ADULT TRANSITION MODEL The Focus of ATM
Provide services/linkages individuals returning to Cuyahoga County prior to leaving prison Straight Release, Judicial Release, Parole, PRC, TC
Program is addressing three overriding principles: Straight releases are not under any supervision. Therefore, this does
not provide direction or set a clear pathway for individuals to access services they are in need of.
The last few weeks of incarceration are the most critical in having a successful transition. The sooner the integration process is introduced and implemented, the chances of successfully becoming a productive community member increase.
Returning to society, offenders are faced with a wide variety of challenges. Without provided support, these challenges during reintegration can be overwhelming.
