Beneficiary Eligibility Criteria
Beneficiary is living

You should also know:
CMS will validate beneficiary data using the Chronic
Conditions Warehouse claims data
A beneficiary may be aligned to only one practice at a time
If a beneficiary is enrolled by more than one practice, an MH
Model Program Officer will adjudicate alignment
Provisionally enrolling new beneficiaries in the Model consists of three
easy steps:
Assess the beneficiary against the MH
Model Beneficiary Eligibility Criteria
Upload demographic, clinical, and attestation
data for the beneficiary into the MH Registry
Submit beneficiary enrollment data in the
MHM Data Registry at least once every sixmonths

The POST Portal will be your source for MH Model and compliance
information. After Go-Live your practice will use POST to:
Update practice contact information and
NPI (one-time only)
View payment-related information, such
as total payments disbursed
View beneficiary information, such as
total beneficiaries enrolled in Model Receive Model documents and
compliance notifications

The MHM Data Registry will serve as the secure data hub for your
practice throughout the MH Model. Your practice will use the MHM Data
Registry to:
Provisionally enroll beneficiaries in
the MH Model by submitting
beneficiary data
Update practice information
required for payment
Track your submissions
Receive important reminders and
notifications
MH Model Payments

CMS values your time and participation in the MH Model. You will receive a
reporting payment for each eligible beneficiary.

Each MH Model participant will be assigned to an MH Project Officer
who will be responsible for four specific tasks.

Answers to Questions You’ve Asked -Randomization
Question Answer
I have not received any information on
randomization. Did I miss something?
• Randomization packets were sent out on June 24
from the MHModel mailbox.
• Please check your organization’s POCs, and then
contact the HelpDesk if they did not receive a
randomization email.
Is it possible to switch groups - from
Intervention to Control, or from Control to
Intervention?
• No. We have carefully randomized practices
between the two groups.
• Any changes would threaten the integrity of the
model's design and subsequent evaluation.
• Your onboarding packet explains further details
about randomization and the Model design.

Who should attend the orientation webinar
and future training events?
• We encourage broad attendance. All MH Model
events are open to any member of your
organization that is involved in the MH Model.
• When we send out invitations, we will specify if
the event or resource is targeted to a particular
audience (e.g. clinical or data managers).
How can our organization update our
POCs? What should my organization do if
our listed POC has not received any
emails from the MH Model Team?
• All point of contact (POC) updates must be made
through the CMMI SalesForce HelpDesk.
• To ensure that we have your correct email

When will I begin collecting beneficiary
data?
• The Model is currently scheduled to go live in late
2016/early 2017.
• You will receive additional training before and after
the Go-Live date.
When and how do I upload beneficiary data
for this Model?
• This fall you can participate in the web-based
training “Using the MHM Data Registry in Practice”.
• There will also be a live Q&A sessions right after
go-live.
I have questions about enrolling
beneficiaries into this Model. How can
I learn more about this?
• Detailed information will be shared in a video titled
“Understanding Beneficiary Enrollment and
Alignment”. We will be sending it out via email this
fall.

resources from this site?
• Absolutely! This site is for the broader Million
Hearts® Initiative.
• You are free to explore it and disseminate any
information that you find useful to members of your
organization.

• Live event registration links will be sent out as part of the event
announcement/invitation.

• We are currently uploading your POC information into POST
Portal.

POST Portal?
• Currently, the MH Model POST Portal contains only your
organization's provider list.
• Later we will add information regarding any corrective action
plans (CAPs) that your organization is subject to or is in the
