Dominica has entered into an agreement with a

Recognized Organization, Det Norske Veritas, to evaluate and accredit training organizations and

courses. Other such institutions can be recognized upon request and qualification. The Office of the

Deputy Administrator for Maritime Affairs and Marine Personnel also will provide course approval,

instructor approval and training institution approval under prescribed quality standards. Fees will be

applied for review and approval procedures, as well as any costs associated with travel, housing and per

diems for inspection.

2.3.4 Dominica will issue STCW 95 certificates as endorsement certificates (endorsements) by

recognizing valid certificates issued by Parties on the IMO list and as original certificates of competence

(certificates). Both will reflect the categories called out in Chapters II through V. The Dominica STCW 95

certificates call out the function regulation cites, capacity, level and limitations.

2.3.5 Although Dominica will issue STCW 95 certificates, either by endorsement or original, as a service

to mariners and shipping companies who, in the near term, still wish to use traditional seafarer grade

terms (i.e., Master, Chief Mate, Second Mate, Third Mate, etc.) within their internal management system,

Dominica also, in the near term, will issue licenses to those mariners who request them. We anticipate

that with time, the traditional terms of hierarchy will not be needed as the STCW terms of “Capacity”

become more familiar as industry uses the new STCW 95 system. The traditional licenses will have

entries giving STCW 95 reference terms and the applicable STCW regulation cites as appropriate.

2.3.6 Certificates will not be issued as a dispensation in compliance with Article VIII of the STCW

Convention 95.

2.4 Certificates

2.4.1. Certificate format – STCW 95 certificates are issued in the STCW 95 format and are printed on

“fraud-proof” security paper to prevent tampering.

2.4.2. Dominica STCW 1995 certificates specify function, level, and limitation and give the appropriate

STCW 95 reference cite with respect to the regulation(s) under which the mariner has been found

qualified. The function term is not included when the function regulation(s) cite is given on the certificate.

This is the preference that is used. The following STCW categories are used and the regulations cites are

per SCTW 95 for the required qualifications:

2.5 Licensing

2.5.1 Licenses using traditional officer terms will be issued to those requesting them in addition to the

SCTW 95 Certificates. Ratings will have the traditional term noted on their Seaman Identification Card.

Qualification for a STCW 95 certificate will be a prerequisite for such licenses, other than for a domestic

license.

2.5.2 Such licenses are issued as explained below with the appropriate officer grade, route, tonnage,

mode of propulsion, kW propulsion properly inserted as necessary. Officer licenses are printed on “fraud-

proof” security paper to prevent tampering. Such licenses will specify STCW 95 function, level, and

limitation and give the appropriate STCW 95 standards section reference citation(s). The traditional terms,

shown in 2.9.4, are used and the SCTW 95 function, capacity, and level are cited on the license.

2.5.3 Grades:

• Four (4) grades of deck officer licenses are issued - Master, Chief Mate, Second Mate and Third

Mate.

• Four (4) grades of engineering officer licenses are issued – Chief Engineer, First Assistant

Engineer, Second Assistant Engineer and Third Assistant Engineer. These licenses are issued

for “steam,” gas turbine, and/or “motor” service and usually with no kW propulsion limitation.

• Four (4) grades of GMDSS radio licenses are issued. This Administration requires two deck

officers to hold General Operator - GMDSS certificates on a GMDSS equipped oceangoing

vessel without a dedicated GMDSS 1st or 2nd Class Radio Operator and Maintainer on board.

The foregoing table gives the typical levels of certification or licensing and is used as a tool for defining

the crossover between traditional and STCW levels of competence. 2.6 Application

2.6.1 Those persons desiring a Dominica STCW 95 certificate, training record book, or seaman

identification document must apply using the applicable form together with the required documentation.

Those wishing a Dominica license in addition to the STCW 95 certificates should so indicate on the form.

Sample forms are attached in Annex 1 and may also be downloaded from the Dominica Registry web

page found at: www.dominica-registry.com. Copies of the form will be provided upon request.

2.6.2 Certificates of Receipt of Application will be issued as temporary three month authority for an officer

to serve in the capacity on the basis of an appropriate and valid certificate while his or her approved

application for a Dominica certificate is being processed. This certificate will be issued to those officers

holding acceptable national STCW 1995 certificates from countries that have been placed on the “White

List” by the Maritime Safety Committee of IMO.

2.6.3 Any shipowner or submittal agent who submits on behalf of an applicant an application for a

Commonwealth of Dominica Certificate, or Seafarer’s Identification Document and/or Training Record

Book, which application contains a willful misstatement of fact or references a document which is forged,

altered, fraudulent or fraudulently obtained, the truth or genuineness of which is capable of being

determined by the submitting party, will be subject to an administrative penalty of $500 for each such

application. Further submissions from such source may be barred until two (2) years after such penalty is

remitted in full.

2.7 Definitions For the purpose of these requirements, the following terms shall have the meanings hereby assigned to

them:

2.7.1 Master - the person having command of a vessel.

2.7.2 Navigational officer in charge of a watch - any person, as the Master’s representative, who is primarily

responsible at all times for the safe navigation or maneuvering of the vessel and for complying with the

International Regulations for Preventing Collisions at Sea, 1972.

2.7.3 Chief Engineer - the senior engineer officer responsible for the mechanical propulsion and the

operation and maintenance of the mechanical and electrical installations of the vessel.

2.7.4 Engineer officer in charge of a watch - any person, as the chief engineer officer’s representative, who

is primarily responsible at all times for the safe and efficient operation and upkeep of machinery affecting

safety of the vessel and is responsible for inspection, operation, and testing of such machinery.

2.7.5 Near Coastal Voyage - a voyage exclusively within 200 miles of the coast of Dominica. Officer

certificates of competency for “Near Coastal Voyages” will have stated geographical limits.

2.7.6 Radio operator - a person holding an appropriate radio Certificate issued under the provisions of

Dominica Maritime Regulation 4.1(3).

2.7.7 Rating - a member of the ship's crew other than the Master or an officer.

2.7.8 Oil tanker - a ship constructed and used for the carriage of petroleum and petroleum products in

bulk.

2.7.9 Chemical tanker - a ship constructed or adapted and used for the carriage in bulk of any liquid

product listed in Chapter 17 of the International Bulk Chemical Code.

2.7.10 Liquefied gas tanker - a ship constructed or adapted and used for the carriage in bulk of any

liquefied gas or other product listed in Chapter 19 of the International Gas Carrier Code.

2.7.11 Passenger ship - a ship that carries more than twelve (12) passengers.

2.7.12 Ro-Ro passenger ship - a passenger ship with ro-ro cargo spaces or special category spaces as

defined in the International Convention for the Safety of Life at Sea, 1974, as amended (SOLAS).

2.8 Officer Certificate Required 2.8.1 No person shall perform the duties of Master, navigational officer in charge of a watch, chief

engineer, or engineer officer in charge of a watch of any vessel registered under the provisions of the IMA

2000, unless duly issued a certificate by the Dominica Maritime Administration (MARAD). (Maritime

Regulations Chapter 7)

2.8.2 The operation of every Dominica ship GMDSS station shall be performed only by an operator duly

issued a certificate by the MARAD, as required by Dominica Maritime Regulation 4.1(3)(a).The holder of a

certificate as GMDSS Operator l may serve as radio operator aboard vessels requiring the services of

either a First Class or Second Class Radiotelegraph Operator.

2.8.3 The operation of every ship radio telephone or ship earth station, and of every automatic

communication device installed in a ship station, shall be controlled by an operator duly certificated by the

MARAD, as required by Dominica Maritime Regulation 4.1.3.b and .d.

2.8.4 No person shall serve in any capacity higher than that for which he is duly certificated except by

force majeure, and no person shall be deemed duly certificated unless the the Maritime Administrator or a

Deputy Maritime Administrator has issued him or her an officer certificate of competence.

2.8.5 Legal and Regulatory Requirements – No person may perform the duties of Master, navigational

officer in charge of a watch, chief engineer, or engineer officer in charge of a watch of any vessel

registered under the provisions of the IMA 2000, unless duly issued a certificate by the Maritime

Administrator or a Deputy Maritime Administrator.

2.9 Period of Validity; Suspension and Revocation of Certificate Each officer certificate shall be valid for a period of five (5) years and shall be subject to renewal under

the conditions set forth in these requirements.

2.9.1 Suspension or Revocation - Any officer certificate may be suspended or revoked at any time by the

Maritime Administrator or Deputy Maritime Administrator for the causes set forth in Maritime Regulation

1.4.4, and in accordance with the procedure set forth in the Rules and Procedures for Marine

Investigations and Hearings (IMA 2000, Section 12).

2.9.2 Surrender of Suspended or Revoked Certificate - The holder of an officer certificate, which is

suspended or revoked, shall promptly surrender it as directed by MARAD.

2.9.3 Display of Certificate or Temporary Permit - Every officer shall cause his certificate or temporary

permit to be displayed in a conspicuous location as required by Maritime Regulation 7.58.

2.10 Form of Application

2.10.1 Form CDMP-5000 - Each application for an officer certificate shall be submitted on official Form

CDMP-5000 (Annex 1), provided by MARAD, and shall be accompanied by all necessary proofs,

supporting data and fees prescribed in the fee schedule.

2.10.2 Certificate of Receipt of Application (CRA) - When an application for a Dominica Certificate is

submitted to MARAD, the applicant may request issuance of a CRA, Form CDMP-3022. The application

must be accompanied by the fee prescribed in the fee schedule. If the application and proof of foreign

certificate are in proper form and legitimate, the applicant will promptly be issued an official CRA in the

grade for which applied as follows:

.1 Each applicant for an officer certificate who qualifies based on recognition of a national

certificate under the provisions of STCW 95 will be issued a CRA on Form CDMP-3021.

.2 While holding the issued CRA together with his or her original valid foreign certificate the

applicant may serve aboard Dominica flag vessels for a period up to 90 days while the application

for a Dominica Certificate is being processed.

2.11 Form of Certificate

2.11.1 STCW 1995 Certificates - Each applicant for an officer certificate who qualifies under the

provisions of STCW 95, will be issued a certificate on either:

.1 Form CDMP-3023 - for officers qualifying based on a Dominica examination; renewals of

existing Dominica certificates, or

.2 Form CDMP-3024 - for officers qualifying based on recognition (“endorsement”) of a national

certificate.

.3 Both certificates state that the officer has been found duly qualified in accordance with the

provisions of the STCW Convention, 1978, as amended in 1995, and in the format prescribed by

STCW 95.

2.11.2 STCW 1995 Special Qualifications - Each applicant for a special qualification certification who

qualifies under the provisions of STCW 95, will be issued a certificate on either:

.1 Form CDMP-3023 - For seafarers qualifying for certification of special qualifications based on a

Dominica examination; renewals of existing Dominica certificates, or

.2 Form CDMP-3024 - For seafarers qualifying for certification of special qualifications based on

recognition (“endorsement”) of a national certificate.

2.11.3 Format - Both STCW 95 Certificates specify the “functions” the officer has qualified for and at

which function, capacity, and “level of responsibility” as follows:

.1 Functions - The abilities specified in the standards of competence are grouped as appropriate

under the following seven functions. The function is delineated in the certificate by reference to

the STCW 95 regulatory cite. Limitations are also specified:

.1 Navigation

.2 Cargo handling and stowage

.3 Controlling the operation of the ship and care for persons on board

.4 Marine engineering

.5 Electrical, electronic and control engineering

.6 Maintenance and repair

.7 Radio communications.

2.11.4 Officer Qualifications for STCW 1995 Certification

.1 Each Deck/Navigational Officer must qualify in functions 1, 2 and 3 (deck). A

Deck/Navigational Officer may then qualify in the additional functions of 3 (engineer), 4, 5, 6, and

7.

.2 Each engineer officer must qualify in functions 3 (engineer), 4, 5, and 6. An engineer

officer may then qualify in the additional functions of 1, 2, and 3 (deck).

.3 A radio operator must qualify in function 7.

2.12 General Requirements for Officer Certificate

2.12.1 Age - No certificate shall be issued to any person under 18 years of age. No certificate in the grade

of Second Mate, Second Assistant Engineer or higher, and no certificate in the grade of Radiotelegraph

Operator First Class, or Radio Communication Operator General, shall be issued to any person under 21

years of age. A candidate shall be required to submit satisfactory proof of age by means of a passport,

birth certificate, certificate of naturalization or other acceptable proof of birthdate.

2.12.2 Citizenship - A candidate for any certificate may be of any nationality and need not be a citizen or

resident of the Commonwealth of Dominica.

2.12.3 Physical Examination - Every candidate for a Certificate shall be required to submit satisfactory

evidence of a physical examination completed within the 12 months immediately preceding application.

The report of examination shall establish that the candidate is in good physical condition and in

possession of all body faculties necessary to fulfill the requirements of the seafaring profession. The

report and certificate of physical examination is to be completed either on the official Physical

Examination Report/Certificate, Form CDMP-3033 or on the official form of a country that is party to ILO

Convention No. 73 on Medical Examination of Seafarers. The report shall specifically indicate that the

candidate was examined as to hearing, vision and general physical condition. The following minimum

requirements shall apply:

.1 Hearing - Every candidate must have hearing unimpaired for normal sounds and be capable of

hearing a whispered voice in the better ear at 15 feet and in the poorer ear at five (5) feet.

.2 Eyesight -Deck/ Navigational Officer - A candidate must have (either with or without glasses) at

least 20/20 vision in one eye and at least 20/40 in the other. If the candidate wears glasses, he

must have vision without glasses of at least 20/160 in both eyes. The candidate must also have

normal color perception, and be capable of distinguishing the colors red, green, blue and yellow.

Engineer or Radio Officer - A candidate must have (either with or without glasses) at least 20/30

vision in one eye and at least 20/50 in the other. If the candidate wears glasses, he must have

vision without glasses of at least 20/200 in both eyes. An engineer or radio officer candidate shall

be examined only as to perception of the colors red, yellow and green.

.3 Blood Pressure - A candidate’s blood pressure must fall within a range average for his age.

.4 Symptoms - A candidate exhibiting symptoms likely to indicate any of the following diseases or

conditions shall be prima facie disqualified: epilepsy, acute mental illness or other lack of mental

capacity or stability, tuberculosis, acute venereal disease or neurosyphilis, AIDS, abuse of alcohol

and/or the use of narcotic substances whether or not prescribed by a physician.

.5 Speech - A deck/navigational or radio officer candidate must have speech unimpaired for

normal voice communication.

Willful concealment by a candidate of any medical history or mental or physical disability that might render

him unfit for service shall be cause for the immediate revocation of any certificate that may have been

issued to him.

2.12.4 Photographs - Every candidate for a Certificate shall submit three (3) passport- size photographs

of himself, taken within the 12 months preceding the date of application, and shall affix his legible

signature on the reverse side of each. The photographs must show a facial front view of the applicant,

and be 2 in. x 2 in. (50 mm. x 50 mm.) in size; the photographs may be black-and-white or color.

2.12.5 Advanced Firefighting - Every candidate for a Master or Mate’s certificate, or an engineer officer

certificate for service on vessels of over 750 kW propulsion power must submit proof of completion of an

advanced marine firefighting course approved by the MARAD.

2.12.6 Radar Simulator Training - Every candidate for a Master or Mate’s certificate must submit proof of

completion of a marine radar simulator course approved by the MARAD.

2.12.7 GMDSS Certification - Every candidate for a Third Mate/Second Mate/Mate’s certificate must hold

GMDSS certification.

2.12.8 First Aid Training - Every candidate for a Second Mate/Third Mate’s certificate on vessels over 500

gross tons; every candidate for a Master/Mate’s certificate on vessels less than 500 gross tons and every

candidate for an engineer officer’s certificate serving on vessels of over 750 KW propulsion power must

complete first aid training.

2.12.9 Survival Craft/Rescue Boat Training - Every candidate for a Master, Chief Mate, Second Mate,

Third Mate or Mate’s certificate and every candidate for an engineer officer’s certificate on vessels of over

750 kW propulsion power must complete “Proficient in Survival Craft/Rescue Boats” training.

2.12.10 Automatic Radar Plotting Aids (ARPA) Training - Every candidate for a Master, Chief Mate,

Second Mate, Third Mate or Mate’s certificate must complete Automatic Radar Plotting Aids (ARPA)

training.

2.12.11 Advanced Medical Training - Effective 1 August 1998, every candidate for a Master or Chief

Mate’s certificate on vessels over 500 gross tons must complete advanced medical training.

2.12.12 National Certificates containing STCW Endorsements that are acceptable to MARAD shall be

considered evidence of completion of the training requirements of sub-items 2.10.6 through 2.10.11

above without having to submit training course certificates.

2.13 Fees 2.13.1 Schedule - Any applicant for a certificate or other document must remit the required fee in

accordance with the fee schedule Should secure delivery of certificates by courier service be desired, a

fixed fee of U.S. $35 per certificate must also be remitted with the application.

2.13.2 Payment

.1 Checks/Money Order: All fees remitted by check or money order shall be in U.S. Dollars,

drawn on a bank or branch located in the United States and made payable to Dominica Maritime

Registry Inc.

.2 Cash - Candidates must not remit fees in cash.

.3 Mailing to us - The use of overnight or courier services is recommended. “Registered mail,”

though safe, is one of the slowest methods of sending documents. When not applying in person,

the application(s) with payment should be sent by overnight or courier service instead of

registered mail.

.4 Mailing from us - If applicants desire expedited delivery, they or their personnel document

submittal agents are strongly urged to enclose, with applications, an additional $35 per SID and

Training Record Book application and $35 per officer certificate (license) application for return

courier delivery. Special qualification certificates will be sent via courier along with SID and

original Training Record Books at no additional charge.

2.14 Fraud, Forgery, Misrepresentation or Attempted Bribery

2.14.1 Any person who submits a fraudulent application for a Commonwealth of Dominica Certificate,

Seafarer’s Identification Document or Training Record Book, or who makes misrepresentations in or in

connection with an application, or who attempts to bribe or subvert any person charged with the approval

of applications and/or conduct of examinations shall be denied his application and shall forfeit any

application fees paid, and in addition to any criminal penalties shall be forbidden to reapply for a period of

not less than two (2) years.

2.14.2 Each individual applicant who willfully misstates a material fact or offers forged, altered, fraudulent

or fraudulently obtained documentation of qualification in connection with an application for a

Commonwealth of Dominica Certificate, or Seafarer’s Identification Document and/or Training Record

Book will in addition to any criminal penalties be subject to an administrative penalty of $500 and may be

barred from holding any Commonwealth of Dominica

Certificate, or Seafarer’s Identification Document and/or Training Record Book until two (2) years after

such penalty is remitted in full.

2.14.3. Any shipowner or submittal agent who submits on behalf of an applicant an application for a

Commonwealth of Dominica Certificate, or Seafarer’s Identification Document and/or Training Record

Book, which application contains a willful misstatement of fact or references a document which is forged,

altered, fraudulent or fraudulently obtained, the truth or genuineness of which is capable of being

determined by the submitting party, will be subject to an administrative penalty of $500 for each such

application. Further submissions from such source may be barred until two (2) years after such penalty is

remitted in full.

3 Based Upon

Equivalent Certification (Endorsement)

3.1 Persons Who Are Eligible

3.1.1 Duly Certificated - An applicant who is eligible in all other respects may be issued a certificate

without further examination, provided he is currently duly certificated by a foreign government whose

certification system is in compliance with STCW 95 and is acceptable to the MARAD. MARAD, however,

reserves the right to administer an examination as part of its quality standards program.

3.1.2 Acceptable Foreign Certificates

.1 No foreign certificate shall be accepted as the basis for obtaining a Dominica Certificate unless

it is proved that such foreign certificate was obtained in full compliance with the rules and

regulations of the issuing government, including successful completion of a comprehensive

examination administered by the proper authority of said government.

.2 Such foreign certificate must be current and valid in all respects, contain the specified STCW

Convention endorsement and will not be accepted if revoked, expired, altered, illegible or

otherwise indicative of non-validity.

.3 Countries whose certificates are recognized by MARAD are those on the “Compliant Parties”

list of countries whose maritime programs have been deemed to be in compliance with the STCW

95 Convention by the International Maritime Organization.

3.1.3 Equivalent Grade - The MARAD may decline to accept a foreign certificate as determinative of the

holder’s eligibility for a Dominica Certificate in the same grade and capacity, if the standards and

requirements for obtaining such foreign certificate are lower than those prescribed by the MARAD for

issuance of a Dominica certificate or the MARAD may determine that such foreign certificate is equivalent

to a lower grade of Dominica Certificate.

3.1.4 Experience - A foreign certificate which authorizes the holder to serve in more than one grade will be

considered equivalent to that Dominica Certificate grade that reflects the applicant’s actual experience.

3.1.5 Restrictions - Any certificate issued upon a foreign equivalent may be similarly limited as to any

restriction applicable to such foreign certificate, depending upon the applicant’s actual experience.

3.2 Additional Requirements

3.2.1 Proof of Experience - Each applicant for an “endorsement” certificate without further examination, in

addition to complying with the general requirements of Item 2. 10 above, must prove at least one (1) year

of total actual sea service during the five (5) years immediately preceding the date of application. An

applicant who is certificated by a foreign government whose licensing system is acceptable to the

MARAD and whose national license bears the endorsement

specified by the International Convention on Standards of Training, Certification and Watchkeeping for

Seafarers, 1978 (STCW 78), will not be required to document sea service and/or nautical training as

specified under Part 4.0 - Certifications and Examinations for Officer Certificates.

.1 Acceptable Documentation - Training record books, certificates of service, forms of discharge

and other documents certifying the amount and character of experience, and identifying the

vessels on which served, shall be examined as to their authenticity and any indication of

unauthorized alteration or other invalidity shall render such documents unacceptable. Normally,

proof of sea service can be satisfactorily established by providing a copy of the applicant’s official

Seafarer’s (discharge) book. For certification of service aboard Dominica vessels, the applicant

must provide a copy of the applicant’s Dominica Seafarer’s Training Record Book, or a certificate

of service in accordance with Commonwealth of Dominica Maritime Regulation 7.30.5.

.2 Originals or Copies - An applicant who files in person may exhibit original proofs of service or

experience; all others must submit copies of their discharge books or certificates of service in lieu

of the original documents. When original documents are submitted they will be returned by the

MARAD.

3.2.2 Recognition of Foreign Certificate

.1 A person applying for a certificate without further examination is required to submit with his

application proof of his foreign certificate. Such proof may be the original foreign certificate, an

official duplicate, or a certified copy.

.2 All documents submitted with applications are subject to verification by the Deputy Maritime

Administrator as per Regulation I/10 of STCW 95.

.3 MARAD reserves the right to require a written examination of the applicant.

4 Certification and Examination

for Officer Certificates 4.1 Persons Who Are Eligible

A person who is not certificated or who desires to raise in grade any certificate currently held by him may

apply to sit for an examination in the appropriate grade, provided such person is otherwise eligible in

accordance with the applicable requirements.

4.2 Service Requirements for Deck/Navigational Officer Ocean-Going Certification For Service Vessels of Over 3000 Gross Tons

4.2.1 All Grades - Service shall have been aboard ocean-going steam or motor vessels of 3,000 gross

tons or over. At least one year of the total service required shall have been within five (5) years preceding

the date of application.

4.2.2 Officer in charge of a navigation watch - to qualify under the STCW 95 Convention - At least three (3)

years of service in the deck department, or graduation from a recognized naval or nautical school whose

program and standards of training are in accordance with STCW 95. Full or partial credit for attendance at

such naval or nautical school may be allowed, up to a maximum

of two (2) years sea time. At least one year of sea service must have been accomplished while the

applicant was enrolled in a program, which integrates shore-based training with on board ship training.

Such training should provide:

.1 Structured, supervised training for at least six (6) months in bridge watch keeping and other

duties specified in STCW 95, Section B-II/1. Such on board training should be managed by the

company which manages the ship where the training is being conducted.

.2 A recording of all such training must be recorded in an approved Training Record Book.

.3 A systematic procedure for assessment of abilities and skills. Such evaluations should be done

in accordance with STCW 95, Section A-II/1. An applicant must submit, with the request to take

the exam for an original Third Mate’s certificate, a copy of his or her record book, a copy of the

evaluations received during training, and a copy of the course completion diploma or certification.

4.2.3 Second Mate - While holding a Commonwealth of Dominica Certificate as Third Mate, or a foreign

certificate in equivalent grade, at least one (1) year of service as Third Mate; or at least five (5) years of

service in the deck department, two (2) years of which shall have been as able seaman, quartermaster,

boatswain or apprentice officer.

4.2.4 Chief Mate - While holding a Commonwealth of Dominica Certificate as Second Mate, or a foreign

certificate in equivalent grade:

.1 At least one (1) year of service as Second Mate; or

.2 At least two (2) years of service as Third Mate.

4.2.5 Master - While holding a Dominica Certificate as Chief Mate, or a foreign certificate in equivalent

grade:

.1 At least one (1) year of service as Chief Mate; or

.2 At least two (2) years of service as Second Mate.

4.2.6 Certificated Service - Where certificated service is required in establishing eligibility, such service

must have been completed while holding a current and valid ocean-going certificate.

4.2.7 Ocean-Going - For the purposes of this Item 4.2, the term “ocean-going” shall mean without

limitation or restriction as to trading area, tonnage or type of vessel.

4.2.8 Tonnage - Certificates issued pursuant to this Item 4.2 shall be valid for service on vessels of any

gross tonnage.

4.3 Service Requirements for Deck/Navigational Officer Near-Coastal Examinations – For Service on Vessels Between 500 and 3000 Gross Tons

4.3.1 All Grades - Service shall have been aboard steam or motor vessels of 500 gross tons or over. At

least one (1) year of the total required service shall have been within five (5) years preceding the date of

application.

4.3.2 Third Mate to qualify under the STCW- 78 Convention - (Certificates issued under the 1978

Convention standards expire 1 February 2002) - At least 3 years of service in the deck department. This

option is only available to those seafarers who commerce their sea going training or sea service prior to 1

August 1998. Reference STCW 95 Regulation: I/ 15.

4.3.3 Third Mate to qualify under the STCW-95 Convention - At least three (3) years of service in the deck

department, or graduation from a recognized naval or nautical school whose program and standards of

training are in accordance with STCW 95. Full or partial credit for attendance at such naval or nautical

school may be allowed, up to a maximum of two (2) years sea time. At least one year of sea service must

have been accomplished while the applicant was enrolled in a program, which integrates shore-based

training with on board ship training. Such training should provide:

.1 Structured, supervised training for at least six (6) months in bridge watch keeping and other

duties specified in STCW 95, Section B-II/1. Such on board training should be managed by the

company which manages the ship where the training is being conducted.

.2 A recording of all such training must be recorded in a training record book approved by the

MARAD.

.3 A systematic procedure for assessment of abilities and skills. Such evaluations are done in

accordance with STCW 95, Section A-II/1. An applicant must submit, with the request to take the

exam for an original third mate’s certificate, a copy of his or her training record book, a copy of

the evaluations received during training, and a copy of the course completion diploma or

certification.

4.3.4 Master - At least six (6) years of service on deck, including not less than three (3) years as

certificated mate or equivalent. The period of service as certificated mate or equivalent may be reduced to

two (2) years when not less than one (1) year of this period was served in the capacity of Chief Mate.

4.3.5 Certificated Service - Where certificated service is required in establishing eligibility, such service

must have been completed while holding a near-trade or ocean-going certificate.

4.3.6 Limitations - Certificates issued pursuant to this Item 4.3 shall be limited by endorsement to service

on vessels of not more than 3000 gross tons. Such certificates may be further limited to service on

vessels under 3000 gross tons, according to the candidate’s actual experience and qualifications.

4.4 Service Requirements for Deck/Navigational Officer Near-Coastal Voyage Examinations - For Service on Vessels under 500 Gross Tons

4.4.1 All Grades - Service shall have been aboard steam or motor vessels of 100 gross tons or over. At

least one (1) year of the total service required shall have been within five (5) years preceding the date of

application.

4.4.2 Mate - At least three (3) years of service in the deck department, or graduation from a recognized

naval or nautical school whose program and staining are in accordance with STCW 95. Full or partial

credit for attendance at such naval or nautical school may be allowed, up to a maximum of two (2) years

sea time. At least one year of sea service must have been accomplished while the applicant was enrolled

in a program, which integrates shore-based training with on board ship training. Such training should

provide:

.1 Structured, supervised training for at least six (6) months in bridge watch keeping and other

duties specified in STCW 95, Section B-II/I. Such on board training should be managed by the

company, which manages the ship where the training is being conducted.

.2 A recording of all such training shall be recorded in a Training Record Book approved by the

MARAD.

.3 A systematic procedure for assessment of abilities and skills. Such evaluations are done in

accordance with STCW 95, Section A-II/1.

4.4.3 Master - At least four (4) years of service on deck, including not less than one (1) year as

certificated mate or equivalent.

4.4.4 Certificated Service - Where certificated service is required in establishing eligibility, such service

must have been completed while holding at least a near trade certificate. The MARAD may evaluate the

qualifications and experience of the holder of any grade or type of certificate for the purpose of

establishing the eligibility of the holder for the issuance of a near trade certificate limited to service on

vessels under 500 gross tons, with or without further examination.

4.4.5 Limitations - A certificate issued pursuant to this Item 4.4 shall be endorsed as limited to service on

vessels under 500 gross tons. Such certificate may be further limited by endorsement, according to the

candidate’s actual experience and qualifications.

4.5 Service Requirements for MODU Deck/Navigational Officer Examinations – For Mobile Offshore Drilling Units (MODUs)

4.5.1 Mate

.1 Three (3) calendar years of service as roustabout, derrick man, driller, barge engineer, tool

pusher, rig superintendent or equivalent; or

.2 Hold a degree as Bachelor of Science from a recognized school of technology and have 18

months of actual service as roustabout or in another capacity listed above.

4.5.2 Master

.1 Six (6) calendar years of service as roustabout, derrick man, driller, barge engineer, tool

pusher, rig superintendent or equal, at least three (3) years of which has been in a supervisory

capacity such as driller, barge engineer, tool pusher or rig superintendent; or

.2 Two (2) years of service as a certificated mate (MODU); or

.3 Hold a degree as Bachelor of Science from a recognized school of technology and have three

(3) years of service on drill units, one year of which was in a supervisory capacity such as listed

above.

4.5.3 Additional Requirements - Candidates for MODU deck certificates must complete the fire-fighting

training requirement as per sub-item 2.10.6 and must also qualify for the Dominica radar observer

certificate as per sub-item 2.10.7.

4.5.4 Service Time - While the Administration does not require a 12-hour work day, calendar year service

is acceptable for MODU certificates if the applicant has worked 30 days on and 30 days off (or equivalent)

and a 12-hour day. A 12- hour day also equates to 1.5 days of actual sea time.

4.6 Service Requirements for Engineer Officer Steam and Motor Examinations – For Service on Vessels of Over 3000kW.

4.6.1 All Grades - No certificate issued pursuant to this Item 4.6 shall be limited as to tonnage but any

such certificate may be limited with respect to type or power rating of main propulsion machinery, or both.

Service shall have been aboard steam or motor vessels of over 3000 kW (4,080 h.p.) propulsion power.

At least one year of the total service required shall have been within five (5) years preceding the date of

application. At least six (6) months of the total service required shall at some time have been served in

the mode of propulsion applied for.

4.6.2 Third Assistant Engineer (Steam) or Third Assistant Engineer (Motor) to qualify under the STCW 78

Convention - (Certificates issued under the 1978 Convention standards expire 1 February 2002.) At least

three (3) years of service in the engine department of steam or motor vessels, including not less than one

(1) year as fireman, oilier, water tender, motorman, GP-1, or apprentice officer; or at least three (3) years

of service ashore as an apprentice in the construction, repair or operation of marine or stationary steam

or motor engines in addition to at least one (1) year of service in the engine department of steam or motor

vessels as fireman, oiler, watertender, motorman, GP-1 or apprentice officer; or graduation from a

recognized naval or marine engineering school whose program and standards of training are in

accordance with STCW 78, an option available only to those seafarers who commenced their seagoing

training or sea service prior to 1 August 1998. [Ref. STCW 95 Regulation: 1/15.]

4.6.3 Third Assistant Engineer (Steam) or Third Assistant Engineer (Motor) to qualify under the STCW-95

Convention - All candidates must have completed an approved education and training course of at least

30 months duration. That training program must coordinate shore-based and seagoing training as stated

in STCW 95 Section B-III/l.

.1 Structured, supervised training for at least six (6) months in engine room watchkeeping. Such

on-board training should be managed by the company that manages the ship where the training

is being conducted.

.2 recording of all such training shall be recorded in a Training Record Book approved by the

MARAD.

.3 A systematic procedure for assessment of abilities and skills. Such evaluations should be done

in accordance with STCW 95, Section A-III/1. An applicant must submit, with the request to take

the exam for an original 3rd assistant engineer’s certificate, a copy of his or her record book, a

copy of the evaluations received during training, and a copy of the course completion diploma or

certification.

4.6.4 Naval or Marine Engineering Degree - All seafarers applying for a certificate as Third Assistant

Engineer who commence their sea service after 1 August 1998 must qualify by graduation from a

recognized naval or marine engineering school. Seafarers who commenced their sea service prior to 1

August 1998 may qualify by sea service or apprentice service listed above until 1 February 2002.

4.6.5 Second Assistant Engineer (Steam) or Second Assistant Engineer (Motor) - While holding a

Commonwealth of Dominica Certificate as Third Assistant Engineer (Steam or Motor) or a foreign

certificate in equivalent grade, at least one (1) year of service as Third Assistant Engineer; or at least five

(5) years of service in the engine department of steam or motor vessels, two (2).years of which shall have

been as fireman, oiler, watertender, motorman, GP-1, or apprentice officer. All seafarers who commence

their sea service after 1 August 1998 must qualify by presenting at least one (1) year of service as a

certificated Third Assistant Engineer.

4.6.6 First Assistant Engineer (Steam) or First Assistant Engineer (Motor) –While holding a Dominica

Certificate as Second Assistant Engineer (Steam), or a foreign certificate in equivalent grade:

.1 at least one (1) year of service as Second Assistant Engineer; or

.2 at least two (2) years of service as Third Assistant Engineer.

4.6.7 Chief Engineer (Steam) or Chief Engineer (Motor) - While holding a Dominica Certificate as First

Assistant Engineer (Steam), or First Assistant Engineer (Motor), as appropriate, or a foreign certificate in

equivalent grade:

.1 at least one (1) year of service as First Assistant Engineer; or

.2 at least two (2) years of service as Second Assistant Engineer.

4.6.8 Motor Certificates - The candidate must prove service on motor vessels in a comparable capacity

and for the same period as required for a steam certificate, with the allowance provided in sub-item 4.6. 1;

however, the certificated service required in sub-items 4.6.4, 4.6.5, and 4.6.6 must have been served

while holding a motor certificate.

4.6.9 Steam and Motor Certificates -A candidate who meets the requirements for examination for steam

and motor certificates may be examined in both categories, and if successful will be issued a certificate

for service in both categories.

4.6.10 Service Validity - Certificates issued pursuant to this Item 4.6 shall be valid for service on vessels

of any power rating.

4.7 Service Requirements for Engineer Officer Limited Motor Examinations

4.7.1 All Grades - Certificates issued pursuant to this Item 4.7 shall be limited by endorsement to service

on motor vessels only and for engines of not more than 3000 kW (4,080 h.p.) propulsion power. At least

one (1) year of the total service required shall have been within five (5) years preceding the date of

application.

4.7.2 Third Assistant Engineer Limited Motor - to qualify under the STCW 78 Convention (Certificates

issued under the 1978 Convention standards expire 1 February 2002), at least three (3) years of service

in the engine department of steam or motor vessels of any power rating, two years of which must have

been aboard motor vessels.

4.7.3 Third Assistant Engineer Limited Motor - to qualify under the STCW 95 Convention, all candidates

must have completed an approved education and training course of at least 30 months duration. This

training program must coordinate shore based and sea going training as stated in STCW 95 Section B-

III/1:

.1 Structured, supervised training for at least six (6) months in engine room watchkeeping. Such

on board training should be managed by the company that manages the ship where the training

is being conducted.

.2 A recording of all such training in a Training Record Book approved by the MARAD.

.3 A systematic procedure for assessment of abilities and skills. Such evaluations should be done

in accordance with STCW 95, Section A-III/l.

An applicant must submit, with the request to take the exam for an original third assistant engineer’s

certificate, a copy of his or her Training Record Book, a copy of the evaluations received during training,

and a copy of the course completion diploma or certification.

4.7.4 Engineer - At least five (5) years of service in the engine department of steam or motor vessels of

any power rating, one (1) year of which must have been aboard motor vessels as certificated assistant

engineer or equivalent.

4.8 Service Requirements for MODU Engineer Officer Examination - For Mobile Offshore Drilling Units (MODUs)

4.8.1 Assistant Engineer

.1 Three (3) calendar years of service as roustabout or above; two (2) years of which have been

as a mechanic, electrician, motorman, subsea engineer or equivalent; or

.2 Three (3) years of service in the machinist trade engaged in the construction or repair of diesel

engines, together with one (1) year of service in the capacity of mechanic, motorman, oiler or

equivalent; or

.3 Hold a degree as Bachelor of Science in Marine Engineering, Mechanical Engineering, or

Electrical Engineering, together with six (6) months service in the capacity of mechanic,

electrician, motorman, subsea engineer or equivalent.

4.8.2 Chief Engineer

.1 Six (6) calendar years of service as roustabout or in a capacity listed above, at least three (3)

years of which have been as a mechanic, electrician, motorman, subsea engineer or equivalent;

or

.2 Two (2) years of service as a certificated assistant engineer MODU.

4.8.3 Firefighting - Candidates for MODU engineer certificates must complete the firefighting training

requirement as per sub-item 2.10.6.

4.8.4 Service Time - While the Administration does not require a 12-hour workday, calendar year service

is acceptable for MODU Certificates if the applicant has worked 30 days on and 30 days off (or

equivalent) and a 12-hour workday.

4.9 Service Requirements for Radio Telephone or Radiotelegraph Operator Examinations

4.9.1 Radio Telephone Operator - Previous service is not required. The candidate must pass the written

examination for a certificate as Radio Telephone Operator.

4.9.2 Radiotelegraph Operator, Special Class - The candidate must have satisfactorily completed an

approved radio communications course.

4.9.3 Radiotelegraph Operator, Second Class - The candidate must have graduated from an approved

radio communications or technical school and have had at least six (6) months service aboard ship or in a

coast station, under the supervision of a certificated radio officer, of which service not less than three (3)

months shall have been aboard ship.

4.9.4 Radiotelegraph Operator, First Class - The candidate must have had at least three (3) years of

service while holding a valid certificate in the grade of Radiotelegraph Operator Second Class,

Commonwealth of Dominica or foreign. At least one (1) year of the required service shall have been

within the five (5) year period preceding the date of application.

4.9.5 Radiocommunication Operator, General Class - The candidate must have had at least four (4) years

of service while holding a valid certificate in the grade of Radiotelegraph Operator, Second Class or First

Class, Commonwealth of Dominica or foreign, including not less than one (1) year as Radiotelegraph

Operator, First Class. At least one (1) year of the required service shall have been within the five (5) year

period preceding the date of application.

4.10 Service Requirements for Global Maritime Distress and Safety System (GMDSS) Examination

4.10.1 Restricted Operator - Previous service not required. The candidate must pass the written

examination for a certificate as GMDSS Restricted Operator or present a foreign certificate in equivalent

grade for an STCW 78 certificate. For STCW 95 certification, the candidate must successfully complete

an approved GMDSS training program complying with STCW Code Section A-IV/2.

4.10.2 General Operator - Previous service not required. The candidate must pass the written

examination for a certificate as GMDSS General Operator or present a foreign certificate in equivalent

grade for an STCW 78 certificate. For STCW 95 certification, candidate must successfully complete an

approved GMDSS training program complying with STCW Code Section A-IV/2.

4.10.3 Second-Class Radio Electronic Operator - Previous service not required. The candidate must pass

the written examination for a certificate as Second Class Radio Electronic Operator or present a foreign

certificate in equivalent grade for an STCW 78 certificate. For STCW 95 certification, the candidate must

successfully complete an approved GMDSS training program including an examination or continuous

assessment, complying with STCW Code Section A-IV/2.

4.10.4 First-Class Radio Electronic Operator - Previous service not required. The candidate must pass

the written examination for a certificate as First Class Radio Electronic Operator or present a foreign

certificate in equivalent grade for an STCW 78 certificate. For STCW 95 certification, the candidate must

successfully complete an approved GMDSS training program including an examination or continuous

assessment, complying with STCW Code Section A-IV/2.

4.10.5 Application Form - The application form to be used for GMDSS Certification is official Form CDMP-

5000, Application for Officer Certificate of Competence. GMDSS training program certification must be

attached to the application.

4.11 Additional Examination Requirements

4.11.1 Proof of Experience

.1 Acceptable Documentation - A candidate for certificate by examination, in addition to

complying with the general requirements of Item 2.10, shall submit satisfactory proof of sea

service and. experience covering at least the period required for eligibility to sit that examination.

Seafarer’s books, certificates of service, forms of discharge and other documents certifying the

amount and character of experience, and identifying the vessels on which served, shall be

examined as to their authenticity and any indication of unauthorized alteration or other invalidity

shall render such documents unacceptable. Normally, proof of sea service can be satisfactorily

established by providing a copy of the candidate’s official Seafarer’s (discharge) book. For

certification of service aboard Dominica vessels, the candidate must provide a copy of the

candidate’s Dominica Seafarer’s Identification Document and Training Record Book, or a

certificate of service in accordance with Dominica Maritime Regulation 7.30(5).

.2 Graduation Certificates - A candidate claiming graduation from a recognized naval, nautical,

marine engineering or radio school in lieu of sea service shall be required to submit a certified

copy or other facsimile of the candidate’s certificate of graduation or of course completion, and a

certified translation into English if such certificate is in a foreign language.

4.11.2 Evidence of Foreign Certificate - A candidate who holds a foreign certificate and who is applying for

examination for a Dominica Certificate in a higher grade is required to submit with the application

satisfactory proof of foreign Certificate. Such proof may be the original foreign Certificate, an official

duplicate, or a certified photostatic copy.

4.11.3 Surrender of Old Certificate - A successful candidate for examination in a higher grade than the

Dominica Certificate currently held shall surrender the old Commonwealth of Dominica Certificate to the

MARAD upon issuance of the new certificate.

4.11.4 English Language Proficiency - Examinations may be taken only in English.

4.12 Application Procedure

4.12.1 How to Apply - Every candidate for examination for an officer certificate shall submit a properly

completed application (see Items 2.6 through 2.10) and shall indicate the a locality at which the candidate

would prefer to take the examination and the approximate date on which the candidate can appear at said

location.

4.12.2 Days and Places of Examination - Examinations may be taken, upon request to the MARAD, at

any location that is mutually agreeable to the applicant and MARAD. The test centers acceptable to the

Maritime Administrator or Deputy Maritime Administrator for the administration of all Dominica

examinations for officer certificates, ratings, and special qualifications will be established through

Recognized Organizations or other accepted organizations on an as-needed

basis. Examinations normally are conducted on weekdays commencing at 0900 hours local time and

terminating at 1700 hours each day; however, a candidate is required to conform to the office hours of the

individual facility where examination hours differ from the normal schedule.

4.14 Conduct of Examinations

4.14.1 Authorized Tables, Publications and Equipment

.1 No candidate for examination shall be allowed to have on the person or in the candidate’s possession,

nor is there to be present in the examination space, any equipment, material, books, notes or other

literature except as provided for the candidate by the examination facility.

.2 Any candidate will be permitted to make use of an English-foreign language dictionary, which may be

taken into the examination space provided such dictionary is also first inspected for unauthorized notes

and loose papers.

.3 A candidate may also provide and utilize the candidate’s own star finder, nonprogrammable calculator

and/or electronic slide rule.

.4 Certain portions of the refresher examinations for certificate renewal are open-book. Candidates may

bring their own material or avail themselves of materials provided at the test center for that portion of the

examination only.

4.14.2 Unauthorized Persons and Communication Forbidden - No unauthorized persons shall be

permitted to enter the examination space, and no candidate for a Certificate shall be permitted to

communicate in any manner with any unauthorized person(s) during the taking of an examination. The

candidate shall not be given any assistance by a proctor in the answering of examination questions.

4.14.3 Personal Conduct - Each candidate for a Certificate shall conform to the particular requirements of

the examination facility and shall always maintain absolute silence during the conduct of examinations.

Each candidate is required to take care not to mark or otherwise abuse the publications, equipment and

furniture provided and shall not remove any such items from the examination facility.

4.14.4 Work Materials - In answering the examination questions, each candidate must show and label all

work. Answers are to be neatly marked on the answer sheets provided. Computations and scratchwork

are to be attached to the answer sheets to which they apply, and must not be taken from the examination

facility.

4.14.5 Penalty - Any candidate who shall receive unauthorized assistance, makes use of any

unauthorized material or is in any other way found not in compliance with the foregoing during the

conduct of an examination will be disqualified from proceeding further and shall forfeit all fees paid.

Candidates failing under these circumstances will not be eligible for re-examination for a period of six (6)

months.

4.14.6 Failure to Complete Examination - Any candidate who commences an examination but who fails to

complete it within the prescribed period, or who otherwise fails to finish all required tests, shall not be

entitled to a refund of any examination fees paid by him except when the failure to complete the

examination is the responsibility of the administering authority.

4.15 Re-examination Upon Failure

4.15.1 Passing Grade - Passing grade for all Deputy Maritime Administrator’s for both officers and ratings

is 70 percent.

4.15.2 Waiting Period - A candidate who fails his examination may not be re-examined earlier than 30

days from notification of failure and may not be re-examined more than twice in 12 months.

4.15.3 Refresher Examination - Refresher examination for certificate renewal must be taken before the

one-year grace period for renewal expires on the certificate being renewed. Failure to do so will require a

candidate to retake the full written examination.

4.15.4 Reviewing Examination Papers - Upon request of the MARAD, the score awarded to a failing

candidate’s examination will be reviewed. The score awarded to an examination following review shall be

final.

5 Replacements and Renewal

of Officer Certificates 5.1 Replacement of Lost or Destroyed Certificate

The holder of a certificate that is lost or destroyed may apply for its replacement, valid for the unexpired

term of the original certificate, by submitting to the MARAD an affidavit in writing, sworn or affirmed before

a Civil Notary or Commissioner for Oaths, a Dominica ship’s master, or a surveyor from a Dominica-

recognized organization. The affidavit must state the circumstances of the loss or destruction and give a

description of the certificate. With the affidavit must be submitted three (3) passport-size (2 in. x 2 in.) (50

mm. x 50 mm.) photographs of the applicant, and the fee prescribed in the fee schedule for issuance of a

duplicate certificate.

5.2 Renewal of Certificate

5.2.1 Allowances

Any certificate may be renewed without re-examination prior to its expiration or within the 12 months

following its date of expiration. A Commonwealth of Dominica STCW 78 Certificate of Competency may

be renewed between the period 1 January 2001 through 30 January 2002 to expire on 31 January 2002.

Upon submission of required documentation for 1995 STCW qualification, however, the 1978 Certificate

will be renewed at no charge for the remainder of its normal term (i.e., five (5) years from the original date

of issue) as a 1995 STCW Certificate. A full term five-year certificate will be issued if the seafarer should

elect to remit the standard renewal fee.

5.2.2 Application - An applicant for renewal of a certificate shall surrender his expired certificate (or a

certified copy) and shall submit a properly completed application Form CDMP-5000 (Annex 1) for

renewal. With the application there shall also be submitted:

.1 two (2) passport-size photographs of the applicant

.2 a report of physical examination within the preceding 12 months on Form CDMP-5000B.

.3 the fee prescribed in the fee schedule for renewal of a certificate

.4 evidence of professional competence.

5.2.3 Evidence of Professional Competence - The evidence of professional competence referred to in sub-

item 5.2.2.4 above which must accompany the application shall be as follows:

.1 evidence of all sea service under the expired certificate, including proof of sea-going service in

the grade of certificate held for a total of at least one (1) year during the preceding five (5) years;

or

.2 evidence of all employment during at least the preceding five (5) years, including employment

ashore closely related to the operation of vessels, which may be considered by MARAD to be at

least equivalent to the sea-going service required by sub-item 5.2.3.1 above; or

.3 evidence of having completed seagoing service in an appropriate supernumerary capacity for a

period of not less than three (3) months immediately prior to taking up the position in which he or

she is entitled to serve by virtue of his or her certificate grade; or

.4 one (1) of the following:

• passing a refresher test; or

• successfully completing an approved refresher course or courses.

5.2.4 Additional Training and Certification - Effective 1 February 2002, an applicant for certificate renewal

must meet the same general requirements for a Commonwealth of Dominica Certificate specified in Item

2.10, General Requirements for Officer Certificate, as applicable, in regard to Firefighting Training, Radar

Observer Training, GMDSS Certification, First Aid Training, Advanced Medical Training, Proficient in

Survival Craft/Rescue Boats Training, and Automatic Radar Plotting Aids (ARPA) Training.

5.2.5 Expired Certificate Renewal - A Certificate may be renewed after 12 months following its expiration

date only upon the terms and conditions set forth in Part 3.0 or 4.0 of these Requirements, including

payment of the fee specified in the fee schedule for initial issuance of a certificate (see Commonwealth of

Dominica Maritime Regulation 1. 4.(5)).

6 Seafarer’s Identification Document (SID)

and Training Record Book (TRB) 6.1 General Information

6.1.1 National Requirement - Each person employed on board a Dominica flag vessel, other than those

persons such as certain hotel staff personnel on passenger vessels, scientists aboard research and

satellite launch vessels, livestock tenders aboard cattle carriers, fish gutters aboard fish factory ships,

riding repair crews, oil production workers on FPSOs and MODUs and other nonmaritime personnel who

are not regularly assigned or required to perform shipboard safety and pollution-related duties, shall have

in his or her possession an official Commonwealth of Dominica “Seafarer’s Identification Document” (SID)

and a “Training Record Book” (TRB). The TRB (Commonwealth of Dominica Publication CDP-302) will

contain a record of the “sign-offs” for the various required skill assessments as well as all certificates of

approved course

completions and special qualification.

6.1.2 Persons Eligible - An officer or rating currently serving aboard any Commonwealth of Dominica flag

vessel, a holder of a valid seafarer’s identity document issued by another maritime state, or an applicant

with a letter of commitment for employment from a shipping company for service aboard a Dominica flag

vessel, is eligible to apply for a SID and TRB.

6.1.3 Fees - A fee schedule is listed in application Form CDMP-5000 and in the fee schedule. Should

secured delivery of Seafarers Identification Document an Training Record Book by courier service be

required, a fixed fee of U.S. $35 per SID/TRB must also be remitted.

6.1.4 Basic Training

.1 Elements - Seafarers beginning their seagoing training or service after 1 February 1997 and

applying for an original “Seafarer’s Identification and Training Record Book” must show evidence

of having received appropriate approved “Basic Training” or instruction in all of the following:

• personal survival techniques;

• fire prevention and firefighting;

• elementary first aid; and

• personal safety and social responsibilities.

That training will be reflected by the insertion of the appropriate Special Qualification Certificate

into the Seafarer’s Training Record Book. (Reference: Section A-VI/1).

.2 Grandfathering - For seafarers who commenced their maritime training and/or sea service prior

to 1 February 1997, these requirements do not become effective until 1 February 2002. Such

“Grandfathered” seafarers may qualify for a “Basic Training” certificate that indicates that their

training complies with the STCW 78 requirements upon submission of proof that they have had

such training. Such certificates, however, will expire 1 February 2002.

.3 Option - If the seafarer already holds a Commonwealth of Dominica Seafarers Identification

Document and Training Record Book and wishes to have this certification, the seafarer must

submit an application on Form CDMP-5000A, proof of training, and the appropriate fee (see the

fee schedule).

.4 Certification - If the basic training “diploma” attached to an application shows that the seafarer

has received STCW 95 “Basic Training,” then the appropriate Commonwealth of Dominica

certificate will be issued with no date of expiry. If the seafarer received “Basic Training” previous

to the inclusion of “personal safety and social responsibilities” training, then a Commonwealth of

Dominica certificate will be issued with a date of expiry of 1 February 2002. Basic Training

certificates will be entered in the TRB at no additional charge if the applicant applies for it

simultaneously with his or her application for a SID.

6.1.4 Passenger Vessels - The Basic Training requirements are limited to those seafarers who are

regularly assigned or required to perform routine ship safety and pollution prevention related duties.

Therefore, those personnel not so assigned (e.g., hotel staff) will not be required to comply with these

requirements.

6.2 How to Apply

6.2.1 Form CDMP-5000 - Each application must be submitted in person on Form CDMP-5000, Application

for STCW’95 Certificate of Competence, Dominica Maritime License, Seafarer’s ID and Training Record

Book (the fee schedule) to an Authorized Receiving/Filing Agent who will review the documents submitted

and witness the applicant signing the application. Detailed instructions are set forth in the “General

Information and Instructions” section on Form CDMP-5000. The applicant must retain a copy of this

application as evidence that the application is being processed.

6.2.2 Complete Application - With each properly completed application form and the fee prescribed in the

fee schedule, there shall be submitted all necessary documents, letters and/or proofs as specified in the

“General Information and Instructions” of Form CDMP-5000, in order to establish eligibility for a Dominica

Seafarer’s Identification Document and Training Record Book.

6.2.3 Physical Examination - Every candidate for a certificate shall be required to submit satisfactory

evidence of a physical examination completed within the 12 months immediately preceding application.

(See Section 2.10.3 for details.) Submitting the application without the necessary fees or supporting

documentation will result in delay and possible rejection of the application.

6.3 SID and TRB Uses

6.3.1 Identification - The SID is issued under the authority of Section 11 of the Dominica International

Maritime Act 2000, Dominica Maritime Regulation 7.30 and in conformity with the provisions of ILO

Convention No. 108 (the Seafarer’s Identity Documents Convention, 1958). It is a document issued to

personnel serving aboard Dominica flag vessels for the purposes of providing the holder with identity

papers for travel to or from an assigned vessel. It is not a passport; it is issued without prejudice to and in

no way affects the national status of the holder. It remains the property of the Commonwealth of

Dominica, and can be withdrawn at any time.

6.3.2 Service Time

.1 The TRB is a means by which a record of the holder’s sea service and special qualification

certification awarded to the holder is maintained.

.2 The holder should retain possession of the TRB for proof of sea service. Sea service pages

remain valid as proof even though the book itself have been filled or expired.

6.4 Period of Validity

6.4.1 First Issue a

6.4.2 Replacement Issues - Replacement SIDs for stolen, lost or destroyed books will be issued with the

same date of expiry as the original SID.

6.5 Replacement

6.5.1 Filled or Damaged - If any TRB becomes filled with entries, requires alteration, or is damaged,

application for a replacement book should be made immediately.

6.5.2 Stolen, Lost or Destroyed - If the SID or TRB is stolen, lost, or accidentally destroyed, notification

should be given immediately to the MARAD in Washington, D.C. followed by an application for a

replacement TRB.

6.5.3 Lost in Mail - SIDs or TRBs mailed by MARAD and not received will be replaced at no additional

charge; however, such replacements will be sent by courier and the applicant will be responsible for the

courier charges.

6.5.4 Renewal - An application for renewal of an SID should be made at least three (3) months before the

expiration date.

7 Special Qualification Certification

7.1 General Information

7.1.1 Certain ratings forming part of the navigational or engineer watches, and all officers and ratings

participating in cargo loading or discharge operations aboard oil tankers, chemical tankers and liquefied

gas tankers, must meet the requirements of STCW 95 and be certified by the MARAD as qualified to

serve in such capacities.

7.1.2 Certification will be by appropriate entries on a Dominica Certificate of Endorsement or on a

Certificate of Competence. The STCW 95 function regulation cite, capacity, level and any limitations will

be given on the certificate issued. A separate special qualification certificate will not be issued.

7.1.3 Initial certification based on “equivalency” requiring an expiry date will be valid only for the period of

validity remaining on the equivalent (base) certificate. Upon expiry, standard revalidation requirements

will be followed.

7.1.4 Record - A copy of the certificate addressing special qualifications issued by MARAD shall be kept

with the Dominica Seafarer’s Identification Document and Training Record Book as appropriate.

7.1.5. Application - Applicants for special qualification certification shall follow the same procures as for a

Dominica Certificate of Endorsement or on a Certificate of Competence.

7.2 Persons Eligible

.1 Watchstanders - Certain ratings forming part of the navigational or engineer watches, and all

officers and ratings participating in cargo loading or discharge operations aboard oil tankers,

chemical tankers and liquefied gas tankers, must meet the requirements of this Part 7.0 and be

certified by the MARAD as qualified to serve in such capacities.

.2 Valid Certification - An applicant who provides proof of a current and valid certificate issued by

a foreign government, whose system of evaluation of education, training, examination,

competency assessment and certification policies are acceptable to the Maritime Safety

Committee of the IMO, may be eligible for an equivalent Dominica Special Qualification

Certificate.

7.2.2 Date of Expiry of Special Qualification Certificates

.1 Ratings will be issued SQCs with no date of expiry. Most officers SQCs will be issued with

dates of expiry.

.2 Certification based on Administration evaluation of examination, training certificates, or renewal

requiring an expiry date will be valid for five (5) years.

.3 Initial certification based on “equivalency” requiring an expiry date will be valid only for the

period of validity remaining on the equivalent (base) certificate. Upon expiry, standard revalidation

requirements will be followed.

7.2.3 Record - Special Qualification Certificates are issued by MARAD for entry in the Dominica

Seafarer’s Identification Document and Training Record Book.

7.3 Requirements for Navigational Watch Certification and Other Deck Department Ratings

7.3.1 Required Deck/Navigation Ratings - Only “Able Seaman,” “Ordinary Seaman,” “GP- I,” and “GP-2”

are required Deck/Navigational Ratings. Others are optional.

7.3.2 Service (Seatime) Requirements

.1 Able Seaman, II/4 (A.S.) - Three (3) years of service in the deck department on ocean-going

vessels, two (2) years of which must have been on vessels over 3000 gross tons. One (1) year of

this service may be replaced by successful completion of a training course approved by MARAD.

Service performed aboard MODUs in the capacity of Roustabout or above is creditable for an

endorsement as “Able Seaman limited to Mobile Offshore Drilling Units (MODUs).”

While the Administration does not require a 12-hour work day, calendar-year service is

acceptable for MODU-limited certification if the applicant has worked 30 days on and 30 days off

(or equivalent) and a 12-hour day.

.2 Ordinary Seaman, II/4 (OS) - Six (6) months of service in the deck department on ocean-going

vessels, three (3) months of which must have been on vessels over 3000 gross tons. Three (3)

months of that service may be replaced by successful completion of a training course approved

by the MARAD. Service performed aboard MODUs as a Roustabout or in a higher capacity is

creditable for an endorsement as “Ordinary Seaman limited to Mobile Offshore Drilling Units

(MODUs).”

While the Administration does not require a 12-hour work day, calendar-year service is

acceptable for MODU-Iimited certification if the applicant has worked 30 days on and 30 days off

(or equivalent) and a 12-hour day.

.3 Bosun II/4 (BO) - Three years’ service in the deck department on ocean-going vessels, two (2)

years of which must have been on vessels over 3000 gross tons. One (1) year of this service may

be replaced by successful completion of a training course approved by MARAD. In addition,

applicant must show either three (3) months service as a Bosun, or a letter from the vessel

operator or crew supplier stating appointment as Bosun. This is an optional certification.

.4 General Purpose, II/4, III/4 - (GP-1), for assignment to vessels with a “Minimum Safe Manning

Certificate” calling for “GP-l” ratings. Applicants must fulfill all requirements for both “Able Seaman

II/4” and “Oiler/Motorman III/4” to qualify for this rating. A total of two and one half (21/2) years of

sea service in a “General Purpose” rating may be substituted for the combined sea service

required for “Able Seaman” and “Oiler/Motorman.”

.5 General Purpose, II/4, III/4 - (GP-2), for assignment to vessels with a “Minimum Safe Manning

Certificate” calling for “GP-2” ratings.) Applicants must fulfill all requirements for “Ordinary

Seaman II/4” and “Oiler/Motorman III/4” to qualify for this rating. A total of nine (9) months sea

service as a “General Purpose Trainee” may be substituted for the combined sea service required

for “Ordinary Seaman” and “Oiler/Motorman.”

.6 Junior Ordinary Seaman (JOS), or General Purpose Trainee (GP) –Anyone who is 15 years

old or older and can meet the physical examination for a Deck/Navigational Officer’s Certificate

as described in sub-item 2.10.3 may apply for these ratings without any sea time. These are

optional ratings and are not required by any regulation.

.7 II/4 Designation - The “II/4” after “Able Seaman,” “Ordinary Seaman” and “Bosun,” “GP-1” and

“GP-2” indicates that the seafarer is a qualified rating, in accordance with STCW Code Section A-

II/4, forming part of a navigational watch.

7.3.3 Age Requirements - An applicant for Junior Ordinary Seaman or GP-Trainee must be at least 15

years of age. An applicant for Ordinary Seaman must be at least 16 years of age. An applicant for Able

Seaman certification must be at least 18 years of age. An applicant for Bosun certification must be at

least 21 years of age.

7.3.4 Physical Requirements

.1 Applicants for AB, GP-1, GP-2, Bosun and OS must meet the physical examination

requirements for a Deck/Navigational Officer’s Certificate as in sub-item 2.10.3.

.2 A physical examination with the results noted on Form CDMP-5000B (in Annex 1) must be

completed within the 12 months immediately preceding application.

7.3.5 Certification and Examination and Demonstration of Ability

Applicants not having completed the required approved courses or successfully completing an approved

in-service training program or not having the appropriate sign-offs in a TRB must take written examination

for the certificate being requested.

.1 An applicant for Able Seaman, Ordinary Seaman or Bosun certification must pass the written

and practical examination for Able Seaman.

.2 An applicant for GP-1 or GP-2 certification must pass the written examination for both Able

Seaman and Oiler/Motorman.

.3 To demonstrate navigation function competence at the support level, the applicant may either

complete an approved training course or supply references from ship Masters attesting to the

applicant’s ability to steer and to keep a proper lookout.

7.3.6 Endorsement Attesting Recognition of a National Certificate

An applicant who presents certification as Able Seaman, GP- 1, GP-2, Ordinary Seaman, Bosun or

equivalent issued by a foreign government whose rating system is accepted by the Maritime Safety

Committee of the IMO may be issued an equivalent endorsement attesting the recognition of that

certificate, without written examination, provided relevant provisions of sub-items 7.3.2, 7.3.3 and 7.3.4

are complied with.

7.3.7 Revalidation Requirements - Ratings are not required to revalidate watchstanding certification.

However, all seafarers must have in their possession a “Fit For Duty” physical examination form

completed by a physician within the past two (2) years.

7.4 Requirements for Engine Room Watch Certification and Other “Engine Room” Ratings (Ref: STCW 95, Code A-III/4)

7.4.1 Required Engine Room Watchstanding Ratings - Only “Oiler/Motorman, III/4,” “Fireman/Water Tender,

III/4,” “GP-1 II/4, III/4,” and “GP-2 II/4, III/4” are required engine room watchstanding ratings. Others are

optional..

7.4.2 Service (Seatime) Requirements

.1 Oiler/Motorman III/4, (O/M) - This combined SQC replaces the previously issued “Oiler, III/6”

and “Motorman, III/6” engine-room watchstander certificates. It requires one (1) year of service in

the engine department of steam or motor vessels, six (6) months of which must have been on

vessels of over 3000 KW propulsion power. Six (6) months of the one (1) year of service may be

replaced by successful completion of a training course approved by the MARAD.

.2 Fireman/Water Tender, III/4, (FWT) - This replaces “Fireman/Water Tender, III/6” previously

issued. It requires one (1) year of engine-room watchstanding service aboard steam powered

vessels, six (6) months of which must have been on vessels of over 3000kW propulsion power.

Six (6) months of the one (1) year of service may be replaced by successful completion of a

training course approved by MARAD.

.3 General Purpose 1, II/4, III/4 - (GP-1) This is for assignments to vessels with a “Minimum Safe

Manning Certificate” calling for a “GP-1” rating. Applicants must fulfill all requirements for both

“Able Seaman II/4” and “Oiler/Motorman III/4” to qualify for this rating. Two and one half (2 1/2)

years of total sea service in a “General Purpose-2” rating may be substituted for the combined

sea service required for “Able Seaman” and “Oiler/Motorman.”

.4 General Purpose-2, II/4, III/4 - (GP-2) This is for assignment to vessels with a “Minimum Safe

Manning Certificate” calling for a “GP-2” rating. Applicants must fulfill all requirements for

“Ordinary Seaman II/4” and “Oiler/Motorman III/4” to qualify for this rating. A total of nine (9)

months sea service as a “General Purpose Trainee” may be substituted for the combined

sea service.

.5 Oiler (MODU), (OILM) - One (1) year of service performed aboard Mobile Offshore Drilling

Units (MODUs) as a Roustabout or in a higher capacity is creditable for certification as “Oiler,

limited to Mobile”, upon completion of a training course in accordance with the STCW 95 Code

requirements.

While the Administration does not require a 12-hour workday, calendar-year service is acceptable

for MODU certification if the applicant has worked 30 days on and 30 days off (or equivalent) and

a 12-hour day.

.6 Pumpman (PM) - One (1) year of service aboard tank vessels is required. Three (3) months of

this service may be replaced by successful completion of a training course in accordance with the

STCW 95 Code requirements. This is an optional rating. It is not equivalent to a watchstanding

rating.

.7 Electrician (EL) - This is an optional rating. It is not equivalent to a watch-standing rating. One

(1) year of service in the engine department of steam or motor vessels is required. Three (3)

months of this service may be replaced by the successful completion of a training course

approved in accordance with the STCW 95 Code requirements.

.8 Wiper or General Purpose Trainee (WIP, GPT) - Anyone who is at least 15 years of page and

can meet the physical examination requirements for an engineer officer’s certificate of

competence as described in sub-item 2.10.3 may apply for this rating without any sea time. There

is no charge for this rating. This is an optional rating and is not required by any regulation.

.9 III/4 Definition - The “III/4” after “Oiler/Motorman,” “Fireman/Water Tender,” “GP-1” and “GP-2”

indicates that the seafarer is qualified, in accordance with STCW Code Section A III/4, to form

part of an engine-room watch.

7.4.3 Age Requirements - Applicants must be at least seventeen (17) years of age, except applicants for

wiper certification who must be at least (15) years of age.

7.4.4 Physical Requirements - Applicant must meet the physical examination requirements for an

engineer officer’s certificate of competency as in [sub-item 2.10.3]. Examination should be completed

within the twelve (12) months immediately preceding application with the results noted on a Physical

Examination Report/Certificate Form CDMP-5000B, (Annex 1).

7.4.5 Written Examination

Applicants not having completed an the required approved courses or successfully completed an

approved in service training program or not having the appropriate sign-offs in a TRB must take written

examination for the certificate being requested.

.1 Applicants for an original issue of the Special Qualification Certificate for “Oiler/Motorman, III/4”

or “Fireman/Water Tender, III/4” must pass the written Deputy Maritime Administrator

examination for engine-room watchstander.

.2 An applicant for GP-1 or GP-2 certification must pass the written examination for both Able

Seaman and Oiler/Motorman.

.3 Applicants for “Pumpman” and “Electrician” must pass examinations specific to these ratings.

.4 To demonstrate competence to perform the marine engineering function at the support level,

the applicant may complete an approved training course or the applicant may supply references

from Chief Engineers attesting to the applicant’s ability to properly relieve, stand, and hand over

an engine-room watch.

7.4.6 Endorsement Attesting Recognition of a National Certificate - An applicant who presents

certification as Oiler, Motorman, Fireman/Water Tender, GP-1, GP-2, Pumpman, Electrician, or

equivalent issued by a foreign government whose rating system is accepted by the Maritime

Safety Committee of the IMO may be issued an equivalent endorsement attesting the recognition

of that certificate without written examination, provided relevant provisions of sub-items 7.4.2,

7.4.3 and 7.4.4 are complied with.

7.4.7 Revalidation Requirements - Ratings are not required to revalidate watchstanding certification.

However, all seafarer’s must always possess a “Fit For Duty” Physical Examination form

completed by a physician within the past two (2) years.

7.5 Certification for “Proficient in Survival Craft/Rescue Boats” (Reference: STCW Code AVI/2, paras. 1 to 4)

7.5.1 Revised Certification - “Proficient in Survival Craft/Rescue Boats” will replace “Survival Craft

Crewman” certificate previously issued.

7.5.2 General Requirements

.1 Age and Sea time - Applicants must be at least 18 years of age and have a minimum of 12

months of seagoing service.

.2 SID/TRB - Applicants must have or have applied for a Dominica Seafarer’s Identification

Document and Training Record Book, except passenger vessel hotel staff.

.3 Hotel Staff - Hotel staff aboard passenger vessels may apply for “Proficient in Survival

Craft/Rescue Boats” without applying for a Dominica Training Record Book.” However, Form

CDMP-5000, including an applicant signature, must be used to create the applicant’s personal

data file. Photos must also be attached.

7.5.3 Experience Required

.1 Evidence of one (1) year of service on ocean-going or near trade vessels of over 3000 gross

tons must be presented. Three (3) months of this service may be replaced by successful

completion of a MARAD-approved training course.

.2 Service performed aboard MODUs in any capacity is creditable experience toward an

endorsement as Survival Craft/Rescue Boats Crewman limited to MODUs. While the

Administration does not require a 12-hour workday, calendar-year service is acceptable for

MODU-limited certification if the applicant has worked 30 days on and 30 days off (or equivalent)

and a 12- hour day.

7.5.4 Physical Requirements - Applicants must meet the physical examination requirements for an

engineer officer certificate as in sub-item 2.10.3. The examination should be completed within the 12

months immediately preceding application with the results noted on a Physical Examination

Report/Certificate, CDMP-5000B. (See Annex 1)

7.5.5 Certification, Examination, Written and Practical

.1 “Proficient in Survival Craft/Rescue Boats” means that the candidate must be able to launch

and take charge of a survival craft or rescue boat in emergency situations.

.2 Applicants for “Proficient in Survival Craft/Rescue Boats” not having completed an approved

course or not having the appropriate sign-offs in a TRB must take a written examination and

practical demonstration. Practical demonstrations are done by arrangements in advance through

the vessel’s safety officer, or similar authority. An oral examination may be substituted for the

written examination if given during the practical demonstration by an examiner approved by

MARAD.

7.4.6 Revalidation Requirements - Neither officers nor ratings are required to revalidate “Proficient in

Survival Craft/Rescue Boats” certification.

7.6 Certification for “Tankerman” (Reference: STCW 95, Code A-V/1)

7.6.1 Levels of Responsibility - Under STCW 95, “Tankerman” is considered to be a “Function” with three

“levels of responsibility”:

.1 Management Level - For senior officers serving as master, chief mate, chief engineer, and first

assistant engineer (second engineer officer);

.2 Operational Level - For other certificated officers, in the deck or engine departments serving

under the direction of an officer at the management level; and

.3 Support Level - For ratings, uncertificated crew in the deck or engine, departments performing

cargo transfer tasks under the direction of an individual serving at the operational or management

level.

7.6.2 “Person in Charge” or “Assisting” - This will also appear on the Tankerman Special Qualification

Certificates, as applicable. Officers shall have “Person in Charge,” and ratings shall have “Assisting.”

7.6.3 Types of Tankerman Certification

.1 Tankerman (Chemical) - This certification will be issued to seafarers with cargo handling duties

aboard liquid chemical carrying vessels.

.2 Tankerman (Liquefied Gas) - This certification will be issued to seafarers with cargo handling

duties aboard liquefied gas carrying vessels.

.3 Tankerman (Oil) - This certification will be issued to seafarers with cargo handling duties

aboard vessels carrying petroleum products not considered to be liquid chemicals or liquefied

gas.

.4 Tankerman (Crude Oil Washing/Inert Gas) - This certificate will be issued to seafarers with

cargo handling duties aboard vessels carrying crude petroleum in bulk and equipped with crude

oil washing and inert gas systems.

7.6.4 General Requirements for All Tankerman Certificates

.1 Physical Examination Report - All applications must include a copy of a physical examination

conducted within the previous twelve (12) months. Results of such examination must be noted on

Physical Examination Report/Certificate, Form CDMP-5000B. (See Annex 1). Deck/Navigational

Officers must meet the physical standards for Deck/Navigational Officers’ certificates of

competency. All others must meet physical standards for engineer officers.

.2 Firefighting - All personnel seeking Special Qualification Certificates as Tankerman (Chemical),

Tankerman (Liquefied Gas, Tankerman (Oil), and Tankerman (Crude Oil Washing/Inert Gas),

must have completed an approved, shore based firefighting course. For those seafarers seeking

the Tankerman (Liquefied Gas) certification, advanced training in practical firefighting techniques

and tactics applicable to gas tankers is required. (See Section 2.10.6.)

.3 Specialized Training - Senior officers and any person with immediate responsibility for loading,

discharging, care in transit, or handling of cargo shall complete an approved specialized training

program relating to specific duties aboard oil tankers, chemical tankers, or liquefied gas carriers

as appropriate to the certification sought. A copy of an appropriate Training Course Certificate

must be included with the application.

.4 If the above required training courses are either 1.) approved by an IMO “Reported”

government, or 2.) issued by a Maritime Training Center that has be certified by a Quality

Registrar authorized by MARAD, then an STCW 95 Endorsement or certificate will be issued by

the Administration. If the training course fully meets the standards of the 1978 Convention but

does not satisfy either of Conditions 1 or 2 above, then a ‘grandfathered” endorsement or

certificate will be issued. Such “grandfathered” endorsements and certificates will expire 1

February 2002.

7.6.5 Sea Service Requirements For Tankerman SQCs

.1 Senior Officers - Masters, chief mates, chief engineers, and first assistant engineers seeking

certification at the Management Level, must show at least one (1) year of service in the past five (5) years

aboard tank vessels. If the other requirements are met and the tank vessel sea time recency requirement

is not met, the certificate will be issued at the Operational Level.

.2 Junior Officers - Second mates, third mates, second assistant engineers, third assistant engineers

seeking certification at the Operational Level, must show at least three (3) months sea service aboard

tankers within the past five (5) years. If all other requirements are met but the tank vessel sea time

recency requirement or the approved training course requirements are not met, the certificate will be

issued at the Support Level.

.3 Ratings - Applicants are required to have a minimum of three (3) months service aboard crude oil,

petroleum, chemical, or liquefied gas tankers or an approved training course. Both the sea service and

the training shall be appropriate to the type of certification sought.

.4 Tankerman (Crude Oil Washing/Inert Gas) - This certification will be granted to officers and ratings

applicants who have completed training in Crude Oil Washing/Inert Gas and meet the sea time

requirements of sub-items 7.5.5.1 or 7.5.5.2, or 7.5.5.3, above. Tankerman applicants who are qualified

for Crude Oil Washing/Inert Gas endorsement should: specifically request it if they want it. Otherwise,

only a “Tankerman (Oil)” SQC will be issued.

7.6.6 Written Examinations

Applicants not having completed an the required approved courses, or successfully completing an

approved in service training program or not having the appropriate sign-offs in a TRB must take a written

examination for the certificate being requested. Such applicants for an original issue of the Special

Qualification Certificate for “Tankerman (Chemical),” “Tankerman (Liquefied Gas)” or “Tankerman (Oil)”

must pass the written Maritime Administrator’s examination.

7.6.7 Endorsement Attesting to Recognition of a National Certificate – An applicant who presents

certification as “Tankerman” issued by a foreign government whose rating system is accepted by the

Maritime Safety Committee of the IMO, may be issued an equivalent endorsement attesting to the

recognition of that certificate, without written examination, provided relevant sea service and

physical fitness provisions are complied with.

7.6.8 Revalidation Requirements (Ref: STCW Reg. I/11)

.1 Firefighting Training - Uncertificated personnel and others issued “Tankerman” Special

Qualification Certificates are required to submit evidence of an approved shore-based firefighting

course to receive STCW 95 Support Level “Tankerman” certificates.

.2 Officers - Certificated Deck/Navigational and Engineer Officers are required to renew their

Management and Operational Level “Tankerman” certification at five (5) year intervals.

Applications for renewal of a Special Qualification Certificate should be on Form CDMP-5000A,

(Annex 1), and be accompanied by a completed Physical Examination Report/ Certificate Form

CDMP-5000B (Annex 1). Applicant must show completion of an approved shore-based

firefighting course and one (1) year (management level) or three (3) months (operational level) of

sea service in the past five (5) years aboard the type of tank vessel appropriate to the

certification. Specialized training for officers serving aboard liquefied gas tankers includes

advanced fire fighting techniques and tactics applicable to gas tankers including the use of water

spray systems.

.3 Equivalent Service - In lieu of the required sea service, the applicant may qualify by completing

an approved refresher or updating training course appropriate to the certification sought. Such

course should include instruction in the changes in relevant regulations concerning safety of life

at sea and the protection of the marine environment. Applicant may also qualify having completed

approved seagoing service, performing functions appropriate to the certificate held for a period of

not less than three (3) months in a supernumerary capacity, or in a lower officer rank than that for

which the certificate is valid. Equivalent service may be accepted on a case by case basis.

7.7 Certification for “Person In Charge (MODU)”

7.7.1 General Information

.1 The person in charge of operations on MODUs must have either a Commonwealth of Dominica

Certificate as “Master, Ocean Going Column Stabilized Drill Rigs (CSDR),” which is now being re-

issued upon renewal as “Master, Mobile Offshore Drilling Units (MODU)”; or hold a valid ocean

Commonwealth of Dominica Master’s Certificate as specified in Item 4.2 and the “Person In

Charge (MODU) Special Qualification Certificate.”

.2 Persons holding the Commonwealth of Dominica Certificate as “Master, Mobile Offshore

Drilling Units (MODU)” are not required to hold the “Person In Charge (MODU)” SQC since the

Master (MODU) certificate supersedes the Person in Charge (MODU) SQC.

7.7.2 Experience Required

.1 Prior Certification - The applicant must hold a valid ocean Commonwealth of Dominica

Master’s Certificate of Competency as specified in Item 4.2.

.2 Background - The applicant must have at least six (6) months total experience in a supervisory

capacity on MODUs, such as driller, barge engineer, tool pusher or rig superintendent, within the

five-year period preceding the date of application.

.3 Service Time - While the Administration does not require a 12-hour work-day, calendar-month

service is acceptable for MODU-limited certification if the applicant has worked 30 days on and

30 days off (or equivalent) and a 12-hour day.

.4 Training - The applicant must have successfully completed a training course for Person in

Charge of MODU operations approved by MARAD and which includes procedures to prevent a

threatened blowout, contain and extinguish an on-board fire, control flooding and maintain

stability of MODUs, and to shut down and abandon MODUs.

7.7.3 Physical Fitness Requirements

.1 An applicant must meet the physical examination requirements for Deck/Navigational Officers

as specified in sub-item 2.10.3.

.2 A physical examination must be completed within the 12 months immediately preceding

application and the results noted on Form CDMP-5000B.

7.7.4 Acceptance of Equivalent MODU Certificates For Commonwealth of Dominica Certification

.1 An applicant who presents certification as “Person in Charge (MODU),” or equivalent issued by

a foreign government whose system is acceptable to MARAD may be issued equivalent

Commonwealth of Dominica certification as “Person in Charge (MODUs)” without examination,

provided that sea service and physical fitness requirements are complied with.

.2 As with other Special Qualification Certificates, the “Person In Charge (MODU)” SQC is to be

placed in the Training Record Book.

7.7.5 Revalidation - “Person in Charge (MODU)” will continue to be issued without a date of expiry.

7.8 Certificate of Competence for “Fast Rescue Boats” (Ref. STCW 95, Code A-VI/2, par. 5 to 8)

7.8.1 General Requirements

.1 Prior Certification - All applicants must have previously received “Proficient in Survival

Craft/Rescue Boats” certification or possess a Commonwealth of Dominica Navigational Officer’s

certificate of competence.

.2 Age and Sea Time - Applicants must be not less than 18 years of age with a minimum of 12

months seagoing service.

.3 Medical - Applicants must meet the physical examination requirements for a certificated

Deck/Navigational Officer.

.4 Training - Applicants must have completed an approved training course in fast rescue boat

operations.

7.8.2 Levels of Responsibility - “Fast Rescue Boats” certification will be issued in three (3) levels of

responsibility:

.1 Management Level - For senior officers serving as master, chief mate, chief engineer, and first

assistant engineer (second engineer officer).

.2 Operational Level - For other certificated officers, serving under the direction of an officer at the

management level.

.3 Support Level - For ratings, uncertificated crew performing tasks under the direction of an

individual serving in the operational or management level.

7.8.3 Revalidation - “Fast Rescue Boats” certificates will be issued without a date of

expiry.

7.9 Certification for “Advanced Firefighting” (Ref. STCW 95, Code A- VI/3)

7.9.1 STCW 95 Requirements - STCW 95 calls for training in “Advanced Firefighting” for those seafarers

with command responsibility in fighting fires. A Special Qualification Certificate will only be issued upon

completion of an approved advanced firefighting course.

7.9.2 Levels of Responsibility - Advanced Firefighting certification will be issued in three (3) levels:

.1 Management Level - For officers serving as master, chief mate, chief engineer, and first

assistant engineer (second engineer officer);

.2 Operational Level - For other certificated officers, serving under the direction of an officer at the

management level; and

.3 Support Level - For ratings who are designated to control fire fighting operations.

7.9.3 Certification - It should be noted that “Advanced Firefighting” is a pre-requisite for the issuance of

any Commonwealth of Dominica deck/navigational or engineer officer certificate of competence.

Therefore, obtaining a separate SQC is optional. (see Sect. 2.10.6.)

7.9.4 Revalidation - If the advanced firefighting course is either 1.) approved by an IMO “Reported”

government, or 2.) is being offered under an ISO 9002 or equivalent certified quality assurance system

addressing the STCW 95 standards, then an STCW 95 endorsement or certificate will be issued by the

Administration. Such certificates will be issued for a five-year period at the “management” and

“operational” levels and with no date of expiry at the “support”

level. If the training course fully meets the standards of the 1978 Convention but does not satisfy. either.

of conditions (1) or (2) above, then a “grandfathered” endorsement or certificate will be issued at all three

(3) levels, “management,” “operational” and “support,” with an expiry date of 1 February 2002.

7.10 Certification for “Medical First Aid Provider” (Ref. STCW 95, Code AVI/4, para. 1 to 3)

7.10.1 STCW 95 Requirements - STCW 95 calls for training as “Medical First Aid Provider” for those

seafarers designated to undertake the duties of providing immediate first aid in case of injury or illness

aboard ship. Certification will only be issued upon completion of an approved Medical First Aid Course.

7.10.2 Levels of Responsibility - “Medical First Aid Provider” certification will be issued in three (3) levels:

.1 Management Level - For Master, Chief Engineer, Chief Mate, and First Assistant Engineer;

.2 Operational Level - For officers other than Master, Chief Mate, chief engineer, and First Assistant

Engineer (second engineer officer); and

.3 Support Level - For ratings (capacities), uncertificated crew performing medical related duties

under the supervision of an individual serving in the management or operational level.

7.10.3 Certification

.1 In addition to a copy of the “completion certificate” received upon completion of an approved

First Aid Course, applicants are required to submit a completed Physical Examination

Report/Certificate Form CDMP-5000B (see Annex 1) with the application.

.2 Since this training is a certificate prerequisite for all deck/navigational and engineer officers,

this Special Qualification Certificate is optional for certificated officers.

7.10.4 Revalidation - If the “Medical First Aid Provider” course is either 1.) approved by an IMO

“Reported” government, or 2.) is being offered under an ISO 9002 or equivalent certified quality

assurance system addressing the STCW 95 standards, then an “STCW 95” endorsement or certificate

will be issued by the Administration. Such certificates will be issued with no date of expiry. If the training

course fully meets the standards of the 1978 Convention but does not satisfy either of conditions (1) or (2)

above, then a “grandfathered” endorsement or certificate will be issued at all three (3) levels,

“management,” “operational” and “support” with an expiry date of 1 February 2002.

7.11 Certification for “Medical Care Person in Charge” Aboard Ship (Ref. STCW 95, Code A-VI/4, para. 4 to 6)

7.11.1 STCW 95 Requirements - STCW 1995 calls for the training of persons to be in charge of medical

care on board ship. Certification will only be issued upon completion of an approved Medical Care course

for the level of responsibility sought.

7.11.2 Levels of Responsibility - “Medical Care Person in Charge” certification will be issued in two (2)

levels:

.1 Management Level - For officers serving as Master, Chief Mate, Chief Engineer, and First

Assistant Engineer (second engineer officer); and

.2 Operational Level - For other certificated officers, serving under the direction of an officer at the

management level.

7.11.3 Certification

.1 One Seafarer on board each Commonwealth of Dominica flag vessel must have this

certification.

.2 Certificated Officers may apply for certification as “Medical Care Person in Charge” by

attaching a copy of their “completion certificate” from an approved “Medical Care Person in

Charge” course to a Form CDMP-5000.

.3 Applicants must also-submit a completed Physical Examination Report/Certificate, Form

CDMP-5000B (Annex 1).

.4 An applicant may submit an equivalent certificate issued by a foreign government whose rating

system is accepted by the Maritime Safety Committee of the IMO, in lieu of a copy of “Completion

Certificate” as required in sub-item 7.10.3.1 above. Since this training is a certificated pre-

requisite for Masters and Chief Mates, the additional Special Qualification Certificate is optional

for the certificate holder.

7.11.4 Revalidation - If the “Medical Care Person in Charge” course is either 1.) approved by an IMO

“Reported” government, or 2.) is being offered under an ISO 9002 or equivalent certified quality

assurance system addressing the STCW 95 standards, then an “STCW 95” endorsement or certificate

will be issued by the Administration. Such certificates will be issued with no date of

expiry, If the training course fully meets the standards of the 1978 Convention but does not satisfy either

of conditions (1) or (2) above, then a “grandfathered” endorsement or certificate will be issued with an

expiry date of 1 February 2002.

7.11.5 Ship’s Doctor - This certification is not required of, nor will it be issued to, ship’s doctors or ship’s

nurses. It shall be the responsibility of the shipowner or operator to verify the validity of such persons’

medical certifications.

7.12 Certification for “Ro-Ro Passenger Vessel Training” (Ref. STCW 95, Code A-V/2)

7.12.1 STCW 95 Regulation V/2 - Effective 1 August 1998, masters, officers, ratings and other personnel

designated on muster lists to assist passengers in emergency situations aboard ro-ro passenger vessels

engaged on international voyages shall receive, prior to being assigned shipboard duties, mandatory

minimum training as appropriate.

7.12.2 Specific Training at Levels of Responsibility - The following training shall be the responsibility of and

be accomplished by the shipowner/operator in accordance with the guidelines in STCW 1995, Regulation

V/2, Code A-V/2:

.1 Management Level - In addition to the courses prescribed for Operational and Support Levels

of responsibility listed below, senior officers and every person assigned immediate responsibility

for embarking and disembarking passengers, loading, discharging or securing cargo, or closing

hull openings on board Ro-Ro passenger ships shall have completed approved training

in.passenger safety, cargo safety and hull integrity. (Reference: Code A-V/2, paragraph 4.) Senior

officers and any person having responsibility for the safety of passengers in emergency situations

on board Ro-Ro passenger ships shall have completed approved training in crisis management

and human behavior as stated in STCW Code section A-V/2, paragraph 5.

.2 Operational Level - In addition to the training prescribed for support level, officers and other

personnel designated on muster lists to assist passengers in emergency situations on board Ro-

Ro passenger ships shall have completed training in crowd management. (Reference: STCW

Code Section A-V/2, para. 1.) Officers and other personnel assigned specific duties and

responsibilities on board Ro-Ro passenger ships shall have completed familiarization training

covering the subjects listed in STCW Code Section A-V/2, paras. 2 and 4.

.3 Support Level - Personnel providing direct service to passengers in passenger spaces on

board Ro-Ro passenger ships shall have completed the safety training specified in STCW Code

section A-V/2, paras. 1 and 3.

7.12.2 Documentation of Training - No Special Qualification Certificates will be issued by this

Administration to certify this training. However, vessels are required to have on board documentation

attesting to the completion of this training by the appropriate personnel.

7.13 Certification for “Passenger Vessel Training” (Ref, STCW 95, Code A-V/3)

7.13.1 STCW 95 Regulation V/3 - Effective 1 January 1999, masters, officers, ratings and other personnel

designated on muster lists to assist passengers in emergency situations aboard passenger vessels, other

than ro-ro passenger vessels, engaged on international voyages shall receive, prior to being assigned

ship-board duties, mandatory minimum training as appropriate in:

.1 Crowd management for personnel designated on the muster list to assist passengers in

emergency situations as specified in section A-V/3, paragraph 1 of the STCW Code;

.2 Vessel familiarization for masters officers and other personnel assigned specific duties and

responsibilities as specified in section A-V/3, paragraph 2 of the STCW Code;

.3 Safety for personnel providing direct service to passengers in passenger spaces a specified in

section A-V/3, paragraph 3 of the STCW Code;

.4 Passenger safety for masters, chief mates and every person assigned immediate responsibility

for embarking and disembarking passengers as specified in section A V/3, paragraph 4 of the

STCW Code; and

.5 Crisis management and human behavior for masters, chief mates, chief engineer officers,

second engineer officers and any person having responsibility for the safety of passengers in

emergency situations as specified in section A-V/5, paragraph 5 of the STCW Code.

7.13.2 Lifeboat Crews - Where more than two (2) survival craft/rescue boat crewmen are required to be

assigned to a lifeboat, the crewmembers assigned who are not certified survival craft/rescue boat

crewmen should be selected on the basis of their ability to remain calm, help others during a period of

stress and follow the directions of the certified survival craft/rescue boat crewman in charge of the

lifeboat. Their documented training should include at least:

• The proper way to put on the personal flotation devices and how to instruct others to do so,

• Where applicable, how to put on thermal protective aids and how to

instruct others to do so,

• Where the fire extinguisher is and how to use it,

• Where the pyrotechnics are and how to use them,

• Where the provisions are and how to open them,

• Where the bailers, buckets and bilge pump are and how to use them,

• Where the first aid kit is,

• How to load and seat people safely in the boat,

• How to safely embark and disembark disabled persons and persons in needof assistance, and

• Where the muster list is and how to use it.

7.13.3 Responsibility - The training of personnel other than masters, officers and ratings need only be

related to the duties assigned to those “other personnel.” This training shall be the responsibility of and

accomplished by the shipowner/ operator in accordance with the guidelines in the STCW Code, section

A-V/3.

7.13.4 Documentation of Training - The shipowner/operator shall ensure that documentary evidence of the

training which has been completed is issued for every person found qualified under the provisions of this

Regulation V/3 and is available for review by proper authorities and ISM Code auditors.

7.14 Familiarization Training

7.14.1 Requirement

All persons employed or engaged aboard Dominica seagoing vessels shall receive “Familiarization

Training.” That training shall be the responsibility of and be accomplished by the shipowner/operator in

accordance with the guidelines in STCW 1995 Chapter VI, Section A-VI/1. It is the intent that the training

be completed after a seafarer is assigned to a vessel and prior to the seafarer assuming routine duties

aboard that vessel.

7.14.2 Certification

Since this familiarization training is primarily intended to be vessel specific, no certificate will be issued to

the seafarer by MARAD attesting to the completion of this training. The training shall be documented by

shipboard log entry or other acceptable written verification available for review by proper authorities and

ISM Code auditors.

7.15 Certification for “High Speed Craft” 7.14.1 Certificate

A type rating certificate will be issued to officers who have completed the training as specified in the High

Speed Craft (HSC) Code, Section 18.3. This HSC Certificate will be specific to the type of craft, model,

and, route of service and shall be in addition to the appropriate certification already required of the

officers by the STCW Convention.

7.14.2 Period of Validity

HSC type rating certificates will be issued for a period of two (2) years. The certificates may be renewed

upon application showing at least six (6) months sea service aboard the same type craft within the

previous two (2) years. An acceptable refresher training course may be substituted for the sea service.

7.16 Certification for “Basic Training” - Refer to Section 6.1.4 under Requirements for the Issuance of a Seafarers‘ Training Record Book.

8 Certification Based on Training Certificates 8.1 General Policy

The general policy on the acceptance of Training or Special Qualification certificates and successful

completion of approved In-Service training documented in an approved TRB for the issuance of

Commonwealth of Dominica certificates shall be the following:

8.1.1 Maritime Training Centers - STCW 95 Certificates of Completion issued by maritime training centers

will only be accepted by the Administration if the course is monitored by a Quality Standards System

acceptable to the Administration and the center has been certified by a Quality Registrar or is certified by

an IMO “Reported” national government.

8.1.2 Administrations

Certificates of Training or Special Qualification issued by administrations approved by the IMO will be

accepted by the Administration for the issuance of equivalent Dominica certificates.

8.2 Important Considerations

Seafarers must bear in mind that STCW 78 training course or program certificates are limited in both their

acceptance and validity. Previous Administration course or program approvals to STCW 78 standards are

not acceptable for STCW 95 certification and will no longer be acceptable for any certification after the

following dates:

8.2.1 New Seafarers - New seafarer’s starting their maritime training and/or sea service on or after 1

August 1998 must complete approved training courses that are to the STCW 95 standards.

8.2.2 Existing Seafarers - Existing seafarer’s who started their maritime training and/or sea service before

1 August 1998 must complete the relevant training to STCW 95 standards not later than 1 February 2002.

8.2.3 New Requirements - Evidence of completion of the new STCW 95 training requirements, which

went into effect on 1 February 1997 for all seafarers, must be provided to the satisfaction of the

Administration before certification will be

issued.

9 Dominica Training Record Book (TRB) 9.1 Background The Commonwealth of Dominica Training Record Book is designed for use in both On-Board

practical assessment as well as in training conducted by approved Training Institutions, Schools or

Training Providers. The book is comprised of an Introduction Section, a Personal Identification Section,

Service Record and Information Section, Training and Record Book Review Section, Vessel

Familiarization Section and the Competency and Assessment Tables for License, Ratings and/or

Required Specialty Training. This book is to be kept available in accordance with Regulation 1/2,

paragraph 9 of the STCW Convention while the mariner is serving on a ship.

9.2 Introduction to use of the TRB

The Commonwealth of Dominica Training Record Book is designed for use in both On Board practical

assessment as well as in training conducted by approved Training Institutions, Schools or Training

Providers. This book also is to be used as a Service Record/Discharge Book. The book is comprised of

a Personal Identification Section, an Introduction Section, Vessel Data and Service Record, Vessel

Familiarization Section, Training Record Book Review Section, and the Competency and Assessment

Tables for License, Ratings and/or Required Specialty Training. This book is to be kept available in

accordance with Regulation 1/2, paragraph 9 of the STCW Convention while the mariner is serving on a

ship.

9.2.1 Vessel Data, Service Record, and Vessel Familiarization

Vessel Data sheets are provided and are to be completed for each vessel upon which time is served.

The Master or Chief Officer may complete the data information. One form should be completed for each

vessel the mariner has worked. The Service Record table on each sheet is to be used for discharge

information and proof of sea service. The Master of the vessel must sign each entry and stamp with the

official vessel stamp. If additional service record space is needed, a new Vessel Data Sheet may be

used, completing only the Ship Reference Number, Name and Call Sign information in the Ship Data

section. The ship details do not need to be completed again once a data sheet has been completed

previously.

A Vessel Familiarization attestation sheet must be completed by an officer of the vessel and signed by

the vessel Master for each vessel upon which the mariner serves.

9.2.2 Assessment Sheets: Assessment Sheets are provided and include the following:

Assessments are to be completed by an Approved Instructor or Shipboard Assessor (Master, Chief/1st

Parts 3.1 and 3.2 of this Introduction Section.

Assessment Sheets are not provided for STCW 95 Regulations VI/1 (Basic Safety Training), VI/2

(Proficiency in Survival Craft, Rescue Boats and Fast Rescue Boats), VI/3 (Advanced Firefighting), and

approved training course/school.

9.2.3 Guidance for the Training Officer

.1 Guidance for on-board instructors

Before giving instruction in a particular skill or ability for the purpose of signing off a mariner as

competent in a specified task in this training record book (TRB), the instructor should (a) be

qualified under the relevant regulations to give such instruction, and (b) determine that the

candidate is qualified on the basis of prior experience and/or training, to receive such instruction.

The instructor should also review the TRB to identify what training the candidate has already

completed, and what training remains to be conducted.

In designing an on-board training activity, the instructor should have clear, measurable, training

objectives. These should be organized by reference to other related skills and abilities needed by

the student to achieve the level of competence being pursued. The training objectives should be

approved by the responsible person supervising the training and assessment program.

The instructor should conduct training only when the necessary equipment is operational and will

be available throughout the training exercise.

The instructor should follow an outline, checklist, or training plan, which organizes information

and instructional activities in a logical and progressive manner.

Instructional activities should make effective use of available teaching media such as videotape,

personal computers, and models.

The instructor should ensure the candidate has sufficient opportunities to observe the skill or

ability being properly performed. When the skill or ability requires the use of certain shipboard

equipment, the instructor should ensure that the candidate is given adequate opportunities for

hands-on use of that equipment along with constructive comments directing the candidate to

preferred or proper ways of using the equipment.

Instruction should include explanations of misuse or improper procedure; problems that may be

encountered and proper corrective actions to take; and descriptions of important differences

which may exist from ship to ship.

The instructor should periodically use a reliable means of assessment to determine that

candidate is in fact making progress toward the objectives stated for the instructional activity.

When the instructor is inexperienced, arrangements should be made for his or her early training

activities to be monitored by the person responsible for supervising the training and assessment

program, with the aim of ensuring that training activities are conducted in the most effective

manner possible.

.2 Guidance for on-board assessors/designated examiners

Before assessing the performance of a particular skill or ability for the purpose of initialing this

TRB, the assessor should (a) be qualified under the relevant regulations to perform the

assessment; and (b) determine that the candidate is qualified on the basis of prior experience

and/or training, to be assessed. The assessor should also review the TRB to identify what training

and assessment the candidate has already completed, and what training or assessment remains

to be conducted. Masters, Chief/1st Mates and Chief Engineers and 1st/2nd Assistant Engineers

are considered qualified as a result of their respective efforts in upgrading their status of

competence.

In designing an on-board assessment activity, the assessor should have clear, measurable,

assessment objectives. These should be organized by reference to other related skills and

abilities needed by the student to achieve the level of competence being pursued. The training

objectives should be; approved by the person responsible for supervising the training and

assessment program and must clearly correspond with this particular TRB.

The following guidelines are taken from section B-II/1 of the STCW Code for the conduct of

assessment and should be taken into account:

competence should, therefore, encompass more than the immediate technical requirements of

the job, the skills and tasks to be performed, and should reflect the broader aspects needed to

meet the full expectations of competent performance as a ship's officer. This includes relevant

knowledge, theory principles, and cognitive skills which, to varying degrees, underpin all levels of

competence. It also encompasses proficiency in what to do, how and when to do it, and why it

should be done. Properly applied, this will help to ensure that a candidate can:

work competently indifferent ships and across a range of circumstances;

anticipate, prepare for, and deal with contingencies; and

adapt to new and changing requirements.

.2 The criteria for evaluating competence (column 4 of table A-II/1 of the STCW Code)

identify primarily in outcome terms the essential aspects of competent performance. They are

expressed so that assessment of a candidate's performance can be made against them and

should be adequately documented in the training record book.

.3 Evaluation of competence is the process of:

.1 collecting sufficient valid and reliable evidence about the candidate's knowledge,

understanding and proficiency to accomplish the tasks, duties and responsibilities listed in

column 1 of table A-II/1; and

.2 judging that evidence against the criteria specified in the standard.

The arrangements for evaluating competence should be designed to take account of different

methods of assessment which car. provide different types of evidence about the candidate's

competence, e.g.:

.1 direct observation of work activities (including seagoing service);

.2 skills/proficiency/competency tests;

.3 projects and assignments;

.4 evidence from previous experience; and

.5 written, oral and computer-based questioning techniques.

One or more of the first four methods listed should be used to provide evidence of ability, in

addition to appropriate questioning techniques to provide evidence of supporting knowledge and

understanding.

Before conducting the assessment, the assessor should:

.1 familiarize himself or herself with the assessment criteria [in column 4 of the relevant tables in

section A-II/1 of the STCW Code] to ensure that the assessment activities will be effective and

comprehensive;

.2 develop scenarios which involve a sequence of events that require the candidate to exercise

good judgment in a realistic amount of time; that include distracters (such as equipment

malfunctions) which test the candidate's ability to react properly to abnormal or emergency

circumstances: and that require the candidate to make effective use of all relevant and available

human resources, hardware and information.

.3 ensure the necessary equipment is operational and will be available throughout the

assessment activity;

.4 be able to articulate the parameters or thresholds, which will, under the circumstances,

represent an acceptable level of performance;

.5 clearly explain to the candidate the purpose of the activity and the steps he or she is to take

during the demonstration of the skill or ability;

.6 ensure that the candidate (a) can concentrate on the task(s) at hand; (b) will not receive

unauthorized assistance during the assessment process; and (c) is not in a position to "learn the

test" by watching the performance of other candidates;

.7 inform the candidate as to the scope and depth of knowledge to be assessed, the length of

time allowed for the demonstration, and the effect of failing to perform part of the demonstration

properly; and establish the candidate's willingness to be assessed under the circumstances

presented.

The assessor should continuously observe the candidate during performance of the skill or ability

and should only note in the training record book when the performance is acceptable. In the event

the candidate does not perform a critical phase of the assessment exercise at an acceptable level

of proficiency, assessment should be suspended and should not be conducted until further

instruction is provided.

.1 Successful or acceptable performance; should be based on the candidate's proved ability to

safely perform:

.1 the assigned tasks in accordance with competency criteria identified in the training

record book;

.2 such tasks in a manner which demonstrates that the required level of skill, knowledge and

ability was never in serious doubt; and

.3 such tasks in a manner that demonstrates sound and professional judgment.

.2 Unsuccessful or unacceptable performance may be based on the candidate's failure to prove

his or her ability in accordance with paragraph 6, or because the candidate otherwise performs

improperly in the judgment of the assessor, based on events such as the following:

.1 an action, or lack of action, by the candidate which required corrective action or

intervention by the assessor to prevent injury, damage, or the development of a hazardous

condition;

.2 the candidate failed to use proper procedures (including appropriate communication

procedures);

.3 the candidate failed to take prompt corrective action when required.

.3 Normally, a single demonstration of skill immediately following instruction should not be relied

upon as the sole basis for judging competence.

.4 When the assessor is inexperienced, arrangements should be made for his or her early

assessment activities to be monitored by the person responsible for the training and assessment

program, with the aim of ensuring that assessment activities are conducted in the most effective

manner possible.

1. READ INSTRUCTIONS CAREFULLY. Enter all required information. Please use typewriter or print legibly. Failure to properly complete this application or to submit required supporting proofs, etc. will result in rejection of the application or delay its approval.

2. WHERE TO APPLY. Applications must be submitted, by mail or in person, to: Dominica Maritime Registry, Inc.

(a) Three (3) photographs - facial front view of applicant, passport size, taken within one (1) year preceding application with applicant's signature and name printed on reverse side. Photos may be black and white or color and be 2.0" x 2.0" (50 mm. x 50 mm.) in size. Please do not staple.

(b) Medical Requirements - All applicants, except as stated below, shall be required to have a physical examination reported on Form CDMP-5000B, Physical Examination Report/Certificate, completed by a licensed medical doctor. The original of Form CDMP-5000B must accompany the application. The applicant must retain a duplicate copy of Form CDMP-5000B, clearly labeled ‘certified copy’ on its face and initialed by the examining physician, as evidence of physical qualification while serving on board Commonwealth of Dominica flag vessels. The physical examination must be carried out not more than 12 months prior to date of making application. In addition, the following minimum requirements apply: (1) All applicants must have hearing unimpaired for normal sounds. (2) All applicants must have average blood pressure, taking age into consideration. (3) Applicants afflicted with or having medical histories such as follows shall be disqualified for seafarer's identity document: epilepsy, insanity, senility, alcoholism, tuberculosis,

acute venereal disease or neuro-syphilis, AIDS, and/or the use of narcotics. (c) Applicants holding a Commonwealth of Dominica certificate which has been issued within 12 months prior to the date of application are not required to undergo a

physical examination. In this regard, they must present their Commonwealth of Dominica certificate and include a copy of the certificate with application for the Seafarer's Identification Document (SID) and/or Training Record Book (TRB).

(d) Identity Document - A copy of a valid passport or other official national identity document, birth certificate, certificate of naturalization, or baptismal certificate or parish record recorded within one year of birth along with a valid seaman's document from another maritime nation (if applicable). A copy of these documents must be submitted with the application. Applications for renewal must include a copy of the previous document as proof of identity. If the documents presented are not in the English language, a certified translation of the documents into English must also be enclosed with the application. These certified translations must identify the translator for verification purposes.

(e) One letter of recommendation - from applicant's most recent Master or employer if applicant is applying for his or her first Dominica Certificate of Competence. Not required for endorsement or renewal or upgrading of a Dominica Certificate of Competence.

(f) Proof of sea service - submit proof of at least the minimum service required to establish eligibility for the license requested. Applicants for Certificate of Competences based on an acceptable national Certificate of Competence must submit at least one (1) year’s total sea service in the last five (5) years. Applicants for Dominica examination must submit the total sea service as specified in paragraph 8. Proof of sea service can be established by providing a photocopy of the applicant's official "Seafarer's Discharge Book." For service on Dominica ships, a photocopy of the applicant's Dominica Training Record Book, CDP-302, should be provided.

(g) Proof of foreign Certificate of Competence - The foreign Certificate of Competence must have a valid “STCW Convention Endorsement” and be issued by a country recognized by the Commonwealth of Dominica. Submit original foreign certificate (which will be promptly returned to the applicant) or a duplicate original of the foreign certificate which is relied upon; or a clear photocopy of the original authenticated by the issuing authority or by a consular officer of the issuing state; or a clear photocopy of the original plus a current certificate of the issuing authority describing the certificate and the holder so as to identify both; or a clear photocopy made by a notary public, commissioner for oaths or other officer authorized by law to administer oaths, verifying that such officer made the photocopy from the original presented to him, and attaching the customary certificate of a public official authenticating the signature of such notary, commissioner or officer.

(h) Proof of completion of nautical, marine engineering or radio school training - submit one (1) copy of course completion certificate. (i) All documents not presented in the English Language must have certified English translations attached and enclosed with the application. (j) Renewal of Dominica STCW’95 Certificate of Competence - submit the certificate to be renewed (or certified copy of the certificate), proof of at least one (1) year's sea service

within the last five (5) years (or employment ashore directly related to the operation of vessels - e.g. Port Captain, Port Engineer). A Dominica Certificate of Competence which has expired for more than one (1) year cannot be renewed. A completely new set of documents must be filed.

(k) Proof of foreign training certificate - radar simulator training, advanced firefighting training, GMDSS certification, first aid training, advanced medical training, survival craft/rescue boat training, and automatic radar plotting aids (ARPA) training - submit one (1) copy of foreign certificate if national Certificate of Competence does not require this training. 6. TRAINING REQUIREMENTS

(a) Firefighting training - Every candidate for a Master or Mate’s certificate or an engineer Certificate of Competence for service on vessels of over 3000kW propulsion power must submit proof of completion of an advanced marine firefighting course. (b) Radar simulator training - Every candidate for a Master or Mate’s certificate must submit proof of completion of a marine radar simulator course. (c)* GMDSS Certification - As of 1 August 1998 every candidate for Third/Second Mate/Mate’s certificate must hold GMDSS Certification. (d)* First aid training - As of 1 August 1998 every candidate for a Second Mate/Third Mate’s certificate on vessels over 500 gross tons; every candidate for a Master/Mate’s certificate on vessels less than 500 gross tons and every candidate for an engineer officer’s certificate serving on vessels of over 3000kW propulsion power must complete first aid training. (e)* Survival craft/rescue boat training - As of 1 August 1998 every candidate for a Master, Chief Mate, Second Mate, Third Mate or Mate’s certificate and every candidate for an engineer Certificate of Competence on vessels of over 3000kW propulsion power must complete survival craft/rescue boat training. (f)* Automatic Radar Plotting Aids (ARPA) training - As of 1 August 1998 every candidate for a Master, Chief Mate, Second Mate, Third Mate or Mate’s certificate must complete automatic radar plotting aids (ARPA) training. * These requirements do not become effective until 1 February 2002 for seafarers who commenced their maritime training and/or sea service prior to 1 August 1998. NOTE: National Certificate of Competences containing the 1995 STCW Endorsement are evidence of completion of the training requirements of (a) through (f) above without having to submit training course certificates.

7. GLOBAL MARITIME DISTRESS AND SAFETY SYSTEM (GMDSS) CERTIFICATES (a) Restricted Operator - Previous service not required. The candidate must pass the written examination for a certificate as GMDSS Restricted operator or present a foreign certificate in equivalent grade. (b) General Operator - Previous service not required. The candidate must pass the written examination for a certificate as GMDSS General Operator or present a foreign certificate in equivalent grade. (c) Second-Class Radio Electronic Operator and Maintainer - Previous service not required. The Candidate must pass the written examination for a certificate as Second-Class Radio Electronic Operator and Maintainer or present a foreign certificate in equivalent grade. (d) First-Class Radio Electronic Operator and Maintainer - Previous service not required. The candidate must pass the written examination for a certificate as First Class Radio Electronic Operator and Maintainer or present a foreign certificate in equivalent grade.

8. SEA SERVICE REQUIREMENTS. Applicants for Dominica examinations must prove the "total" amount of sea service/marine training required for the grade of certificate for which application is made; e.g. an applicant for Second Mate must show three (3) years service for Third Mate and one (1) year service as a certificated Third Mate or five (5) years service in the deck department. For sea service requirements for Dominica examination see REQUIREMENTS FOR MERCHANT MARINE PERSONNEL CERTIFICATION (CDP-300) obtainable upon request to the Office of the Maritime Administrator. Applicants for equivalent Dominica certificates to their national Certificate of Competences must show one (1) year’s sea service in the last five (5) years. (a) Deck/Navigational Officers - Required service shall have been on board ocean-going vessels of 3000 or more gross tons. At least one (1) year of the total service required shall have been within five (5) years preceding the date of application. (b) Engineer Officers - Required service shall have been on board steam or motor vessels of 3000 or more kW propulsion power. At least one (1) year of the total service required shall have been within five (5) years preceding the date of application. At least six (6) months of the total service required shall at some time have been served in the mode of propulsion applied for.

In the event approval is not granted, the documents listed in paragraph 3 above together with the fee shall be returned to the applicant at the mailing address indicated. If the application is approved, the SID/TRB will be sent to the mailing address indicated. The ‘certified copy’ of Form CDMP-5000B is to be retained by the seafarer as evidence that the application is being processed.

12. Seafarer’s ID: This identification document conforms to the requirements of the International Labor Organization (ILO) Convention No. 108 (Seafarer's Identity Documents Convention, 1958). It is issued to seafarers of Commonwealth of Dominica flag vessels for use when traveling to or from an assigned vessel or pursuant to instructions from the master of such a vessel. Other uses of the book must be in conformity with Commonwealth of Dominica regulations. ILO 108 does not in any way restrict the right of a member nation from preventing any particular individual from entering or remaining in its territory.

Seafarer's Identification Document and Training Record Books are issued under the authority of Chapter 8 of International Maritime Act 2000, as amended. The SID and TRB remains the property of the Maritime Administrator and may be withdrawn at any time. It may not be altered in any way nor be allowed to pass into the possession of an unauthorized person. If the TRB becomes filled with entries, requires alteration, becomes damaged, application for a replacement TRB should be made immediately. If the SID/TRB is stolen, lost, or accidentally destroyed, notification should be given immediately to any office of the Maritime Administrator, and an application for a replacement SID/TRB may be made.

MEDICAL REQUIREMENTS All applicants for an STCW’95 Certificate of Competence, Seafarer’s Identification Document and a Dominica Training Record Book or certification of special qualification shall be required to have a physical examination reported on this Medical Form completed by a certificated physician. The completed medical form must accompany the application for Certificate of Competence, application for seafarer’s identity document, or application for certification of special qualifications. This physical examination must be carried out not more than 12 months prior to the date of making application for an Certificate of Competence, certification of special qualifications, or a seafarer’s book. Such proof of examination must reestablish that the applicant is in satisfactory physical condition for the specific duty assignment undertaken and is generally in possession of all body faculties necessary in fulfilling the requirements of the seafaring profession. In addition, the following minimum requirements shall apply: 1. All applicants must have hearing unimpaired for normal sounds and be capable of hearing a whispered voice in the better ear at 15 feet and in the poorer ear at 5

feet. 2. Deck officer applicants must have (either with or without corrective lenses) at least 20/20 vision in one eye and at least 20/40 in the other. If the applicant uses

corrective lenses, he must have vision without corrective lenses of at least 20/160 in both eyes. Deck officer applicants must also have normal color perception and be capable of distinguishing the colors red, green, blue and yellow.

3. Engineer and radio officer applicants must have (whether with or without corrective lenses) at least 20/30 vision in one eye and at least 20/50 in the other. If the applicant wears glasses, he must have vision without glasses of at least 20/200 in both eyes. Engineer and radio officer applicants must also be able to perceive the colors red, yellow and green.

4. An applicant’s blood pressure must fall within an average range, taking age into consideration. 5. Applicants afflicted with any of the following diseases or conditions shall be disqualified: epilepsy, insanity, senility, alcoholism, tuberculosis, acute venereal

disease or neurosyphilis, AIDS and/or the use of narcotics. 6. Deck/Navigational officer applicants and Radio officer applicants must have speech which is unimpaired for normal voice communication. 7. Applicants for able seaman, bosun, GP-1, ordinary seaman and junior ordinary seaman must meet the physical requirements for a deck/navigational officer’s

certificate. 8. Applicants for fireman/watertender, oiler/motorman, pumpman, electrician, wiper, tankerman and survival craft/rescue boat crewman must meet the physical

requirements for an engineer officer’s certificate.
