Health Care Workers
A healthcare worker (HCW) is an individual who may have
the potential to acquire or transmit an infectious agent during
the course of his or her work in the healthcare setting while
providing direct care. It may include all persons working in
the healthcare facility comprising nursing staff, physicians,
lab technicians, home-care workers, students and others.
Infections that occur in workers due to

3 Viruses (HBV, HCV, HIV) account for
most cases of occupational infections
• Among ~ 35 millions of HCWs in 14 WHO regions, there
were ~ 3.35 millions HCWs exposed to at least
1 percutaneous injury with a sharp object contaminated
with HBV, HCV, and HIV
• The annual incidence rate of sharp injuries was 0.18-4.68 per HCW-year

Epidemiology of Occupational Infection
due to HBV HCV and HIV
The global burden of hepatitis B (HBV), hepatitis C (HCV), and human immunodeficiency virus (HIV) infection due to percutaneous injuries among health care workers (HCWs) is estimated:
• Overall, 16,000 HCV, 66,000 HBV, and 1,000 HIV infections may
have occurred in the year 2005 worldwide among HCWs due to their occupational exposure to percutaneous injuries. The fraction of infections with HCV, HBV, and HIV in HCWs attributable to occupational exposure to percutaneous injuries fraction reaches 39%, 37%, and 4.4% respectively.
• These infections are highly preventable and should be eliminated

Transmission of Blood-borne Pathogens
in Health Care Settings
• The 2009 global burden of disease study revealed that as
many as
- 5%–10% of new HIV infections
- 30%–32% of new HBV infections
- 40% of new HCV infections
in low and middle-income countries may be attributable to
exposure in health care settings, including unsafe injections,
unsafe blood and occupational exposures .

HIV PEP • Current multidrug regimens are likely much more effective
However, even when properly provided, PEP does not
assure 100% prevention of HIV infection (ZDV 81%)
• PEP should be initiated within 1-2 hours of exposure.
Benefit of PEP greatly after 24-36 hrs, likely not > 72 hrs
• Several options exist for PEP, and ARV side effects are an
important consideration
• Duration 28 days adherence
1.Primary prevention of occupational exposures
2.Prompt management of occupational exposures and,
if indicated, initiation of PEP as soon as possible after
exposure
3.Selection of PEP regimens that have the fewest side
effects and that are best tolerated by prophylaxis
recipients
4.Anticipating and preemptively treating side effects
commonly associated with taking ARVs

ARV Regimens for HIV PEP
• Raltegravir is well tolerated, has few d-d interactions,
achieves higher concentrations in male and female
genital tract than do PIs and efavirenz
• Atazanavir and Darunavir are well tolerated ,cause less lipid abnormalities than Lopinavir/ritonavir, and each is single daily dosing.
• If selected, Atazanavir and Darunavir should be given with ritonavir, which boosts the serum levels of the parent drug.
• Maraviroc is of interest, since new infection involves CCR5 tropic virus exclusively. However, experience with this drug is still limited recommended at the current time

FOLLOW-UP OF HCP EXPOSED TO KNOWN OR SUSPECTED HIV-POSITIVE SOURCES (1)
• Exposed HCP should be advised to use precautions to prevent secondary transmission, especially during the first 6–12 weeks post-exposure
• If PEP is prescribed, HCP should be informed regarding:
• need for monitoring
• possible drug interactions
• the need for adherence to PEP regimens
• Consider re-evaluation of exposed HCP 72 hours post-exposure, especially after additional information about the exposure or source becomes available

• HIV Ab testing for 6 months post-exposure (e.g., at 6 weeks, 12 weeks, and 6 months)
*if the clinician is certain that a 4th generation combination HIV p24 antigen –HIV antibody test is being utilized, then HIV testing could be performed at baseline, 6 weeks and 4 months after exposure
• Extended HIV Ab testing at 12 months is recommended if health care worker contracts HCV from a source patient co-infected with HIV and HCV

“Exposure to a source patient with an undetectable
serum VL does not eliminate the possibility of HIV
transmission or the need for PEP and follow-up testing.
While the risk of transmission from an occupational
exposure to a source patient with an undetectable
serum viral load is thought to be very low,
PEP should still be offered.”

What are situations in which
expert consultation is advised?
• Delayed exposure report (eg. longer than 72 hours)
• Unknown source (eg. needle in sharps disposal)
• Known or suspected pregnancy in exposed person
• Exposed person breast-feeding
• Known or suspected ARV drug resistance in source patient
• Serious medical illness in exposed persons
• Toxicity occurring in exposed person taking PEP regimen

หลกการของ Standard Precautions
1. การปฏบตงานใด ๆ ทเกยวของกบหรอคาดวาจะสมผสกบเลอดหรอสารคดหลงตองระมดระวงตลอดเวลา
2. Standard precautions เปนมาตรการเสรมเพอใหการปฏบตงานดานการแพทยปลอดภยมากขน และตองใชรวมกบมาตรการอน ๆ เชน aseptic techniques , antiseptics usage
3. การปฏบตตาม universal precautions ประกอบดวย 3.1 การปองกนอบตเหตทจะน าไปสการรบเชอหรอแพรเชอ 3.2 การใชเครองปองกนรางกายอยางเหมาะสม 3.3 การสขาภบาลและอนามยสวนบคคลทด

confirmed infection status, in any setting in
which healthcare is delivered”
The application of Standard Precautions during patient care is determined
by the nature of the HCW-patient interaction and the extent of anticipated
blood, body fluid, or pathogen exposure
สรป Occupational Post-Exposure Prophylaxis (oPEP)
• เปนการใหยาตานไวรส “หลง” การสมผสหรอสงสยวาสมผสเชอเอชไอวจากการท างาน (Occupational exposure)-vบคลากรทางการแพทย เชน เขมเจาะเลอดต านว, เลอดกระเดนเขาตา เปนตน และ จากการประเมนความเสยงตอการตดเชอ มขอบงชในการใหยา
• ใชยาตานไวรสอยางนอย 3 ชนดและตองเรมใหเรวทสด(ภายใน 1-2 ชม.หลงสมผสและไมเกน 72 ชม.) และกนเปนเวลานาน 28 วน
• ความรวมมอในการกนยาอยางสม าเสมอและครบก าหนดมความส าคญมาก ซงขนอยกบปจจยส าคญคอผลไมพงประสงคของยา ปฏกรยาระหวางยาและการบรหารยา
• การใหค าปรกษา การเจาะเลอด ผปวยและบคลากรเปนพนฐาน และการตดตามอาการและตดตามตรวจเลอดบคลากรทสมผสตามมาตรฐานทก าหนด เปนสงจ าเปน
• การปองกนการสมผสขณะท างานของบคลากรทางการแพทยเปนหวใจส าคญในการปองกนการตดเชอเอชไอวจากการท างาน
