This is a wonderful accomplishment in your goal of advancing to the 2018 US Youth Soccer National Championships in Frisco, Texas, and competing for a National Championship. Work to host this event began over three years ago. I have been working with the Local Organizing Committee over the past few years to help ensure an amazing experience for you in Westfield. The 31 fields at Grand Park are beautiful – the finest complex in the country. Following this letter are a variety of items to help prepare you for your upcoming trip to Westfield. We will update the event updates page of our website throughout the event. We will also update our Facebook page (US Youth Soccer Region II) and Twitter feed(@USYS_RII) while we’re in Westfield. Please note that the schedule of games will be available upon completion of all State Cup and Midwest Regional League qualifying events. It is scheduled to be released on Tuesday, June 12. The preliminary schedule grid, without teams or brackets, is already available for download from the link on our website. Be sure to visit the Team Info page of our website and follow the link next to Event Management System Registration for Team Manager Instructions to ensure that you register your team for the event and enter your team’s roster for approval prior to the deadline provided by your State Association. Important Team Information

Note: Information in this document does not replace the need to be familiar with the “Rules & Policies for the US Youth Soccer Midwest Regional Championships” available at: championships.usyouthsoccer.org/midwest

Members Passes and Bench Pass Credentials

Each player and team official listed on a team roster must have a valid US Youth Soccer member pass at the competition. The US Youth Soccer member pass must carry a current photograph of the player or team official. All member passes shall be laminated. Teams without US Youth Soccer member passes shall not be allowed to participate in the Midwest Regional Championships.

Bench Personnel are limited to four (4) individuals (this includes trainers, head coaches, assistant coaches, managers, club coaches, and/or any other team officials) per technical area per game. Each individual must have a valid US Youth Soccer pass and wear a “Bench Pass” credential at all times while in the technical area. Bench Pass credentials are provided by your State Rep. Be sure to bring them with you as replacements will not be available from the Midwest Region at the event.

Uniforms

Each player shall have a number on the player’s jersey. The number shall be affixed to the back of the jersey and shall be clearly visible. Each player on a team must wear a number different from the number of every other player on the team. No taped or altered numbers will be allowed.

The home team shall wear light colors. In the event of a color conflict, the home team shall change to an alternate set of uniforms.

Each team shall wear the "State Champion" patch presented to them at their state championships on the right sleeve of one (1) set of jerseys. Each player, team and team official may only have on his/her/their apparel the name, logo, or other identifying mark of US Youth Soccer or a member directly or indirectly of US Youth Soccer.

US Youth Soccer Concussion Protocol

Please be advised that after a player receives a completed US Youth Soccer Concussion Form due to displaying signs and symptoms of a concussion, that player may not return to play until a signed clearance form from a medical doctor or doctor of osteopath who specializes in concussion treatment and management has been received. Please refer to the form for further details. The form is available on our web site at championships.usyouthsoccer.org/midwest. For clarification, whether an M.D. or D.O., the physician must specialize in concussion treatment and management.

The clearance must be on a document from the medical doctor's office stating that the player has been cleared to return to play and is clear of symptoms. The document must be signed by the official.

If a player is cleared during the event, please notify your State Rep and bring the signed clearance to the event headquarters. If a player is cleared post-event, please contact the US Youth Soccer National Office at 972-334-9300.

The ATCs contracted for this event are considered primary and any team or club ATCs are secondary.

To: State Champions, Midwest Regional League Qualifiers, State Representatives, and State Presidents From: 2018 US Youth Soccer Midwest Regional Championships Westfield Local Organizing Committee RE: Event Updates and Information The Indiana Soccer Association, the Local Organizing Committee (LOC), and the City of Westfield are proud hosts of this tremendous event. We look forward to welcoming you to our great city and look forward to having you return sometime. Below is some important information for you as attendees and participants in the event. Please read carefully. Games at Grand Park All games will be played on the 31 beautiful fields at Grand Park in Westfield, IN. A map of Grand Park is included with this memo. Please note that fields 2-8 are synthetic turf fields, while fields 9-31 are natural grass fields. This will also be noted on the official schedule. Animals Animals are NOT allowed at the park (exception: service animals with proper documentation)! Tobacco & Alcohol Grand Park is a NO-Smoking facility. Furthermore, alcohol may not be brought into the premises. Please be respectful of those around you. This is for the kids. Welcome Tent A welcome tent area will be available at Grand Park (at the south end of fields 24 & 26)) to assist with directions, housing, and team goody bag pick-up. Hours on Thursday, June 21, will be 3:00pm-7:00pm. Teams can also pick up their goody bags any time on Friday, June 22. Team Goody Bag Pick-up The Midwest Region is providing each team with a Nike backpack. Inside are event pins, flyers, coupons, and other information from US Youth Soccer, the LOC, and our sponsors. Team managers must pick up their team’s bag at the welcome tent at Grand Park, during the hours listed above. Parking at Grand Park There is no parking fee at Grand Park. However, to help with parking flow, please carpool as much as possible and park in the lot closest to your field.

Hotels Hotel accommodations for all teams were selected by your State Association based on the Midwest Region hotel draw. Site Search handles all hotel reservations through. You should have received information from your State Association or Midwest Regional League. Site Search will be at the Park during the event and additional information is included in this informational packet. Registration Team registration takes place in advance through each state association prior to arrival in Westfield. There will be NO on-site team registration, other than the check-in conducted by the referees prior to each game. Questions about the event can be answered by each state association’s representative. Photos All Star Action Photography will be the official photographer for the event. Coaches or managers need to register at the website below to schedule a time for a team picture. All teams must have a team photo scheduled before noon on Sunday, June 25. Orders will be completed on site by the end of play, Sunday, June 25. All Star will also be offering PRE-ORDER Discounts on Team Packages and Individual orders. If you have any questions, please contact them at 248-477-9500 or www.allstarproductions.us Videos Sports Video USA will be the official video service provider for the event. They film entire soccer games in a professional manner using HD cameras mounted on 30-foot tall tripods. They are offering a special team package for $540 for all three preliminary round games; $380 for two games; and one game for $199. If you would like them to film any games, simply email your team information (team name, age group, gender) to [email protected] in advance. Availability is limited and demand is incredibly high for this event, so please do schedule as soon as possible to ensure that your games are filmed. Severe Weather Information The Midwest Regional Championships committee and staff will be constantly monitoring weather conditions using high-tech weather tracking equipment and lightning detectors. They will be in constant contact with the Grand Park Operations staff if severe weather is approaching. Should it be deemed necessary to suspend games due to severe weather, it will be announced using the parkwide PA system which includes verbal message, warning horns signals, and flashing red lights in the park. Information will also be communicated to teams by field marshals from the event committee. As soon as the warning is sounded, everyone must remove themselves from the fields and seek immediate shelter in their vehicles. Do not seek shelter in a building. There is simply not enough room. Grand Park Operations will give the “all clear” signal. At that point players, coaches, referees, and fans may return to the fields. Schedule updates and other event specific announcements may also occur during a weather delay or at any time at championships.usyouthsoccer.org/midwest - also posted on our Facebook page (US Youth Soccer Region II) and Twitter (@USYS_RII). Medical Medical services will be provided by Methodist Sports Medicine. Athletic trainers will be on site along with EMTs. Free taping services will be provided; however, medical issues and injuries have priority. Practice Fields Practice fields will be available at Grand Park. All practice fields are synthetic. One-hour slots are available on Thursday, June 21, and Monday, June 25. Park General Rules

• NO ANIMALS (except service animals with paper work and bib) beyond parking lots.

• NO outside food and beverages.

• NO gum or seeds allowed on fields.

• NO wading, swimming, fishing, or ice skating.

• NO tobacco products, illegal drugs, or chemicals of any kind.

• NO spectator coolers or containers of any size.

• NO fireworks of any kind.

• NO missile / projectile like objects.

• NO glass bottles or aluminum cans.

• NO air horns, Vuvuzelas or whistles of any kind.

• NO skateboards, roller blades, roller skates, roller shoes, etc.

• NO un-authorized field or batting cage usage – all usage must be approved in advance.

• NO un-approved pamphlets, handouts, displays, advertisements, etc.

• NO un-authorized merchandise, food, beverage, or photography sales of any kind.

• NO un-authorized vehicles, scooters, carts or the like beyond parking lots.

• NO un-authorized Personal Training.

• All RV/Camper’s should park in Parking Lot C; Overnight parking is not permitted.

Medical

• Methodist Sports Medicine is the official medical provider of Grand Park. As part of the

partnership, all sporting events at Grand Park are staffed by medical personnel from Methodist

Sports Medicine, including athletic trainers and orthopedic physicians. Methodist Sports

Medicine is best known as the official team physicians for the Indianapolis Colts, as well as more

than a dozen Indiana high schools and universities including Indiana University, Purdue

University, Indiana State University and the University of Indianapolis.

• The new, permanent Methodist Sports Medicine clinic at Grand Park is located on the second

floor of the massive Grand Park Events Center. The 2,500-square-foot facility is staffed by

athletic trainers, physical therapists and a chiropractor. The clinic has X-ray capability, plus sells

durable medical equipment (DME) such as crutches, joint braces, cryotherapy and rehab items.

An orthopedic surgeon is also on-call and available for all major tournaments at Grand Park.

Severe Weather Policy

• Alarm will sound when lightning is within 10 miles (This is a solid tone siren, beacon lights will

turn on).

• If the park experiences technical difficulties with the notification system, air horns will be used

in a backup situation.

• There will be a 15-minute delay from the last lightning strike before the all clear sounds (This is a

pulsing high/low tone, beacon lights will turn off).

• Grand Park management team, which includes anchor organizations, will communicate directly

with Event Directors in the event of severe weather that requires evacuation.

Evacuation Plan

• When severe weather alarm sounds, the red beacon lights will become active and all visitors and

athletes must evacuate immediately to cars and stay in cars until the all clear signal sounds.

• Any guest unable to get to their vehicle quickly due to a handicap may seek safety at the nearest

shelter building.

• In case of a tornado warning or other life-threatening emergency, all visitors and athletes will be

instructed to evacuate the park and seek shelter away from Grand Park.

• Concessions will be closed throughout the delay.

CONGRATULATIONS TO YOUR TEAM ON ADVANCING TO THE 2018 US YOUTH SOCCER MIDWEST REGIONAL CHAMPIONSHIPS!

SiteSearch will be your housing coordinator this year and will facilitate all of your team's reservations. As soon as we receive your team's contact information from your State Representative, we will be sending you a separate letter with your hotel block information and booking link(s). Please note that your team may only have up to 48 hours to make their reservations so please make sure your team parents are ready to book as soon you receive the information. Please see below for general housing policies: IMPORTANT NOTES: DO NOT CONTACT THE HOTEL TO MAKE/CHANGE/CANCEL YOUR RESERVATION. If you need assistance, please email us at [email protected]. The hotels will not receive your reservation until close to the arrival date. It is also very important that you check to make sure you received your email confirmation as soon as you submit your reservation. If you do not receive a confirmation from SiteSearch, please email us at [email protected] to make sure the reservation went through. CANCELLATION POLICY & 3 NIGHT MINIMUM: All guests are expected to arrive on Thursday, June 21, for a 3- night required stay. Individuals may cancel their reservations up to 15 days prior to arrival which is June 6th, 2018, at 4pm EST or will be charged a cancellation fee of the first night's room and tax. If your team qualifies after the cancellation deadline, your reservation is non-cancellable once it is made. MIDWEST REGIONAL CHAMPIONSHIPS POLICY: Each team is required to reserve a minimum of 8 rooms for 3 nights at their assigned hotel to remain within the housing guidelines. The penalty for not complying with this rule is a substantial fine plus potentially losing eligibility to participate in the competition. Questions regarding this policy can go to [email protected]. POLICY WHEN ADVANCING PAST THE PRELIMINARY ROUND: If/When you advance past the 3-day preliminary round, most teams will want to stay at their same hotel. However, if you choose to move hotels or if you are at a hotel that only had rooms for 3 nights, SiteSearch must coordinate this move for you. You cannot move hotels on your own. On Sunday after you advance (and if you are needing to move hotels), please email us at [email protected] or call us at 502-257-7507 and we will have options for you. Also, if it looks like you may advance after your second game, please go ahead and contact us on Saturday so we can be ready to relocate you on Sunday. HOTEL POLICY FOR SUNDAY IF YOU ARE UNSURE ABOUT ADVANCING: East hotel will have its own policy regarding Sunday checkout times. We highly recommend each team manager contacts their hotel's sales manager before arrival to discuss Sunday's check out time policy. It is especially important that you discuss in advance the check-out procedures if you may be advancing and need to check back in to the hotel. On Sunday morning, please touch base with the hotel manager on duty to let them know if you are leaving or coming back on Sunday night. 2018 US Youth Soccer Midwest Regional Championships Westfield, Indiana June 22-27 Team Participants, Coaches and Families: Hello there! My name is Conrado Portnoy and I work for Sports Video USA, a company dedicated to filming soccer tournaments in different parts of the country. What we do is film entire soccer games in a professional manner using HD cameras mounted on 30-foot tall tripods. Filming from this height allows us to capture the best and most advantageous view of the field (the same way soccer games are seen on TV). Sports Video USA is proud to be again the official video service provider for the 2018 US Youth Soccer Midwest Regional Championships in Westfield, Indiana. For the tournament, we have created a special "team package" to offer at the event. This team package includes us filming your entire games and then uploading online in 1080p high definition, for which purchasing teams receive a private digital link. Games are uploaded online, and links to stream the games are sent out via e-mail approximately 2/3 weeks after the end of the tournament. Past customers will recall that we previously offered physical DVD copies of games, but this year we have opted to provide convenient digital links instead, as the video quality is much better, and distribution among team members is a lot easier, as well. Filmed games serve as wonderful keepsakes and are used by many teams for tactical analysis. This special team package is offered for $540.00 (First three games), two game pack $380.00. One game pack $199.00. Any additional game will be $180.00. Please note: if a team or player needs video files of games for the purpose of editing or creating a highlight reel, we can accommodate and provide an SD card with the game files (Add $30), which will be mailed out within the same time frame as the links. The files can then be shared among the team members wishing to utilize them.
