MUST BE COMPLETED BY ALL COACHING STAFF(It is anticipated that this form will be filled out approximately every 2 years unless there is a change in details

and then a new form will have to be filled out to reflect those changes.

To make the paperwork process more streamline, it would be appreciated if you could bring along copies of your ID or ID to be copied

EXISTING BLUE CARD HOLDERS(A copy of your Bluecard will be required.)

Your blue card needs to be validated .Do not assume that just because you have a card that it is VALID for BGS.

There are a number of different forms that need to be filled out depending on your Blue Card Status

1. Authorisation to ConfirmIf you have a current blue card and it was not generated by BGS you will need to fill this form out.

2. Exemption Card Application This form is to be completed by Queensland registered teachers and Queensland police officers proposing to start or continue in child-related employment.

3. Volunteer to Paid Transfer This form is to be completed where a volunteer applicant/blue cardholder is proposing to undertake child-related activities in a paid capacity and needs to transfer from a volunteer 'V' card to a paid 'P' card.
