Editorial

PERFORMANCEGO FAST! Anyone with a Mooney understands this sentiment. Most pilots who own or want to own a Mooney understand the Texas-made aircraft’s reputation for speed. The M20V Acclaim Ultra is the fastest production piston-engine airplane in the sky; and the M20U Ovation Ultra can easily top 195 KTAS without a turbocharger. But before I sound too much like a commercial for a new Mooney, let me continue. Going fast is nice and all, but there are several factors in the environment that effect aircraft performance and speed beside raw horsepower and slick aerodynamics.

Altitude, pressure, temperature and humidity influence air density which in turn affects aircraft and engine performance. When air density decreases, engine and aerodynamic performance are reduced. This is because with a lower air density, air molecules are farther apart so there are less air molecules per cubic meter of air. Since air is necessary for the generation of lift, fewer air molecules means a reduction in lifting capability of an airfoil (wing) or thrust available (propeller). Less air density also means less power available from the engine without the addition of turbocharging.

The key to a Mooney’s performance (especially a normally-aspirated model) is air density. Unfortunately, all our reference materials don’t use the actual density of the air. Instead, our calculations are based on density altitude – “air density expressed as a height above mean sea level.” Another way of thinking is density altitude is pressure altitude adjusted for non-standard temperature. An increase in air temperature, a decrease in atmospheric pressure or an increase in humidity will cause an increase in density altitude and a decrease in air density. When conditions are hot and humid, the density altitude can be significantly higher than the actual altitude. This will have a tremendous effect on aircraft performance. Finally, the relation between indicated and true airspeed are also subject to air density changes.

When there is an increase in density altitude – whether due to high altitude, high temperature or both – the reduced air density will negatively affect aerodynamic performance and horsepower. This will result in an increased takeoff distance, power available in a normally aspirated engine and rate of climb. In addition, there will be an impact to landing distance due to the higher TAS resulting in a longer landing roll.

Because high density altitude has particular implications on takeoff/climb performance and landing distance, it is important to determine the reported density altitude and check the appropriate charts in the Pilot’s Operating Handbook or Pilot Manual. While later versions of the M20, beginning with the M20J in 1977 have very precise takeoff and landing distance graphs that take into consideration pressure altitude and temperature (density altitude) early versions only have tabulated charts that require some amount of interpolation. If the airplane manual is not available or to help calculate the approximate temperature and altitude adjustments for aircraft performance, the Koch Chart may be useful. Using the Koch Chart, connect the temperature and airport altitude by a straight line and then read the increase in takeoff distance and the decrease in rate of climb from standard conditions.

There is a performance chart in late model Mooney pilot manuals that I wonder how many pilots know for what it can be used. It, like most of the other Mooney performance charts, contains graphical data used to find an aircraft’s climb rate performance (based on predictable data) given pressure altitude, temperature, weight and power.

When I first saw this chart, I wondered how often I would use it in flight. When ATC requested an altitude change from 10,000 to 12,000 feet would I pull out the POH, find this chart and determine what the climb rate would be given the parameters listed? Or would I just pitch up to a climb rate of approximately 500 FPM and go for it? I think the latter is my procedure. So what good is this very precise chart?

Well, what if I was departing runway 34 at Leadville, CO (KLXV) in my 2010 Ovation 2 GX with a field elevation of 9933 feet at a temperature of 180 C (650 F), altimeter setting 30.47, wind calm and a density altitude of 11,900 feet (pressure altitude 6000 feet)? What rate of climb could I expect on climb out at 105 KIAS and will that be sufficient to miss the rocks?

First, we will have to look at the departure procedures for KLXV. When we look up the airport in the “Takeoff Minimums (Obstacle) Departure Procedures” list we see for our departure runway 34 we must use the DAVVY ONE (RNAV) procedure.

On this chart, we see that in order to meet the climb gradient we must maintain a minimum climb of 364 feet per NM to 13,500 MSL. That’s great but our Mooney climb chart only shows rate of climb in FPM not FPNM. And this value, based on current conditions is 820 FPM. Will that be enough? What do we do?

In the DOD Terminal Procedures publication, there is a table (Climb/Descent Table) for converting climb gradient in ft./NM to climb rate in ft./min.

Using this table, we see that for a 364’ per NM at 105 KIAS (best rate of climb) which with no wind becomes our ground speed, the gradient requires a rate of climb of 632’ per minute, well within the capability of our Ovation under these conditions.

With the hot weather upon us, it becomes very important when departing “hot and high” airports that we fully understand our Mooney’s takeoff and climb (and landing) performance so that we don’t suddenly meet the NTSB or FAA, or end up the subject of “Incidents & Accidents in a future MAPA LOG.

After writing an article for the Air Line Pilots Association regarding abuse of the common aircraft emergency frequency known as Guard, I began wondering how many GA pilots monitor Guard with their number 2 radio. Military pilots vigilantly listen on UHF 243.0 and VHF 121.5 (if VHF equipped) to aid their fellow pilots in distress. And while most airline pilots also monitor 121.5 for the same reason, for as long as I’ve been receiving or giving civilian flight instruction, I don’t recall monitoring Guard in your backup radio as ever being addressed.

Guard was created so distress calls would gain immediate attention by bypassing all other Air Traffic Control frequencies. While in the military, I heard several pilots announce they were in trouble or ejecting. As an airline pilot, I’ve heard calls from desperate civilians, most either lost and out of fuel, or disoriented in the weather. Should you ever hear “Mayday, Mayday, Mayday!” listen to the pilot in distress and absorb what he/she is saying. Never make any radio transmissions until they have completed their call(s), and even then, realize their hands are full tending to the emergency and don’t need any distractions. If no one else has answered the distress call, acknowledge it and relay the information to Air Traffic Control. ATC will then assign a remote frequency so they can communicate directly with you and/or the emergency aircraft without interruption. Becoming an on-scene commander is a crucial responsibility, and it’s easy to forget about flying your own airplane. Set a maximum time that you can remain on scene before you must return to base for a safe landing, and hold to it. (The military refers to this as BINGO fuel.)

While most civilian pilot training includes a dialogue about transmitting on 121.5 in an emergency and setting your transponder to 7700, that’s usually where

the Guard talk ends. Nowadays, because of the possibility of being intercepted after straying into restricted airspace, it is critical that every pilot understands the importance of monitoring Guard. Should you suddenly find yourself penetrating a TFR, the controlling agency will attempt contact on 121.5. Should you fail to respond and are then intercepted, the intercepting aircraft will also attempt contact on 121.5. Unless you fly a Mooney Mite, you will likely have two VHF radios in your aircraft. If your number 2 radio isn’t tuned to 121.5 (if it’s not required for other ATC communication), you may be doing yourself and your fellow pilots a disservice.

When flying cross-country, it’s a good idea to pack a handheld radio as a back-up because it’s unlikely you’ll have cell phone service in remote areas. Should you be forced to put your airplane down, you can use this handheld radio to broadcast your position in the blind on 121.5. Always be clear and concise with emergency transmissions. If people on board require medical help, your Guard transmission(s) could make the difference between life or death.

Airplanes are generally quite safe and reliable, but we must always be prepared for off-field landings or ditching’s. Hopefully, in an emergency you will have time to make a radio call or two before having to set your airplane down. Preceding any emergency call with “Mayday, Mayday, Mayday!” should instantly give you command of the air waves. If you are already in ATC communication, remain on this frequency and calmly state the nature of your emergency, souls on board and fuel remaining. Never let a radio call distract you from maintaining aircraft control.

Please remember that unnecessary chatter on Guard is the equivalent to calling 911 for a pizza complaint. If you require non-emergency communication for flights with multiple aircraft, use Air-to-Air Common on 123.45, but keep your words brief because this frequency is shared by all, and anyone monitoring can hear whatever is being said.

If you apply the same professionalism in flying as you do in your workplace, then life is pretty easy. Have fun, fly safe and monitor Guard.

The following are questions recently asked by our customers regarding the pilot section of their policy, and our answers.

Q. “The Open Pilot Warranty on my aircraft insurance policy requires a Private Pilot with an Instrument Rating who has at least 750 hours total time; 250 hours in retractable gear aircraft; and 25 in the make and model. I needed to have my aircraft delivered to the maintenance shop for an annual and had trouble finding a pilot who checks all the boxes – especially the make and model time. If the pilot is close on the qualifications, will the company be ok with that?”

A. Insurance policies are a contract between you and the company. In order to be in compliance, you always need to get any pilot who does not fully meet the policy requirements approved by the underwriter before allowing them to fly the aircraft. We have had claims denied for this very issue.

An exception to this rule is most policies suspend the pilot requirements while the aircraft is in the care custody and control

of a maintenance facility for the purpose of maintenance or test flights. So, if the repair facility had sent a pilot to pick up your aircraft, you probably would not have to worry. But make sure your policy includes that wording just in case.

Q. “I currently have myself and three other pilots named on my policy. When I called in to add another pilot, I was told I would have to delete a pilot or lose the open pilot warranty off my policy. Why is that?”

A. Most companies assume that the more pilots you have named on the policy, the more the aircraft is flown. Since more hours flown equals more chance of an accident, most companies will limit the number of named pilots on the policy. Some companies limit it to four named pilots while others will accept five or six. When you exceed the magic number, they will delete the open pilot warranty (OPW). Remember, pilots who meet the OPW and only occasionally fly the aircraft do not need to be named. Often, after evaluating this, the number

of named pilots can be lowered under the maximum allowed and the OPW can be retained. If for some reason, the number of named pilots is still more than the number permitted by the carrier, the policy might need to be re-rated as a flying club.

Q. “I am scheduling an IPC check with a CFII and they are asking to be added to the policy as an additional insured with a Waiver of Subrogation. What are the risks of doing this?”

A. Most Insurance companies are willing to add a training provider as an additional insured since they are providing a service that is considered to be enhancing the safety of the risk. By being added as an additional insured, they share in the policy limit and the insurance company promises to protect them if they are sued for an occurrence arising out of their training you in the aircraft. So, while there is a benefit to adding them to your policy, there is the unavoidable issue that the limit of liability could possibly be diluted. That is one of the reasons we like to recommend higher limits of liability. Truth is,

if you really want to use this CFII, you may not have a choice.

The Waiver of Subrogation is another story. If the insurance company agrees to this, they are promising not to go after the CFII if they pay a physical damage claim that is caused by the CFII’s negligence. Most companies will

include this for a training event at no charge. Some might require a small additional premium. The main downside to you the owner is that if the CFII causes a major loss and your company pays the claim, the responsible party is held harmless and the company cannot recover the damages which follow you on your loss history for a few years.

MEDICAL MATTERS

Rick’s vision had been declining for several years. At first, an increase in the strength of his glasses was helpful. Rick began to need brighter and brighter illumination to read. Rick began to have difficulty driving at night. He noticed that there were halos around the oncoming headlights. Rick was experiencing classic symptoms of cataracts. (TABLE 1)

CATARACTS

A cataract is a clouding of the clear lens of the eye. (FIGURE 1) The lens of the eye focuses the images we see upon the retina (film). The lenses contain proteins which coagulate causing the images our brain’s see to become cloudy and discolored. These changes occur with normal aging. They are also seen in patients who have diabetes, or who have been on steroids for a prolonged period of time. There are many risk factors associated with cataract formation. (TABLE 2)

DIAGNOSIS

Rick was seen by an ophthal-mologist (eye physician). Using a slit-lamp lens, Rick was diagnosed as having central cataracts. Surgery was recommended.

SURGERY

Cataract surgery is performed on an outpatient basis. Under local anesthesia, utilizing magnification, an incision is made exposing the cavity in which the lens lies. An ultrasonic probe is then used to pulverize the lens into micro particles. The particles are then removed by suction. The new, synthetic lens, is inserted. The new lens is compressed, and it expands like a rose blossom. The incision is so small that sutures are unnecessary. The eye is patched for 24 hours. Complete healing takes place in about 3 weeks.

SUMMARYCataract surgery is the most common form of surgery performed in this country. The FAA does not require that the airman report the

procedure between FAA physical examinations. In some cases, lenses with refractory correction may be placed.

Question:I am not mechanically inclined. What can I do as a pilot to keep my Mooney safe for flight?

Answer:In one word, “LOOK!” You don’t have to be mechanically inclined to be able to spot trouble before it becomes an emergency. Let’s talk about some of the things you can look for on your Mooney that will keep you out of trouble.

Whenever you wash, polish or clean your Mooney, whether inside or out, always look for anything out of the ordinary. One should be familiar with what is normal – normal being everything in its proper place, nothing broken, no leaks or cracks. In reality, you are now doing what a maintenance technician does at an annual inspection – checking with your eyes for things that are obviously incorrect or broken. You have an advantage because you are able to inspect your airplane every time you are around it, not just once a year during an annual inspection. Of course, your inspections do not substitute for the annual inspection by a qualified maintenance

technician. However, it does supplement it and can possibly save you a great deal of money.

A very good place to inspect regularly is the engine and engine components when the cowling is removed. A guide on some important areas to look at under the cowling and on the airframe in general can be found in FAA Advisory Circular AC 43-16A. Items that we should be looking for per AC43-16A are: bulges on the muffler, aileron skin cracks, engine oil on the cabin floor, landing gear actuator attachment rod cracks, cracks in elevator balance weight, aircraft battery smoking (no water), throttle cable chafing the battery cable, engine mount cracked, engine mixture control cable broken, severe corrosion on the fuselage skin, fire resulting from fuel hose splitting, exhaust header cracks, both brake master cylinder’s flexible hoses were stiff, brittle and seeping fluid, left forward engine lord mount was severely corroded, oil cooler supports cracked, excessive metal in the engine oil screen and filter.

These examples were listed in only four issues of AC43-16A. There were many more listed, however, I only mentioned the above items because they could pertain to our Mooneys. The common denominator with all these discrepancies is that they can be found and fixed simply by looking!

It does not take an experienced technician to find these items. On most of the listed items you can say that chaffing is a primary problem, and chaffing can be eliminated by the pilot.

The bottom line is this. As pilots we can go a long way to keeping our airplanes safe if we’ll just take the time to do thorough preflight inspections. The best tools we have to prevent mechanical failures are our two eyes. Keep them focused for trouble and you just might save yourself an expensive trip to the shop.

Answer:Magnetos require a thorough inspection at each 100-hour inspection. They do not require removal from the engine except in case something wrong is discovered during the inspection. Slick magnetos require removal and disassembly for inspection every 500 hours in addition to the 100-hour inspections.

Bendix magnetos with the impulse coupling require removal for the 500-hour inspection on the coupling in accordance with an Airworthiness Directive. I highly recommend that Bendix magnetos be removed at 500 hours and thoroughly inspected on the bench. Bendix magnetos with “Shower of Sparks” do not require removal at 500-hour inspection. However, keep in mind the magneto is the heart of your engine. On many Mooney’s the magneto is not readily accessible so it is ignored and a very brief inspection is made.

This statement has proved to be true many times after an accident inspection where the magneto is involved indicating “lack of inspection.”

Mooney owners with the single drive dual magneto should obtain a copy of the Lycoming Service Instruction 1508 dated April 17, 2002. It recommends a check of the torque on the attachment nuts within the next 50 hours. The dual magneto is a good unit; however again, poor and improper maintenance procedures have given it a bad name.

REMEMBER, anytime you re-move a magneto a new gasket-properly installed- and new lock washers must be used when reinstalling the magneto. The last thing after timing the magneto is that it must be torqued with the properly calibrated torque wrench. Federal regulations require that

the magneto be run on a bench magneto tester any time parts are replaced.

Coils and capacitors should be replaced approximately every ten years. They deteriorate faster and may have to be replaced more often in hot, dry climates.

Lastly, and probably the most important of all details regarding a magneto is that any time you disconnect and reconnect the “P” lead, check out its operation the first engine run after magneto installation. It can be checked by shutting down the engine with the key at idle. Anytime you touch your propeller, treat it as if it is HOT. The ”P” lead can break at any time. There have been several serious accidents with propeller resulting from broken “P” leads. Be maintenance safe-inspect your magnetos and “P” leads regularly.

If there is one thing I have learned with a lifetime aloft flying everything from ultra-lights to sophisticated airlines, pilots want to go faster, higher and longer. That being said, all those desires come with a price tag.

My wife and I love our 1989 Mooney 201. In our “personal airliner” we have flown coast to coast multiple times. It is used primarily to get me to my performances, but is used for visiting family and going on vacations as well. We would love a Mooney BRAVO, but can’t wrap our heads around paying $100/hour for fuel and higher maintenance cost. The question was raised “what can we financially afford to get more performance out of our 201?”

One of the great things about being a member of MAPA is the incredible resources available from other Mooney owners that are willing to share “real” performance numbers. Two items were immediately recommended regarding adding performance to my 201: the K&N air filter and the Power Flow tuned exhaust system. For about $200 the K&N air filter will increase takeoff manifold pressure by .5” and unlike the standard dense foam filter, it can be cleaned and used over and over. The new filter simply reduces the restriction on the incoming air. Changing to the K&N filter was very easy and the results were as advertised. Granted it is only a .5” increase, but that is more than what I had. One has to fill out a simple STC form, send it to the filter manufacturer and make a logbook entry.

Upgrading to the Power Flow system (PFS) was not as easy or inexpensive. PFS had five Mooney 201 operators contact me to share their experience with the PFS. Without exception, all were happy they had made the change to the PFS.

Some had their PFS for more than eight years. Each confirmed about 150fpm ROC increase, speed increase of 2-4 KIAS and about 0.5gph increase in fuel burn at cruise. They also confirmed that the higher one flies the larger the benefit from the PFS. Because I spend most my time at 10,500-14,500’ this seemed like a good fit.

With the PFS system, exhaust gases flow evenly with each cylinder’s dedicated exhaust tube, much like headers on a hot rod. It is not uncommon for most exhaust systems to leave 20% of the burned fuel/air mixture behind; leaving less room for the fuel/air mixture on the next intake stroke of the engine. In short, the engine is not “breathing” like it should; causing some back pressure. With a tuned exhaust 95% of the fuel air mixture is used, resulting in more power. This is accomplished by using similar lengths for each exhaust header which creates a negative pressure (vacuum) on the exhaust side.

All PFS users commented how “robust” the PFS was over the original exhaust system. With over 5,000 PFS installed and with a Money Back Guarantee it seemed like no brainer to me.

In February 2017, I ordered the PFS through my local dealer (Twin Oaks Aircraft maintenance). Going through the local dealer gives the customer a break on the shipping. PFS will also give you a break on the cost if you are not in a big hurry to get your system. The savings are significant, so I accepted a delivery date of May 2017. To prepare oneself for delivery one can go to You Tube to see what you get, how to install it on an older model Mooney. Going to PFS’s website, I downloaded the installation instructions and studied them. As a lifelong A&P I would be doing my own installation.

The system showed up on schedule. The first thing I noticed was the outstanding quality of the workmanship and the thickness of the headers. Headers are 40% thicker than the original. I didn’t fully appreciate the thickness till I had to drill holes for my EGT probes. When drilling stainless, turn the drill slow and use a good quality lubricant.

Removing the old exhaust system and installing the new PFS was for the most part straight forward. PFS does a good job identifying the parts and the depth the headers are to go into the “collection box.” Where it got dicey was working around and with electric cowl flaps. For one thing, the clearance between the cowl flap motor mount fixture and the collection box is very tight. This would also have been a great time to have a helper as I dashed in and out of the cockpit numerous times as I watched to see if there was adequate clearance between the cowl flap linkage and the PFS. If your Mooney has the manual cowl flaps this will be a non-issue. It took about 8 hours to remove the old system and install the PFS.

PFS offers a “pre-heat” box to increase the amount of hot air going into the cabin. If you have a Mooney 201 this option WILL NOT FIT!!! It might on older Mooney models, but not the 201. At this juncture, we find the heat output as good as the original heat muff.

Over the years as a mechanic and having built three award winning kit planes (Q2, Q200, Glasair Super 2 FT) at least two things were learned. 1. Don’t be in a hurry. 2. have another A&P look over your handy-work before going “throttle up.”

Performance Numbers

Flight Test: To make this upgrade more meaningful, I took accurate performance numbers before replacing the original exhaust system. During flight test, I used the JPI/EDM700 with the fuel flow feature.

Takeoff: The first thing noticed was the fuel flow increase from over 15gph to 17.2gph. The PFS was doing what was advertised. More Fuel=More heat=More power.

Climb: At 110KIAS I averaged 1,200fpm to 5,000’. At 10,000’ at 100KIAS I showed 500fpm. Overall a solid 10% increase in rate of climb. PFS also claim an increase of Service ceiling by at least 2,500’. I have not checked that out yet,

but based on what I have seen so far, I would not be surprised.

Cruise Speed: I witnessed a 2-knot increase at 10,500’ and a 4-knot increase at 12,500’. These results were done in calm air, RAM air closed, full throttle and 2,500rpm. Operators who saw a larger increase in speed, switched to the PFS at the same time they overhauled a worn-out engine.

CHT readings: Across the board I saw about a 10 degree decrease in my CHT readings

Customer Support: In short it was outstanding. I had an issue where the “collector” touched the electric cowl flap motor. PFS paid for the shipping each way, made the proper correction; all without charging me a cent.

Money Back Guarantee: If after 60 days of first flight should the customer not be pleased for any reason, PFS will take the exhaust system back and give a full refund.

Warranty: 12 months is standard, but if you get your propeller balanced shortly after the PFS installation, warranty is increased to two years.

Overall this was a good investment for my 201. As I have grown as an entertainer my area now includes Colorado, New Mexico and the high country of Arizona. Getting off the ground and climbing faster simply adds to safety. Peace of mind for everyone is an extra bonus.

Dizziness affects many of us and is a common reason to not fly. Dizziness used to be defined as vertigo, presyncope, disequilibrium, and lightheadedness. We now define dizziness as peripheral or central. Most people can identify the timing and triggers of their dizziness.

The gross anatomy of the ear consists of the ear lobe, the external ear canal, the eardrum, the middle ear which consists of three bones connected to the eardrum and a part of the inner ear. The Eustachian tube which relieves the middle ear pressures to the back of the throat. Finally, the inner ear which consists of the cochlear apparatus involved with hearing; and the semicircular canal which is involved with the position in space. Nerves are attached to the apparatus and conduct sensory messages to the brain.

An infection in the middle ear from a blocked infected Eustachian tube builds up pressure which can push on the inner ear and cause dizziness. Swimmer’s ear is drainage and infection of the outer ear external ear canal.

Peripheral causes of dizziness can be of many causes. Infections are a common cause like viruses and bacteria. Usually infections can cause dislodgement of canaliths in the semicircular canal in the inner ear. Canaliths are fine sand particles in a solution in a canal.

Meniere disease is a spontaneous unilateral hearing loss which is caused by excess fluid in the semicircular canal (inner ear). This fluid is referred to as endolymph.

Otosclerosis is caused by abnormal bone growth in the middle ear which is referred to as

conductive hearing loss. Conductive hearing loss is tested with a tuning fork. Some may have been examined by a doctor using a tuning fork where he or she hits the fork and touches it to the bone below the ear (mastoid bone). Usually one should be able to hear the sound of the fork by its high frequency conduction through the bone through the middle ear bones and conducted into sound by the inner ear. If the sound is distorted or not equal or not heard, then one could have a conduction or peripheral hearing loss of which one cause could be otosclerosis.

Central hearing loss is usually spontaneous and much more serious. Some examples will now be discussed.

Cerebrovascular disease can affect the central vestibular system (equilibrium). Central causes can affect up to 25% of dizziness experienced. Common causes are vestibular migraine and vertebrobasilar ischemia (CVA), a stroke.

Vestibular migraine is a common cause of headaches in children. A family history of vestibular migraines and women are three times more prevalent to have one. These migraine headaches are accompanied with dizziness. Sometimes there can be vestibular symptoms (dizziness) without the headache. Treatment is anticonvulsive medications, beta blocker, calcium channel blockers (heart medications), tricyclic antidepressants, butterbur, and magnesium. A pilot can’t fly on antidepressants for migraines or anticonvulsant medications.

Vertebrobasilar ischemia would affect blood flow to the back of and base of the brain and starve the vertebrobasilar system resulting in dizziness. 48% of those with vertebrobasilar ischemia will develop dizziness. This can lead to a stroke with permanent damage. It is usually rapid in onset.

More medications to control dizziness are nausea medications (antiemitics) like such as Reglan and Compazine. These are not FAA approved due to sleepiness. Antihistamines such as Benadryl, Phenergan, and antivert, are also not FAA approved. Because of a sedating effect, Benzodiazepines such as valium and Ativan; one cannot fly on these medications for the same reasons.

There are some common causes of dizziness that I see in my family medicine practice. Infections of the ear are probably the most common. Benign paroxysmal positioning vertigo (BPPV) is common. Arrhythmias of the heart are common causes of dizziness like atrial fibrillation.

Patients suffering from recurrent spells of dizziness lasting under several minutes per spell, associated with changes in head position (often provoked by rolling over in bed), usually have benign paroxysmal positioning vertigo. The term “positioning vertigo” is more accurate than “positional vertigo” because it is provoked by changes in head position rather than by the maintenance of a particular posture.

The typical symptoms of positioning vertigo occur in clusters that persist for several days.

There is a brief (10-15 sec) latency period following a head movement before symptoms develop, and the acute dizziness subsides within 10-60 seconds, though the patient may remain imbalanced for several hours.

Labyrinthitis is also common, in my opinion.Patients with labyrinthitis suffer from acute onset of continuous, usually severe dizziness lasting several days to a week, accompanied by hearing loss and tinnitus (dull sounds). During a recovery period that lasts for several weeks, the dizziness gradually improves. Hearing may return to normal or remain permanently impaired in the involved ear. The cause of labyrinthitis is unknown.

Acoustic neuroma is among the most common intracranial (brain) tumors. Most are unilateral, but about 5% are associated with the hereditary syndrome, neurofibromatosis. Other tumors may be meningiomas and other intracranial and spinal tumors (non-cancerous). Diagnosis is made by enhanced MRI. Treatment consists of observation, microsurgical excision, or stereotactic radiotherapy, depending on such factors as patient age, underlying health, and size of the tumor at presentation. Bevacizumab (vascular endothelial growth factor blocker) has recently shown promise for treatment of tumors in patients.

Vascular compromise from multiple sclerosis can cause dizziness. Patients with multiple sclerosis may suffer from episodic dizziness and chronic imbalance. Hearing loss in this disease is most commonly unilateral and of rapid onset. Spontaneous recovery may occur.

Vertebrobasilar insufficiency is a common cause of dizziness in the elderly. It is often triggered by changes in posture or extension of the neck. (see Table 1)

It is clear from the above that there are many other causes of dizziness of which we will save for later. If you experience dizziness for any reason, do not fly. Self-awareness and grounding yourself is appropriate. Take whatever medications work and only fly when off the meds and when they are out of your

system. I would not fly for at least 48 hours after the dizziness has gone away. This is my rule, not the FAA’s. Common sense should prevail in no walking up or down stairs, driving a car, etc.

I hope this was informative. One pilot referred to the inner ear as a gyro but more complex. I think he was right. Don’t take your inner ear to the radio shop. They’re not qualified to fix it. They don’t have the right tools.
