instructions. 6. It was stated to insure Special Inspection and Testing Services are addressed. Refer to Ohio

Building Code, Section 1704.1.1-2. This is required for all projects that require Construction Building Permits from the State.

7. It was stated that Prevailing Wage requirements are required. 8. Progress Schedule: The following items and schedule were noted.

Critical path schedule is to be developed by the General Contractor, with the input from all of the other sub-contractors. No pay application will be considered until the schedule is approved by all parties.

The schedule line items should coordinate and correspond with the schedule of values (SOV). Pre-Bid: Wednesday, April 17 at 11:00 AM @ Cole Service Building Contract Documents are available for purchase from Miami University, Office of Facilities

Contracting, 181 Cole Service Building, Oxford, Ohio 45056, for $50 per set, payable to MIAMI UNIVERSITY

Final day for questions: Friday, April 19 at 2:00 PM Final bid due date/time and opening: Thursday, April 25, 2019 at 2:00 PM

• Sealed bids, based upon the following estimates will be received for General Contractors, published bid estimate $416,387

Progress completion/turn over and training if needed all completed by August 2, 2019.

9. Miami University “Front-End Specification Overview” a. The bidding conditions are described in State of Ohio Statue153 and should be followed for this

project. Bids are due in a sealed envelope.

b. Bid forms and other materials are available on Miami’s FCO website (www.pfd.miamioh.edu/fco)

c. Documents can be purchased from Miami University for a non-refundable cost of $50.00 plus shipping.

d. Betsy reminded everyone to review front-end Specification bidding information.

e. The yellow pages in the document represent Prevailing Wage information.

f. Completed Bid Form

i. Two (2) forms are needed to be filled out: Bidder Affirmation and Discloser forms.

ii. The Commitment to Participate in Edge form has three options: A, B & C. Betsy explained these options. In order to be a responsive bidder one of the three options must be check with the document and submit along with the bid.

iii. The back of the Bid Form should have a signature of the person who is authorized to commit the company to a contract. The Bid Form must be signed with “wet signature(s).”

iv. Betsy described bid security options that must be followed.

v. Contractor must submit Bid Security form. MU prefers the hybrid Bid Guarantee & Bond type or if bonding isn’t achieved a security amount of 10% is required of the bid amount in the form of a cashier’s check, certified check or letter of credit.

g. Apparent bidder award requirements:

i. A current EEO Certificate must be submitted.

ii. Insurance requirements must be met as evidenced in an accord form submittal.

iii. A current Workers Compensation Certificate must be received.

iv. A Bidders Qualification Form must be submitted. Norman recommended submitting this with the bid in an effort to expedite the post bid review process. TAG will check and review the details of apparent successful bidder and review qualifications.

v. A certificate from the Bureau of Workman’s Compensation is required indicating compliance with their Drug-Free Safety Program (DFSP).

h. During the project payment applications must be supported by certified payroll reports or the pay request will not be processed.

i. The project is tax exempt. MU will provide tax exempt certificate. 10. Project Description: Architecture, Engineering

Jacob and Norman presented a brief walk-through of the scope of the construction drawings. 11. Items to not miss in documents: The following items were pointed out specifically and reviewed.

A designated parking area will be established by the General Contractor for parking, lay down area, and receiving materials. Review 00 73 00 Supplementary Conditions (General Contracting) 6.10.9 for fencing requirements.

E1.1 and E1.2 Plan Note #1 – All fixtures to be removed, existing 2X2 fixtures have LED retro fit kit installed. Electrical contractor shall remove LED retrofit kit and return complete kit to Owner. Electrical contractor shall dispose existing housing, remove existing LED flat panel fixtures in select location and return to Owner.

E1.1 Note #2 – Track lighting to be removed and returned to Owner. F1.1 Plan Note to be added by Addendum #1 – replace all existing sprinkler heads with new

concealed sprinkler heads. Turn over existing sprinkler heads to Owner.

12. Project Management Reminders: The following reminders were reviewed. a. The project is single prime with "General Contractor" contracting Mechanical, Electrical, and

General Subcontractors as required. b. Overtime/Premium time is to be included in the bid as needed. c. Parking Procedures

1. See the attached construction fencing/laydown/parking plan. 2. If the contractors run out of room and need to park elsewhere, they will need to purchase

a parking pass from parking services for a time period of no more than 30 days. 3. Red Parking passes are $35 per month and are the responsibility of the contractor. 4. See PM for approval of parking pass.

d. State Inspections: General Contractor to obtain and schedules required inspections. e. Project Clean-Up

1. Contract Documents 2. Daily cleanup is required. 3. Salvage Materials: Nothing may be burned or sold on site.

f. Deliveries: Must be received at project site. MU will not accept deliveries for job sites.

13. Bid Questions: The following were noted. a. Bid questions must be in writing (email only). b. All questions regarding the front end documents must go through Miami University Facilities

Contracting Office (FCO). c. All technical questions must go to The Architectural Group, Inc. (Both Norman Butt and Jacob

Griffin) with a copy to Tricia Price at Miami University. d. Individual tours may be scheduled as requested through MU Project Manager.

14. Addendum One will be published by Monday - April 22, 2019 by 2:00 pm. Document 00 10 00 – Solicitation General Contracting

• Revise “Trade, General Contract ……. Estimate $478,387 to read bid estimate at “$416,387”

• Pre-Bid meeting minutes will be published along with attendee sign-in sheet • The Art Building was built in 1985; Miami only has asbestos reports for those

buildings built in 1981 or earlier. Test samples have been taken of the stair mastic and tiles on the Second Floor. Test results will be published in the Addendum.

• All correspondence/questions should be emailed to Norman Butt, TAG at [email protected] and Jacob Griffin, TAG at [email protected] no later than Friday – April 19, 2019 by 2:00 pm.

Site Construction Start: Monday, May 20, 2019 Substantial Completion: Friday, August 2, 2019

16. Project Site Visitation Walkthrough: The following were noted during the walkthrough:

a. The corridors and stairwells were reviewed by attendees. b. The scope of painting was discussed that some high and difficult to reach areas would need to

be addressed in the bids. c. It was pointed out that considerations were made on the drawings to how the drywall on the

exterior wall faced with quarry tile would be detailed. d. The painting of the backside of the drywall black to be installed covering the windows was

pointed out. e. It was noted by Tricia that the water fountains on the basement and second floors should be

replaced similarly to that on the first floor. f. Bathrooms were reviewed by attendees. g. It was noted that some of the existing wall base that is to be removed in the basement goes

under some of the existing wall material.

Sample condition acceptable upon receipt except where noted. Estimates of concentration are semi-quantitative and are made on an area basis. Results apply only to portions of samples analyzed. Analyst responsible for technical content of report.The reporting limit (RL) for asbestos in bulk materials is 1% and is a function of the quantity of sample analyzed, the nature of any matrix interferences, sample preparation, and fiber size and distribution. Results reported as ND indicate that no asbestos was detected. Results reported as "Trace" indicate that asbestos was detected at some level confidently determined to be <1% which is considered inconclusive according to New York ELAP. Results reported as "Near 1%" indicate that while asbestos was detected at a level confidently determined to be <1% as prepared, the inherent uncertainty of the quantification technique(s) employed and the concentration nearing the 1% mark necessitate the recommendation that verification of these results by a more accurate and precise method be made. ALS performs variety of PLM methods for asbestos in bulk building materials including EPA 600/R-93/116, NIOSH 9002, ELAP 198.1, and ELAP 198.6. In addition, we perform a modified uncertified version of EPA 600/R-04/004 for asbestos in vermiculite which reports asbestos as present or absent only, an in-house developed uncertified method ALS SOP ENV 004 for asbestos in soil, and asbestos in soil by ASTM D7521. Regardless of the method requested, all samples are examined according to mandatory method protocol. Any optional method protocol are eliminated from the initial analysis but may be performed upon client request. These may include; insufficient sample volume rejection*, phase separation of layered or heterogeneous samples, ashing to remove organic interferences, acid dissolution to remove mineral carbonate interferences, point counting**, and analysis by transmission electron microscopy (TEM) to verify ND results.All samples are examined by stereomicroscope for the determination of homogeneity, texture, friability, color, and extent of fibrous components. Non-asbestos materials such as foil, paper, metal, plastic, pebbles, or organic debris are ignored and a subsample of the remaining material homogenized by some means for examination by polarized light microscope (PLM). Information obtained via both stereomicroscope and PLM are used in the final qualitative and quantitative analysis of fibrous components. insulation, concrete, etc., are not included in the soil analysis. If present, these are considered possible asbestos containing materials (ACM) and may be analyzed as separate samples upon client request.*Sufficient sample volume is material dependent. For samples such as floor tiles, roofing felts, sheet insulation, etc., three to four square inches of the layered material is preferred. For materials such as ceiling tiles, loose fill insulation, pipe insulation, etc., one cubic inch (~15cc) is preferred. For samples of thin coating materials such as paints, mastics, spray plasters, etc., a smaller sample size may be suitable. For vermiculite analysis, a one gallon ziploc bag full of dry, loose material is acceptable. For ENV 004 soil samples, a 4oz jar is recommended. The ASTM D7521 Soil method requires a minimum of 8oz and a maximum of 16oz of homogeneous soil.**PLM samples at or near the 1% detection limit may be analyzed by the 400 point count analysis which refers to method EPA 600/M4/82/020, or AHERA method EPA 40 CFR Part 763, Sub. E, App. E as these are synonymous.
