Grace is the newest member of the LifeBridge Health family. Acquired by LifeBridge Health in 2019, Grace Medical Center, formerly Bon Secours Hospital, serves thousands of patients at its Baltimore Street location in West Baltimore. LifeBridge Health is committed to renovating the 100-plus-year-old facility founded by the Sisters of Bon Secours in 1919. We are proud to reveal Phase One of the renovations with a new state-of-the-art Emergency Department (ED) and surgery suites, plus all-new specialty clinics.
As you can see, the emergency department, operating rooms and specialty clinics are now complete, and the team is now actively
renovating and expanding the dialysis center, vascular and mammography departments. Along with the renovations in Phase One, LifeBridge Health has also expanded programs and services by adding an OB/GYN, pediatrician, vascular and orthopedic surgeons to the team, as well as opthalmology services.
This guide will give you a glimpse at our progress at Grace thus far and a look ahead to plans for the West Baltimore campus. You will also meet the Grace leaders and physicians in this guide. Thank you for your support, and we look forward to serving you and your family.

OUTPATIENT SERVICES
Outpatient Behavioral Health410-362-3558
What is a behavioral health home?A behavioral health home (BHH) is team-based care focused on the needs of the participant and the family. This type of care is centered around the whole person and includes physical, behavioral and social care.
Often times when a person is managing a substance use disorder and/or mental illness, it can be difficult to also manage other health issues. A patient-centered approach helps with health literacy, offers tools and resources for self-managing chronic conditions, and is founded on trust and respect between the participant and the provider in order to develop a partnership.
Outpatient Mental Health Clinic (OMHC)The OMHC believes in the connection of mind, body and spirit, and recognizes and appreciates the uniqueness of each individual in building an alliance toward the healing process. A wide range of therapeutic modalities are tailored to meet the needs of each individual.
Partial Hospitalization Empowerment Program for Adults (PHEP-A)PHEP-A is a structured, intensive day treatment program for adults in need of psychiatric stabilization. PHEP-A is a therapeutic program facilitated by a multidisciplinary team committed to providing quality healthcare and to the empowerment of the patients we serve.
Partial Hospitalization Empowerment Program for Children (PHEP-C)PHEP-C is a therapeutic intensive day treatment program for children ages 5-12. It is a culturally sensitive, highly structured program that teaches children adaptive skills and healthy approaches to solving real problems.
Intensive Outpatient Program (IOP)Our IOP offers intensive, outpatient psychiatric services to patients who require less than 24-hour care, but who need more comprehensive psychiatric evaluation and treatment that can be provided in a traditional outpatient setting. Treatment focuses on relief of acute symptoms, establishment of healthy coping skills and collaboration with family and community support resources.

UPCOMING PLANS FOR GRACE MEDICAL CENTER
As we move into Phase Two of construction and renovations, we are excited to announce the construction of a new 20,000-square-foot outpatient behavioral health facility, along with creating green space for the community. This phase will begin with the demolition of the old hospital starting in 2021.
James Roberge, LifeBridge Health’s vice president of capital improvements and support services says, “We are excited about each phase in this project. The surgery suites and state-of-the-art emergency department are now complete. In subsequent years, construction milestones include demolition of the existing hospital and completion of the new outpatient behavioral health facility by June 2023.”
“Along with the renovations and construction, we are also expanding service lines at Grace,” says Rebecca Altman, vice president and chief integration officer, LifeBridge Health. A dedicated OB/GYN physician and pediatrician are recent additions to the team. Ophthalmology and orthopedic specialties are coming soon.
