#5The PBS Implementation Checklist (PIC) is designed to provide schools a snapshot of where they are in terms of implementing PBS in their school. The PIC provides a snapshot of how implementation looks across the Tiers and more comprehensively how implementation is occurring within Tier 1.

The PIC is completed twice each year; in October and in February to serve as a guide to action planning and on-going team activities related to behavior support in the school.

Florida PBS Website: http://flpbs.fmhi.usf.edu/#7Log onto the PBSES evaluation system through Floridas PBS website; either through the Coaches Corner on the left side of the homepage, or through the PBSES LOGIN link at the top right of the page.

Log in here.Directions, username and passwords will be provided by DC.All are CASE sensitive.

If you have not entered the information for the current year and the evaluation system is open for data entry, you will see the words Pending Submission. Select Proceed to continue to data entry.

Review purpose of PIC.Click Proceed. Complete the PICPBS Coach scores the PIC having knowledge and experience with the team. Takes ~15 minutes to read/score 44 items.#14This page gives a brief explanation of the PIC along with instructions for the coach on completing the instrument. As a coach, you will score each of the 44 items on the PIC using your knowledge and experience as a coach with this school. Each item is rated on a scale of 0 to 2 with 2 being Yes, 1 being somewhat and 0 being no.

#15Before scoring the items, select your name from the dropdown coach name list. If your name is not on the list, select other. Rate each item. You must answer every item; no not leave any blank. Answer to the best of your knowledge. The first several sections contain questions related to Tier 1:Universal PBS

Tier 2: Questions 28-34

Answer all Items, even if your school is not implementing Tier 2 supports.#16Items 28-34 contain items related to Tier 2: Supplemental PBS. Even if your school is not implementing at this tier, answer each item.

Tier 3 : Questions 35-44

Answer all Items, even if your school is not fully implementing Tier 3 supports.#17Items 35-44 contain questions related to Tier 3 Intensive level supports. Again, if your school is not implementing or not fully implementing Tier 3 supports, still answer each question.

If you wish to print a copy, do so BEFORE submitting.You MUST click Submit for the data to be entered.Saving and Submitting PIC Data#18After scoring each of the 44 items you may print the form before submitting if you wish to retain a paper copy. However, you can always view evaluation items submitted in the current year and from previous years through the main menu. Whether you print the PIC at this point or not you MUST click the submit button in order for the data to be transferred.

After submitting, you will receive this message verifying the data has been entered.

#19When you have answered each of the 44 items and submitted the data, you will receive this message verifying that your data has been successfully entered. You can go back to the main menu through the link below that text, or by selecting from the menu at the top of the screen.

After submission, the data entry menu will read Completed. Click View to see the completed instrument.#20After you have submitted the data; the mid-year evaluation menu page will look like this; with the word completed and an option to view the current years data as submitted.
