When Will Coverage Begin?

• Insurance coverage begins on the first day of the month following your hire date (unless otherwise indicated)

• For example, if you are hired on August 18th, your coverage will begin on September 1st

Who is Eligible for Coverage?

• Generally, full time employees are eligible for health insurance coverage as well as their dependents, who may include:

• Legally married spouses

• Children up to age 26, including natural, adopted or step-children or children for whom the employee is the legal guardian

• There are special circumstances for coverage of disabled dependents after age 26

Proof of all dependent’s eligibility is required by the State of TN. Dependents must be verified by submitting a copy of the required documentation before they will be enrolled.

Annual Enrollment Transfer Period (AETP)

During Annual Enrollment, you may: • Add insurance coverage • Cancel insurance coverage • Change insurance carriers • Switch between plans

Changes are effective January 1 of the following year

Annual Enrollment occurs each year during the fall, this year it will be

October 1 – November 1

Adding Coverage

There are only three times you may add coverage:

1. As a new employee

2. During the Annual Enrollment Transfer Period

3. If you experience a special qualifying event A specific life change, such as marriage, the birth of a baby or something that

results in loss of other coverage Must submit paperwork within 60 days of the event or loss of other coverage

Canceling Coverage

There are only three times you may cancel coverage: 1. During the Annual Enrollment Transfer Period

2. If you become ineligible to continue coverage

3. If you experience a qualifying event

You cannot cancel coverage during the plan year, outside of the Annual Enrollment Transfer Period, unless you have a qualifying event or lose eligibility under the plan.

Notice to TennCare Enrollees

• You must contact your caseworker at the TennCare within 10 days of your date of employment

• Report to TennCare your new job, salary and that you have access to medical insurance with your new employer

• Employees cannot be enrolled in both TennCare and a State Group Health Insurance plan

• Visit any doctor or hospital you want • You will pay less when using an in-network provider • You will pay more for services from an out-of-network provider

• There are two plan options available:

• Partnership PPO • Standard PPO

Partnership PPO

• Members are rewarded with lower costs for selecting the Partnership PPO and taking an active role in their health

• Members (and their spouses if enrolled) must commit to a “Partnership Promise” each year

• Partnership Promise is not required for covered children

• The Partnership Promise requirements may change from one year to the next

• Healthways administers the Partnership Promise

• Members will pay more for monthly premiums, annual deductibles, copays, medical care co-insurance and out-of-pocket maximums

• Not required to fulfill the Partnership Promise - but these members do have access to the ParTNers for Health Wellness Program

Pharmacy Benefits • Administered by CVS Caremark

• The covered drug list is the same for both the Partnership PPO and Standard PPO, although copays differ between the two

• There are three drug levels:

Generic Drug (tier one) is a generic medicine that is FDA-approved and equal to the brand-name product in safety, effectiveness, quality and performance – Least expensive option

Preferred Brand (tier two) is a brand-name drug included on the drug list – More expensive option

Non-preferred Brand (tier three) is a brand-name drug not on the drug list – Most expensive option

Basic Term Life, and Accidental Death and Dismemberment

• The State provides, at no cost to every full-time employee • $20,000 of basic term life insurance

• $40,000 of basic accidental death and dismemberment (AD&D)

• If you are enrolled in health insurance, your coverage automatically increases, based on your age and salary • Up to $50,000 of term life insurance

• Up to $100,000 of AD&D insurance

• Members are responsible for the additional cost of coverage

• Dependents enrolled in health insurance are each covered for $3,000 in basic term life, and an amount of basic AD&D based on your salary and family composition

Retirement

• All regular full time employees of the University of Memphis are required to participate in a State of TN retirement program

• Regular part time employees are eligible, but not required to participate

• Employees who are non-US citizens on F-1 or J-1 visas are not eligible for retirement membership (As soon as you gain H-1 visa status or become a permanent resident, you must contact University Benefits Administration to enroll)

• Employees who have current membership (because of previous State of TN employment) in either TCRS or the ORP may be eligible for the legacy TCRS/ORP programs

• Employees paid on an hourly basis will be enrolled in the Hybrid Tennessee Consolidated Retirement System (TCRS)

• Regular academic, executive, and administrative employees shall have the option of becoming a member of either TCRS or the ORP

Annual Leave, Sick Leave and Sick Leave Banks Annual Leave: • Exempt – 15 hours/month • Non-Exempt – 7.5 hours/month (increases based on years of service) • Nine, ten, and eleven-month faculty are not eligible to accrue annual leave

Family Medical Leave Act (FMLA) Provides eligible employees up to 12 work-weeks of job-protected leave during a twelve month period for specified family and/or medical reasons.

• Eligibility: • Worked 1250 hours during the previous 12 months • Worked 1 year at the University of Memphis

• Qualifying Events: • Serious illness of employee, spouse, child, or parent • Birth or adoption of a child • Military Caregiver provision (up to 26 weeks)

• Non-Paid leave: • Must use personal Sick/Annual Leave to be paid while on FMLA

Workers’ Compensation

• Any job-related injury must be reported immediately to your supervisor

• Contact HR as soon as possible

• Complete both 1st Report of Injury forms & submit to the Benefits office:

• Provides insurance continuation for up to 18 months

• COBRA notifications are mailed from the State Benefits Administration office

• Notifications are generated, then mailed, to the employee’s home address approx. 7-10 days after insurance coverage has terminated (not employment)

Your Privacy • Your personal health information is strictly confidential

• Your health privacy rights are protected through the federal law, “HIPAA”

• Benefits Administration can only discuss benefits information with the head of contract (HOC)

• The Authorization for Release of Protected Health Information form must be completed before Benefits Administration can discuss benefits information with your spouse or any other authorized representative
