What is this book about?
This book is from CCS Disability Action.
It is about our Contract Board services for disabled people who
live with a host family.
It can be a good way to stay part of
your community.
Living with a host family means that:
their home becomes your
home too
you share a lot of everyday
life with them.

Our contract board services can
support you to keep doing things
that you care about like:
working
spending time with friends
doing things in your community.
To make this happen we work with:
you
your host family.

pages.
The kinds of support we offer

There are 4 kinds of support that weoffer:
1. Good information
2. Support to take part in
your community
3. Support to make choices
4. Advocacy.

1. Good information
It can be hard to get the
information you need at the right
time.
CCS Disability Action can give you
information about things like:
disability
services and activities in
your community
help that you can get with money
the choices you have
your rights

2. Support to be part of your community
We can support you with things like:
finding things to do that
you are interested in
getting to know people
in your community
finding people whose
life has been like
yours in some way
starting study
starting work.

3. Support to make choices
We can support you to plan for the
life you want.
We can help with:
talking about what your
dreams are
working out steps
towards your dreams
finding your way
through systems
that can help you
supporting you with
things that get in the
way of your plans.

We can also support you to get
things you need like:
support from other people
things that help
you to get around
things that help
you to
communicate
changes so that
your house works
for you
money to pay for the
kind of things on this
list.
We can help make sure that
services work together so you get
the right support at the right time.

4. Advocacy
We can go to meetings with you.
We can also work with people so
they understand what you want.
Advocacy can include things like:
o supporting you to
decide what you want
o supporting you to
speak for yourself or
your family
o supporting you to
work with another
organisation.
How we work together

We will treat you with respect.
We will do things in a way that is
right for your culture.
We want you and your family /
whānau to know what is going
on.
We will share information in a
way that works for you.
You get to make the decisions
that affect you.

You will be able to see if we are
doing what we said we would do.

deal with any problems that
come up.

stay positive

Our support does not cost
anything. It is free.
There are 2 ways to start using our
services:
Your Needs
Assessment and
Service Coordination
agency (NASC) can
contact us.
You can contact us yourself.
On page 15 are the ways you
can contact us.

Together we can talk about
whether Contract Board is the
right service for you.
If it is not then we can work out
what other services might be better
for you.

disability service provider that
works all over New Zealand.
We offer:
information
support
advocacy.
CCS Disability Action speaks up
so that everybody gets their
rights.
We support people to have
choices and control in their own
lives.

families / whānau.
We also work in the community
to make it easier for everyone to
take part.
Our services are flexible.
Being flexible means that we will
change how we do things to meet:
