DECLARATION OF PEGGY THIELSEN UNDER 28 U.S.C. § 1746
I, Peggy Thielsen, respectfully state as follows:
1. I am a resident of Franklin County. I am retired. Before I retired, I was a state employee working on regulatory issues.
2. In the 2015 general election, I had to vote by provisional ballot.
3. I showed my photo ID (my driver's license) to the poll-worker. I would not have checked the box stating that I showed my ID because I believe it is the responsibility of the poll-worker to do that—otherwise any voter could just check the box whether or not they actually showed an ID.
4. I did not receive any paper about providing or correcting my ID information after the election. The only thing I received was a sticker saying I voted.
5. I do not believe the Board should have thrown out my ballot.
I declare under penalty of perjury that the foregoing is true and correct.
Executed on March ,23, 2016.

DECLARATION OF COURTNEY WHITE UNDER 28 U.S.C. ÿ 1746
I, Courtney White, respectfully state as follows:
1. I am a resident of Delaware Count),. I am currently a college student in Toledo.
2, In the 2014 general election, I attempted to vote using an absentee ballot. On theabsentee application, I provided an alternate mailing address to which to send nayabsentee ballot--namely, my current college address.
3. I wrote that same alternate mailing address in the "voting residence" field on the
absentee ID envelope, because that is the address where I was staying when I
filled out the application (and where I was physically present when voting theabsentee ballot).
4. I did not interpret "voting residence" on the form to mean the residence where I
was registered.
5. I received no notice of an error in my form.
6. I was unaware that my ballot had not been counted until I was contacted about
the NEOCH, et al. v. Hltsted, et al. lawsuit.
7. I do not think the Board of Elections should have discarded my ballot.
I declare under penalty of perjury that the foregoing is true and correct,
Executed on March Zk, 2016.
