The purpose of this memorandum is to set forth a plan for training thesenior students of the Polytechnic University of the Philippines Laboratory HighSchool under the subject, Office Procedures (with Office Practice and StorePractice.)
The student agrees to work and study diligently both when receivingbusiness experiences and when attending school. The student also agrees totake advantage of every opportunity to improve in skill, knowledge andpersonality in order to enhance her employability should she chooses to work tosupport herself in her college education. It is understood that the trainee willobserve the same rules and regulations that apply to other employees.
The employer agrees to assist the University in giving the student a well-
rounded training by assigning the student to several phases of work during thetraining period. Training sponsors, usually supervisors or department heads of senior clerks, may be appointed to direct the training in each of the workassigned. At the end of the training period, the employer will complete the JobProficiency Rating Sheet which gives an evaluation of the job performance andpersonality of the student-trainee.
The coordinator will assist the employer in carrying out the continuoustraining of the student. She will conduct the classroom instructions and correlatethis instruction with on-the-job experiences received by the student. Thecoordinator shall confer with the employer and visit the trainee in the workplaceto discover and to correct the weaknesses of the student-trainee.
The parent of guardian of the student has been informed about theprogress and permission has been secured of the student to receive theclassroom instructions and the business experiences in the actual office.
The training period, covering the second semester of the school year 2011-2012 will start on the date when the recommendation letter has beensigned by the coordinator and approved by the employer or representatives. Thestudent will report to the office from_____to_____a.m/p.m. The trainingagreement may be terminated for just cause by any person named in thismemorandum, with the understanding that the notice be given to all concernedparties.
